Freedom to move forwardAccording to estimates from the National Heart, Lung and Blood Institute (NHLBI), approximately 190,000 Americans are affected by Acute Respiratory Distress Syndrome (ARDS) every year.1 Despite receiving therapy with mechanical ventilation, which is widely considered the standard of care, approximately 40% of patients diagnosed with ARDS will die as a result of the disease.2 Veno‐venous extracorporeal life support (VV ECLS) is an emerging therapy designed to provide a higher level of support by infusing oxygen directly into the blood using an oxygenator, a medical device that acts as an artificial lung. In one large, randomized, controlled clinical trial of 180 adult ARDS patients, the use of advanced VV ECLS therapy was shown to reduce the rate of death and disability by 31% compared to conventional management with mechanical ventilation.3
The PROTEK DuoTM cannula is designed to address two major roadblocks that have prevented VV ECLS therapy from becoming a new standard of care: the difficulty of initiating and maintaining a patient on VV ECLS support, and suboptimal gas exchange rates with devices that are currently available. Insertion of the PROTEK DuoTM cannula can be performed through a minimally
invasive percutaneous procedure, and patient management is simplified by combining with the lightweight and compact TandemHeart® pump, which can be placed directly on the patient. The TandemHeart® platform is designed to simplify patient ambulation and enable freedom of movement that is not possible with mechanical ventilation or any alternative ECLS devices.
Omni-directional inflow ports are designed to mitigate recirculation and provide 360 degree access for ease in positioning.
Patient management is simplified with the lightweight, compact TandemHeart® pump, which can be placed directly on the patient to allow for ease in ambulation.
The tapered introducer tip provides gentle entrance into the vessel for smooth cannula placement.

The design of the Korean Heritage Room is based on the historic Korean
academic institution, SUNGKYUNKWAN. The room will be equipped with
a state-of-the-art audiovisual system including an interactive touch screen
LCD monitor. Thus, through this endeavor, we will have an opportunity to
showcase our splendid 5000-year history and cultural image, as well as the
intellectual and economic prosperity of South Korea.
The Korean Heritage Room Committee sincerely thanks you in advance
for your kind and generous support of this exciting community project.
In 2007, the University of Pittsburgh approved the installation of a
new Korean Heritage Room (KHR). Since then, a KHR Committee has
been chosen and has been diligently planning and working on this
special project.
With an estimated cost of $650,000 to complete, there is much funding
work to be done. We have secured approximately $340,000 of generous
contributions by the Korean Community, the Korea Foundation, and the
Poongsan Incorporation of Korea. Additional grants are expected from
Korea, but that leaves a substantial amount of funding needed to complete
the Korean Heritage Room.
The Cathedral of Learning at the University of
Pittsburgh is a historic landmark admired locally
as well as internationally.
One of the Cathedral’s most unique features is
that it serves as home to twenty-nine Nationality
Rooms... each exhibiting its own unique cultural
heritage by way of historic renderings and artworks,
and each serving as a permanent ambassador of
the nations represented.

THERMAL REGENERATION
A quality activated carbon product for precious metals recovery can be thermally regenerated and reused multiple times. To regenerate, spent activated carbon is processed in a horizontal kiln at high temperatures to devolatilize the material and reopen the carbon’s pore structure. When choosing an activated carbon product for gold recovery circuits, it is of paramount importance to consider the hardness of the carbon. A hard carbon particle will resist abrasion during operation and regeneration and maintain its usefulness for a longer life-cycle.
A variety of extraction techniques are employed to liberate gold from crushed raw ore. These processes make the gold available for release into the cyanide solution.
CARBON-IN-COLUMNCIC is most commonly used to recover gold from heap leaching operations, processes in which a sodium cyanide solution percolates through a “heap” of crushed and agglomerated ore, causing the gold to leach into the solution. The gold-bearing solution is then pumped to a series of adsorption columns containing activated carbon. Heap leaching followed by CIC circuits is a preferred method for gold recovery from low grade surface deposits and waste rock because of its low capital and operating costs.
CARBON-IN-PULPIn CIP operations, mined ore is milled and mixed with water and thickeners, creating a slurry of ore and water called pulp. The pulp is then pumped to air-agitated leaching vessels, where the gold is leached out of the ore using a sodium cyanide solution. The pulp is transferred to a series of adsorber tanks containing activated carbon, which mixes with the leached pulp and flows countercurrent to the pulp in a series of tanks. The gold-bearing activated carbon is separated from the pulp by a mesh screen that blocks the carbon from passing through, while the smaller pulp particles are filtered away.
CARBON-IN-LEACHCIL operations are very similar to CIP, but activated carbon is instead added directly to the vessels in which cyanidation is taking place. CIL is an especially beneficial technique when ore contains high levels of carbonaceous material. Naturally present carbon competes with the activated carbon in adsorbing gold in a process called “preg robbing,” which causes gold losses. Therefore, in CIL, the cyanidation process is carried out simultaneously with adsorption on activated carbon, which is in direct contact with pulp in the cyanide solution. The CIL process minimizes the time that cyanidated gold can be in contact with competing carbon native to the ore.
ADSORPTION PROCESSES WITH ACTIVATED CARBON

