Anamnesis: Patient was brought by ambulance, due to loss ofconsciousness. Inadmission, patient was intubated, not sedated, without conscience, without pulls,without BP, without spontaneous breathing, with mechanical breathing assisted byAmbu, pale with central cyanosis.CCG: In monitor VF, Asystolic.

Immediately were taken reanimation steps, such are RCP, defibrillation, and adequatetherapy.THERAPY:1 .Defibrilation: 200 J, 300 J, 360 J, 3x360 J, 3x360 J2.Amp. Adrenaline Img, Img-t-1 in bolus3.Amp. Lidocaini, 100 mg, bolus4.Amp.Amiodarone, 300 mg, bolus, + 150+150 mg5.02, 101/min6.Sol. NaCI 0.9% 500 ml+500ml _

11.30Even thou we used all reanimation efforts, patient was without life signs, no pulls, noBP, no spontaneous breathing.Pupils maximally dilated, with no reaction, general cyanosis. ECG-A systolia.Death was Confirmed at 11.30, 30.06.2006Nose: Redness nose mucosa in both sides.

SUBJECT/OBJECT: Death of National Staff Mr. Visar Fazlija (Index 755321)

INTRODUCTION

• On June 30, 2006 at about 1020 hrs the national staff member Mr. Visar Fazlija withUN ID number LS01105 whilst sitting with colleagues in his office felt in dead faint.

• The National Staff Mr. Visar Fazlija was transported to Prishtina Hospital EmergencyCenter by his colleagues with UN vehicle. --

• On Ju;io 30, 2006 at about 1130 hrs, cite doctors from Prishtina Hospital EmergencyCenter confirmed the death of Mr. Visar Fazlija.

• The death of Mr. Visar Fazlija was naturally caused from heart attack ( AnestCardiorespirator - Exitus Letatis)

• The National staff member Mr. Visar Fazlija was assigned to the Engineering Section inMaterial Management Unit and his office is located in UNMIK Log Base.

METHODOLOGY

• The Investigations carried out by Security Special Investigations Unit is only toestablish the facts of the case.

• UNMIK Security Special Investigations (SIU) conducted interviews of all personsrelevant to this matter, collected and analyzed documents and records, concerning thedeath of National Staff Mr. Visar Fazlija

INVESTIGATIVE DETAILS

The following is a summary of the incident:

• On July 3, 2006, the writer has visited the scene of incident and identified thewitnesses.

• On July 4, 2006 the writer contacted and held interview with Mr. Maroun Hasrouni,acting OIC of Engineering MMU.

• On July 4, 2006 the writer contacted and held interview with Mr. Fadil Llugiqi fromthe office of Engineering MlvIU.

• The Prishtina Hospital Emergency Center was contacted and a copy of medicalreport and death certificate of Mr. Visar Fazlija was obtained by the writer."

SUMMARY OF INVESTIGATIVE FINDINGS:

After going through the above mentioned information, the findings are as follows:

Mr. Visar Fazlija was UNMIK staff member with UN ID number LS01105 and wasassigned to Engineering Section in Material Management Unit as a store man.

On June 3W, 2006 at about 1025 hrs, national staff member Mr. Visar Fazlija was sitting inhis office together with colleagues Mr. Fadil Llugiqi (LS05492) and Mr. Mustaf Fejzullahu(LS01067).

According with statement of Mr. Fadil Llugiqi, while they were sitting in the office Mr,Visar Fazlija suddenly fainted. This was noticed by Mr. Mustaf Fejzullahu, who tried to callhim several times but without getting any answer from him. In mean time they tried toreanimate him and due to unsuccessful attempts they contacted and informed UNMIKClinic in ordejr to send an ambulance. Meanwhile and due to the unconscious stage of Mr.Visar Fazlija, they transported him in a UN vehicle to the Prishtina Hospital where hisdeath was later on pronounced by doctors. Further to this he declared that while workingwith Mr. Visar Fazlija he never saw him taking any tablets or antibiotics and he is notaware that he had problem with his health previously.

• According with statement of acting QIC of MMU, Mr. Maroun Hasrouni, he was out ofcompound for official purposes when he received information at about 1200 hrs that Mr.Visar Fazlija was evacuated to the hospital due to the heart attack After receivinginformation he went to TSS compound where he discussed the matter with ChiefEngineering Officer Mr John De Renzi, who was aware of the matter and accordingly heinformed Personnel Section.

• The doctor's of Prishtina Hospital Emergency Center despite all efforts for reanimation,they confirmed the death of Mr. Visar Fazlija on June 30,2006 at 1130 hrs.

• Based on medical reports and death certificate issued by Prishtina Hospital, ,Mr. VisarFazlija died a natural death caused by Heart attack (Anest Cardiorespirator - ExitusLetatis),

• The Autopsy report was not done by Prishtina Hospital, as the body of Mr. Fazlija wasn'treferred by the Hospital to mortuary. "£

• The office of Chief Security was informed on June.3.0,2006 at about 1521 hrs via email,however it should been noted that in such cases staff members should reported the caseimmediately to Security.

• The office of UNMIK. Clinic was informed and requested for ambulance vehicle bymembers of MMU on June 30,2006 at about 1025 hrs, which had been dispatched to theUNMIK Engineering Log base, however they did not provide any medical care to thevictim.due to the fact that Mr. Fazlija was transported to Prishtina Hospital by hiscolleagues five minutes prior arrival of ambulance.

CONCLUSIONS

Based on the facts and information obtained it seems that Mr. Visar Fazlija died a natural deathcaused by Heart Attack (See Medical report and death certificate attached)

On Friday, 30"1 of June 2006, due to a sensitive case that I had to inspect in GCnnla Building Iwas out of office. When I received a call around 12:00 from one of my staff that Mr.Visar Fazlijawas evacuated to the hospital .due to the heart attack, I immediately came back to TSS and thematter was discussed with CE Mr. John de Renzi who was aware of the matter and Informedpersonel section accordingly.
