RE-POWER is an innovative and collaborative research trial comparing three models of obesity treatment in rural primary care practices. Participating practices will be randomized to one of the three treatments:

Our goal is to find the best approach for
obesity treatment in rural practices. Thirty-
six practices across a four state region will participate in this cutting-edge research.
The benefits will reach your patients, your
community, and your practice. Project
funds will cover your staff’s time spent in
study-related activities.
A patient-centered medical home (PCMH)
model that incorporates group visits
A disease management (DM) model that
uses remote phone counseling

WHAT WILL YOUR PRACTICE BE ASKED TO DO?
Be willing to be randomly assigned
to one of the three approaches (you
do not currently have to have PCMH
designation)
Recruit 40 patients ages 20 to 75 with a BMI between 30-45 kg/m2
Receive training on the study
procedures and in weight loss
counseling techniques
Deliver weight loss counseling
sessions if randomized to fee
–for-service or PCMH approaches
Collect data on each of the 40
patients 4 times over 2 years (i.e.
blood pressure, weight, height,
waist circumference, blood draws)

management to ensure it is done in a