Typically, gold is not directly recovered. Instead, ore is crushed to release gold, which then reacts with sodium cyanide in a process called cyanidation. The process is characterized by the following chemical reaction:
After the metal complex (NaAu(CN)2) is formed, activated carbon will adsorb the gold cyanide complex from the gold bearing stream (pregnant liquor). To maximize loading on the carbon and overall yield, it is important to select a high quality carbon with the appropriate physical characteristics, which impact the performance of activated carbon for gold recovery applications. These properties include:
• Hardness Number Hardness is the resistance of particles to crack and/or break apart on handling. Poor activation can negatively impact hardness, resulting in high attrition and platelet generation within the gold circuit.
• CTC/Butane Activity CTC/Butane Activity are both measurements of the area available for the gold complex to bond and relate to the adsorption capacity, or k-value.
• Particle Size Distribution or Internal Screens The size of individual activated carbon particles has a significant impact on kinetics, the speed at which the gold complex is adsorbed. Smaller particles tend to adsorb more quickly, while larger particles tend to have slower kinetics. It is important to have a consistent particle size distribution or internal screen distribution to minimize gold loss and maximize gold loading.

Elemix concrete additive, a newly patented1 engineered lightweight synthetic particle that is code-compliant, has been specially formulated for use in concrete.
Elemix additive has no regional material limitations and has contributed to consistent lightweight concrete— batch to batch, region to region and country to country. Elemix additive has successfully been used in commercial projects involving elevated decks, floor toppings and manufactured concrete products including precast and tilt-up panels.

The primary use of structural lightweight concrete is to reduce the dead load of a concrete structure, allowing the designer to reduce the size of columns, footings and other load-bearing elements. Concrete made with Elemix additive results in lightweight or reduced density concrete with many improved physical characteristics. For example, extensive third-party testing shows improved fire resistance for concrete made with Elemix additive and normal weight coarse aggregate. The typical thickness of a 2-hour–rated slab on metal deck was reduced by 1 inch with concrete produced with Elemix additive and normal weight coarse aggregate. A reduction in other fireproofing elements such as spray-applied fire-resistive materials (SFRMs) may also be achieved depending upon design. The improved fire performance resulting from the use of traditional lightweight coarse aggregate is also realized when used in combination with Elemix additive.
Specifiers can use Elemix additive in construction projects designed in accordance with the 2006 or 2009 International Building Code® or International Residential Code.® Testing conducted on hardened concrete bears evidence that mechanical and durability characteristics as defined by applicable building codes are maintained.
Elemix additive is code-compliant as validated by evaluation report ESR-2574.
Elemix concrete additive is approved as a partial replacement for conventional normal weight and lightweight aggregates, and its use can add up to significant overall project cost savings.
Elemix additive lightweight synthetic particles distribute uniformly throughout the concrete, lowering density without the need for air-entraining admixtures.

