St Anne’s was founded in 1971 as a shelter forhomeless men in Leeds. From these smallbeginnings St Anne’s has gradually expandedover the years to become the organisation it istoday, with over 1500 staff supporting in excessof 5000 clients across the North of England.

Our very first service was shaped around people’sneeds and wishes and we have maintained thisethos, with clients at the centre, throughout all theservices we provide.

We are proud to support people with learningdisabilities, mental health, substance use andhomelessness issues and to help them achievetheir aspirations during their time with us. We striveto make a difference every day and we have manygreat examples of this in our 2015 Annual Review –we hope you enjoy reading them.

One of the most delightful things about 2015for St Anne’s has been that when we read allthe CQC reports on the different services thatwe provide.

This focus on caring for the client and putting the clientat the centre of everything we do is at the heart of theSt Anne’s philosophy of care and something we willtake forward and reinforce in 2016.

There are numerous examples of this care whichincluded a member of staff who said her biggestachievement this year had been to ensure a 25 yearold man from Huddersfield Mental Health Serviceshad not been to prison in the last 6 months. This wasthe first time in his adult life he had been out of prisonfor so long. We could not help but be impressed bythe story of a young man with learning disabilities,who within years of being with the service was notonly walking but also working in a bar for a couple ofhours some evenings a week. These stories andthose of staff who regularly work extra, or changetheir hours so that clients can go on a visit or holidaymake St Anne’s so special. As one Manager said tome, “to many of us it is not a job but a vocation”.

It was pleasing to note in 2015 we were able to openup some splendid accommodation for our clients.Working in partnership with Doncaster we are proudto be supporting individuals in highly specifiedaccommodation provided by Highstone Homes.

All organisations in the health and social care fieldhave faced major financial and service delivery issues.I am really pleased that St Anne’s was once again notonly able to manage a small surplus, but also developa number of new services.

These services included:• New service in Doncaster• New nursing service in Halifax• Opening Chapeltown Road developmentin January 2015

• Expanding our provision of AlcoholRehabilitation services

• Re-developing a service in Dewsburywhich will re-open in January 2016

However none of this could have been achievedwithout the huge commitment of staff. This was selfevident in our Celebration of Achievement event whichtook place in April, our sign-up to the Stonewall Charterand a volunteer from Croft House being nominated forthe prestigious learning disabilities “Tenfold” award.

The Board would very much like to thank all our stafffor living up to the St Anne’s Philosophy of Care. Wevery much look forward to working with you in 2016to overcome the challenges facing all social/healthcare organisations.

Stuart’s Bowling Success

Hello everyone.

My name is Stuart, I have Cerebral Palsy and a mildlearning difficulty, I’m also profoundly deaf andcommunicate using BSL.

I came into St Anne’s Community Services in October2004 and I haven’t looked back.

I attended Sunderland Hylton College and learnt manylife skills which has given me confidence andindependence to live on my own, I could not progressany further and found myself a job as a chef.

I took up bowling as a hobby and enjoyed it and soonrealised I was good at it, I joined two bowling clubs(Washington T to T Club and Sunderland ChampionsLeague Bowling) and have won a lot of trophies mostrecently winning the league, best player and thehighest season score in July 2015.

I would like to share my success and show that hardwork dedication can make you reach your goals.

Emma’s Art Project

One of the staff team has recently supported a clientput forward some of her art work exhibition in Leeds.Emma received second place in the textiles part.

Emma has been in the carer breaks service in Kirkleesfor 6 years and this great achievement has really builton her low confidence and self esteem.

“All through the trials of my life, art is the one thing thatalways consoles my soul. It started when I was young.I was very ill and had multiple operations, my mumwould always bring copious amounts of art suppliesinto hospital to keep me occupied until I recovered.When I was in my early teens, I lost most of my sight,was left with blurs of colour and could see somemovement. But I was never going to give up art, so Iadapted what I did. I have Borderline personalitydisorder, I have struggled with suicidal thoughts, butart is always there when I need a distraction. I usuallyhave poor self-esteem and think I’m rubbish but my artis the only thing I would say I'm good at” - Emma

Alcohol Service’s New Rehab Facility

On 16 March 2015, the new rehab facility at AlcoholServices in Leeds was officially opened. Theopening was the culmination of 14 months’ work inturning an aspiration into reality.

