ASSESSMENTS ADMINISTERED A variety of proven assessments are available

depending upon the individual’s needs.

Examples include:

Trauma

Risk

Needs

Mental Health

The Assessment Center can offer referrals to community services or programs offered by the court for youth with at-risk behavior or delinquency charges.

IS IT NECESSARY TO GET AN ASSESSMENT? Results of assessments provide specific data about each youth. In addition to observations from providers and others who work with a youth, data can provide a more targeted direction in which to steer a youth toward appropriate supports.

ASSESSMENT MEETING & PROCESS

The meeting may take place at the Assessment Center, a trauma-informed space, or other identified location. The meeting shall include:

Providing informational materials and orientation.

Reviewing documents including confidentiality and release of information.

Interviewing the parent(s)/guardian(s).

Interviewing the youth.

Using validated tools to conduct screenings and assessments.

Confidential results of the screening and assessment are discussed with an inter-disciplinary team of professionals, including a Mental Health Therapist and representatives from various social service agencies. After team discussions:

Recommendations for community services and resources will be made with a focus upon ensuring the lightest touch possible to meet the presenting needs.

The Assessment Services Coordinator will contact the youth and family to review results and team recommendations and help coordinate services.

The Assessment Services Coordinator will follow-up with the youth and family periodically for up to one year to ensure needs are being met.

OUR MISSION The Greene County Youth Assessment Center shall promote public safety by providing risk/needs assessment and referral services that respond to youth and family risk behavior with a focus on diverting youth away from court involvement towards community supports that lower risk.

ASSESSMENT CENTER REFERRALS The Assessment Center accepts referrals made from juvenile court, law enforcement, schools, community agencies, parents/guardians and youth self-referrals.

Referrals are received by the Assessment Service Coordinator who conducts a brief interview as soon as possible with the child and parent screening for risk and needs including criminogenic needs, signs of abuse and neglect, mental health including suicide risk and exposure to human trafficking.

WHO IS ELIGIBLE Youth referred do not have to be involved with the courts. Youth who appear to be struggling in some way emotionally, behaviorally, or socially in school, at home, with peers, or in the community may be referred for assessment.