One of the most important numbers for your health is actually two numbers: your blood pressure. These numbers are important indicators of your heart health because they indicate the force being applied to your artery walls as your heart pumps blood throughout your body.
Blood pressure often sounds like a fraction with one number over another. The top number is the “systolic” measure and shows the pressure when your heart is at work pumping blood. The bottom number is the “diastolic” that reflects the pressure when your heart relaxes between beats.
If you have your blood pressure checked on a regular basis, you may be surprised that it is a bit different each time it’s checked. That’s normal because it constantly changes throughout the day and night to reflect different levels of activity.
If your blood pressure is consistently high, you may have hypertension, which can damage your blood vessels, heart and kidneys. That damage can happen even if you don’t feel any symptoms, which is why it’s so important to have your blood pressure checked. It’s quick, painless and only takes a few minutes. While your health care provider is the best resource to determine what your blood pressure goal should be, this chart can help you to understand the goal of any lifestyle tips your provider will suggest to improve your blood pressure numbers.
Chances are most people don’t hear the word “lipid” very often. Even so, lipids are important to consider when you’re thinking about your health. They are organic compounds that make up fats in the foods we eat and, as a result, in our systems. When it comes to numbers, there are two types of lipids you’ll need to learn about: cholesterol and triglycerides.
Cholesterol is a soft, waxy substance that is in your bloodstream and other cells in your body. Your body uses it to build cells and some hormones. If you’re over the age of 20, you should be having your cholesterol checked at least every five years to discover potential indicators of your risk for heart disease. Because not all cholesterol is created equal, your “lipid profile” – or cholesterol measurement – results in three numbers:
• Total cholesterol
• HDL or “good” cholesterol. This helps protect you against heart disease. The higher this number is, the better.
• LDL or “bad” cholesterol. This is the main culprit for heart disease because it causes blockages in your arteries. The lower it is, the better.
The easy way to remember what your numbers mean is the “H” in HDL should be higher and “L” in LDL should be lower.
Then there are triglycerides. They store unused calories and can provide your body with energy – in the right amounts. When you eat, your body converts any calories it doesn’t need to power your system right away into triglycerides. Over time, if you eat more calories than you burn, you may have high triglycerides, which can lead to blocked vessels.
Lipids – cholesterol and triglycerides – are another great topic for a conversation with your health care provider, who is in the best position to determine how often you should be tested and what your goals should be. And because they are constantly reading new information and studies, they can provide you with lifestyle tips to improve your numbers.

1 Per government rules, annual enrollment is the one opportunity for you to change who is covered through your benefits.
2 You must enroll by the date on your enrollment worksheet or you will receive default coverage (see page 16). To make changes at any other time, you must experience a qualified life event (e.g., getting married or divorced, having or adopting a baby, or your spouse gaining or losing employer-sponsored benefits).
3 If you enroll dependents in your coverage, you will be required to certify their eligibility (see page 8).
You can enroll in Alcoa Benefits on the Your Benefits Resources™ website, which is available 24 hours a day Monday through Saturday, and after 1 p.m. Eastern Standard Time on Sunday.
During your enrollment period, when you log in to the website, you will see a reminder that it is time to enroll and a link. Click on that link, and then follow the steps to learn more about your benefits and complete your enrollment.

Save with Flexible Spending Accounts
You can elect two types of flexible spending accounts (FSAs)—the Health Care Fund and the Dependent (Day) Care Fund. These accounts help you save because your contributions are made with pre-tax dollars. You make pre-tax payroll contributions to one or both funds throughout the year, then when you incur eligible expenses, you are reimbursed from your account.

The Health Care Fund can be used for a variety of out-of-pocket medical, prescription drug, dental, and vision expenses—for you and your eligible family members—that are not covered by any other benefit plan. The Health Care Fund allows you to set aside pre-tax dollars to pay for eligible health care expenses for you and your family. You may contribute $100 to $2,500 per year.
Dependent (Day) Care Fund
The Dependent Day Care Fund can be used to cover the cost of care for your dependent children under age 13 or adult day care while you (and your spouse if you are married) work or attend school full time.
The Dependent Day Care Fund allows you to set aside pre-tax dollars to pay for eligible child day care and adult day care expenses. You may contribute $100 to $5,000 per year (maximum is $2,500 per year if you and your spouse file separate tax returns). If you and your spouse both have access to a dependent care spending account, your combined contributions are limited to $5,000 per year.
IMpORTANT: According to IRS rules, you must spend any money in your flexible spending account in 2014. You have until March 31, 2015, to submit your 2014 claims.

In addition to using a Health Care Fund to pay for deductibles and coinsurance, Alcoa employees tell us that they use their fund to buy these items:

