06/90-11/95 - National Director of Epidemiological Surveillance: Responsible for thedesign and setting up of the national surveillance system for the country’sprincipal public health problems, driven initially by the renewed threat ofcholera in Latin America. The implementation of the system involved theinstallation of computer hard- and software at local Health Authority levelthroughout the country, together with a national radio communicationsnetwork and the establishment of “Sentinel Points” for the monitoring ofinfectious childhood diseases. An important product of this system wasthe cholera data routinely collected from November, 1991, to the presentday, which enabled research into the definition of cholera cases. Thisresearch forms the basis for my M.Sc. project (London, 1994) which waspresented at several institutions including WHO (Cholera Task Force,Geneva) and CDC (Centres for Disease Control, US Dept of Health,Atlanta Georgia, USA).

The Epidemiological Surveillance Department began in 1990 with 2members of staff. By 1995 it employed 14, including a rapid responsesupport team of 2 epidemiologists to attend unusual disease outbreaks ornational disasters when necessary. For example, the eruption of CerroNegro Volcano in March, 1992, and the repatriation of the 23 victims ofthe Aviateca air crash in El Salvador in August, 1995.

Apart from the important achievement of the implementation of a nationalsurveillance system, three regular bulletins, daily, weekly and quarterlywere published as well as more than 400 newspaper articles werepublished on epidemic problems. “Situation Rooms” , with detailed mapswere established in all local Health Authorities and major Health Centresas well as a map room to monitor the national situation at headquarters.

Management of medical attention in the ex-war zone to 3000 disarmedex-Contras and their families and to war-injured patients. This involvednegotiating entry into Guerrilla-controlled areas and the supervision ofseveral emergency manoeuvres to rescue and / or attend populationswithout access to medical care. JS has substantial experiences in:- International health care policy, especially in cross-border health care (including

microbiological aspects). Under his co-ordination, the Euregion Meuse Rhine becamea model region for the EU in this respect.

- Health care policy in the Netherlands, ranging from primary and chronic care mattersto topclinical care in university hospitals

- Clinical and applied genetics (patient care; laboratory studies; clinical andfundamental research; JS discovered two genetic syndromes leading toimmunodeficiency and cancer at early age)

- Ethical aspects of health care, especially concerning genetics and genetic research- Co-operation with newly accessed countries, especially Poland- Scientific publications
