WHAT WILL YOUR PRACTICE BE ASKED TO DO?

Be willing to be randomly assigned

to one of the three approaches (you

do not currently have to have PCMH

designation)

Recruit 40 patients ages 20 to 75 with a BMI between 30-45 kg/m2

Receive training on the study

procedures and in weight loss

counseling techniques

Deliver weight loss counseling

sessions if randomized to fee

–for-service or PCMH approaches

Collect data on each of the 40

patients 4 times over 2 years (i.e.

blood pressure, weight, height,

waist circumference, blood draws)

It is important to have a professional's

input and guidance in weight

management to ensure it is done in a

healthy manner. My primary health

care provider knows me, has my

health history and my family's history.-Mike K.
