Hazard Analysis
• Analysis used in the workplace to
identify potential hazards with
objective to prevent accident

WHAT IS HAZARDS
• A hazard is any source of potential damage, harm or
adverse health effects on something or someone under
certain conditions at work.
• Basically, a hazard can cause harm or adverse effects (to
individuals as health effects or to organizations as
property or equipment losses).

2) Change Analysis
• An investigating method used in problem-
solving and determining the cause of
accident
• Analysis based on comparing situation that
does not exibit a problem to one that does.

7 Steps of Changes Analysis
Step 1• Define Problem. What Happened?
Step 2• Establish the norm – What should have happened?
Step 3•
Identify, locate and decribe the changes –
what, where, when, to what extend?
Step4• Specify what and what was not effected
Step 5• Identify the distintive features of the changes
Step 6• List the possible causes
Step 7• Select the most likely causes

Consideration
• Single accurance
• Focus on element of factors changes
•
Used of 5W (what, where, who, why andwhen) and 1 H (how).

• AKA Job Safety Analysis
• Analysis of work process in detail by analysisng the
process step-bey step to identify protential hazard.

Risk Vs Cost
Risk Assesment Factor = Consecuences x Exposure x Probability