Medical Coverage for Pre-Medicare RetireesYou’ll choose from four medical plans: Basic, Comprehensive, Health Reimbursement Arrangement (HRA), and Health Savings Account (HSA).
Each option:
• Has the same network of health care providers (Highmark Blue Cross Blue Shield, which uses the National Blue Cross Blue Shield PPO network of providers).
• Covers the same services.
• Includes the same prescription drug plan. When you enroll yourself and your eligible dependents in one of the medical options, you automatically are enrolled for prescription drug coverage.
• Has different deductibles, coinsurance amounts (may depend on what kind of provider you see and where you receive services), and annual out-of-pocket maximums.
• Has different price tags as shown in your enrollment materials.
• Effective October 1, 2015, has access to TeladocTM .. Teladoc provides access to board-certified primary care doctors and pediatricians by phone or online video for you and your dependents 24 hours a day, every day of the year. It’s like a house call, whether you are at home, traveling, or need a quick doctor visit for non-urgent care. For the Basic Comprehensive, HRA, and Network Not Available options, you pay $8.00 per visit, no deductible. For the HSA option, you pay $40 per visit until the deductible is met and $8.00 per visit after the deductible is met, per IRS regulations. Prescription Drug PlanPre-Medicare and Medicare-eligible retirees have the same prescription drug coverage.
There’s only one prescription drug plan, which is automatic with any of the medical options.
There are three coverage categories in the drug plan:
Generic Drugs
• No deductible
• Free when you fill through the mail order program
• At retail, you’ll typically never pay more than 10%
Brand 80/20 Drugs
• Brand-name prescription drugs with no or limited generic alternatives to treat your condition
• Brand-name drugs that treat asthma or diabetes
• No deductible
• Plan pays 80% of the cost and you pay 20%
Brand 50/50 Drugs
• Brand-name prescription drugs with appropriate generic alternatives
• $50/$100 (individual/family) deductible, combined for retail and mail
• Plan pays 50% of the cost and you pay 50%
Your Out-of-Pocket Costs
• The deductible is combined for retail pharmacy and mail delivery. It applies only to Brand 50/50 drugs. The deductible is $50 for an individual and $100 for a family.
• If you choose a brand-name drug when a generic equivalent is available, the Plan pays only the cost that would apply if you chose the generic drug. You pay the generic coinsurance plus the difference in cost between the brand-name and the generic drug.
• If your doctor believes that a brand-name medicine is medically necessary, he or she can provide clinical reasons to the CVS Caremark Prior Authorization Department. If the brand-name drug is approved, you’ll pay the brand-name drug deductible and/or coinsurance as applicable.
• If you fill your prescription for a maintenance medication more than three times at a retail pharmacy other than CVS, you’ll pay 10% extra. Avoid the extra costs by using the Maintenance Choice Program.
• The maximum coinsurance you’ll pay per fill is $100 at a retail pharmacy or $200 if you use mail order. This maximum applies to the coinsurance but does not apply to any extra costs if you choose a brand-name drug over a generic or purchase a maintenance drug at a retail pharmacy other than CVS instead of through mail order.

Your Prescription Drug Out-of-Pocket Maximum Under the Basic, Comprehensive, HRA, and Network Not Available Options
The Basic, Comprehensive, HRA, and Network Not Available medical options have separate out-of-pocket maximums for medical and prescription drug expenses.
The prescription drug out-of-pocket maximum limits the amount you pay each year for covered prescription drug expenses. When your share of covered drug expenses (including your deductible and coinsurance amounts) reaches the prescription drug out-of-pocket maximum, the Plan pays 100% of your covered prescription drug expenses for the rest of the calendar year. There are separate individual and family out-of-pocket maximums for prescription drugs as shown in the following table.

The prescription drug out-of-pocket maximum for a family can be met when covered drugs are used by multiple covered family members, even when no single family member meets the per-person maximum. Also, when two or more family members meet their prescription drug out-of-pocket maximum for covered drugs, there is no need for the remaining covered persons in the family to meet the individual out-of-pocket maximum
The prescription drug out-of-pocket maximum does not apply to:
• Expenses covered under the medical, dental, or vision plans
• Any amounts that do not apply to the Maximum Coinsurance Amount
• Drugs purchased at out-of-network pharmacies
• Expenses not covered under the Plan
Prescription Drug Out-of-Pocket Maximum Under the HSA Medical Option
If you are enrolled in the HSA medical option, you must meet the integrated medical and prescription drug out-of-pocket maximum before the Plan starts to pay 100% for covered medical and drug expenses. Any prescription drug expenses that you incur apply to the HSA out-of-pocket maximum.
You have three ways to purchase covered prescription drugs:
• At a participating CVS Caremark retail pharmacy, including most chain and many local pharmacies. Your deductible applies, and you pay the coinsurance for up to a 30-day supply.
• Through the CVS Caremark mail-order service for long-term medications or any prescriptions not needed immediately. No deductible applies, and you pay the coinsurance for up to a 90-day supply.
• Using the Maintenance Choice Program, which allows you to pick up your 90-day supply at a CVS pharmacy and receive the mail-order level of benefits.
