Non-routine. Show 920 ft. buffer and 10 acre blocks. All Collier (100%) minerals. See comments below.

Existing or new infrastructure:

• Describe existing and new roads, flowlines, etc: Access to site via existing roads . There is no description of improvements(?) or utilities to these roads.

• Wetlands or uplands: site is located in an active agricultural uplands location. This site is within the Big Cypress Water Shed but committee will not be convened due to uplands & agriculture usage. Department (DEP) agrees!

Bore-hole logs: Copies of borehole geophysical logs, including digital format logs must be furnished to this office when well is completed. Drilling Fluid Program (Mud): Approved by Engineering (J. L. and L. S.). Current proposed schematic: Proposed schematic is adequate. Final schematic must be supplied after drilling is completed. Geologic Cuttings or Core Sampling Program Ch 377.22(2)(h): We require a split of sample/cuttings. Contact DEP inspectors for mailing address (FGS). Directional Drilling Proposal: This application is for a horizontal well with a pilot hole. Questions and/or Comments:

1. Submit a ‘Directional Plan’ for the pilot hole. See Exhibit #17. 2. Pressure Tests: modify intro-section pg. 4 to indicate pressure testing surface

casing to required psi, and the same goes for Exhibit #16, pg. 1. See Rule 62C-27.005(4), F.A.C.

3. Please specify on pg. 3 of Exhibit 11, and in the Intro-Section pg. 3, a depth at which the H2S plan will be activated.

4. The Application (Fm. 3) needs to be amended and re-issued. See letter to Dan A. Hughes from Levi Sciara dated 11/19/2012. And also on Fm 3, name the well 1349H.

5. Add unit size to the survey plat, Exhibit #6. 6. Exhibit #13, ‘SPECIAL INSTRUCTIONS’ includes wrong contact numbers

(phone) for the Department’s Ft. Myers O&G staff. Correct to: Mr. Paul Attwood work cell is 239.229.4293, and Mr. Pierre Bruno’s work cell is 239.229.4291.

7. In Exhibit #11, pg. 29, replace David Files name with Levi Sciara 850.488.8217 Ext 64, and John Leccese 850.393.6309 work cell phone. On pg. 30, remove Dot Black from this list. On pg. 30, add Pierre Bruno’s name, work cell (239.229.4291) and home number (239.260.1738). Correct DEP’s Ft. Myers Field Office phone and FAX number. (Office is 239.344.5611, FAX 850.412.0590.

8. Please describe any road improvements or new utilities brought in to the site.