In 2014 Public Health England welcomedexpressions of interest from substance use servicesin funding capital grant projects on the theme ofrecovery. Backed by Leeds Public Health and AdultSocial Care at Leeds City Council, St Anne’s weresuccessful in their bid of redeveloping AshtonHouse as an extension of the existing rehab facilityat St Mark’s House. For those who have beenaround long enough, the ground floor of AshtonHouse used to be the old detox, where police andambulances would drop off intoxicated people for aperiod of sobering-up or detoxification.

Now completely transformed after a 5 monthbuilding project, Ashton House is now home to 4clients in a facility that provides a little moreindependence from the other part of rehab, andallows for enhanced preparation before leavingrehab. With the new facilities we now have theopportunity to allow our clients to practice more ofthe coping skills that they have learnt in rehab.Project Managed by Robert Pitt, the building workwent relatively smoothly and ahead of schedule.

The official opening of the service allowed us toexpress our thanks to all who have made the projectpossible and to tell people more about the workdone in the detoxification and rehabilitationservices. In addition to the extra rehabilitationfacilities, the service is also able to report that LeedsPublic Health has supported St Anne’s in having anadditional detoxification bed, taking the total numberof detox beds to 5 and rehab beds to 18.

Current clients, volunteers and staff showed thevisitors around the service, which included theribbon-cutting by Cllr Mulherin, Chair of Leeds CityCouncil Health &Wellbeing Strategy Board and DrIan Cameron, Director of Public Health in Leeds.Over 70 people from a variety of different servicesattended the event, finding out more about detoxand rehab, as well as sampling some freshly bakedcakes made by clients and staff.

Thanks go to Robert Pitt and all the Head Officestaff who made the project possible; to PublicHealth and Adult Social Care at Leeds City Councilwho supported the project; to Public HealthEngland for providing the capital funding; to theclients who participated in the day; and lastly, to allthe wonderful staff and volunteers who work sohard and with so much care and dedication to makeit a fantastic project.

Winners of the Linda McEnhill Award

St Anne’s Community Services would like toannounce that Oxfield Court Nursing service inHuddersfield and Nicky Lyall from the Learning andDevelopment department is the Winner of the 2015Linda McEnhill Award.

Also many congratulations to Gerard Wainwrightfrom Heatherstones Nursing Service in Halifax andMarnie Walker from 8 Oxfield Court in Huddersfield,together with their teams, for being awarded HighlyCommended for their outstanding end of lifesupport for an individual.

The judges felt that there were a lot of outstandingand innovative aspects in the work done by StAnne’s around end of life care: to have EoL trainingturned into a requirement for contracted services;to use GSF accreditation; to adapt GSF (POLE); toachieve the level of CCG support and funding thatyou have achieved; to include families and carersin the development of training resources; the DVD.The list goes on.

This is outstanding partnership work with practicaltools, leading to evidently improved EoL care. Wefeel that your work needs to gain nationalrecognition and be published and shared widely.

Evidence of this outstanding work is the excellentcare provided to clients at 8 Oxfield Court andHeatherstones. Both stories were inspirational anddemonstrate what can be achieved with goodsupport, good systems and good people. Wetherefore decided to give both homes a “HighlyCommended” award.

All these awards were announced and presented atthe PCPLD Network annual conference in Londonon 16th November 2015.

The winner were presented with a certificate and theannual trophy and The “Highly Commended”winners will get a certificate.

Stonewall Diversity Champions

St. Anne’s is pleased to announce that we havebecome a Stonewall Diversity Champion. Stonewallis a charity which works for equality and justice forlesbians, gay men, bisexual and transsexual peoplethrough a variety of ways including policydevelopment and lobbying for legal change.Stonewall’s Diversity Champions Programme isBritain’s best practice employers forum for sexualorientation and gender identity equality, diversityand inclusion. Currently at 750 members, thecombined total of employers equates to almost aquarter of the UK workforce.

Stonewall can support St. Anne’s in engaging itsstaff through:

• Reviewing internal policies (such as bullying andharassment; equality and diversity; and parentalleave) to ensure they are LGBT inclusive and aprogramme promoting such policies.

• Reviewing the training we offer staff and how thisis tailored to specific job roles

• Supporting us to increase our staff data regardingsexual orientation and how to utilise such data.

• Advising on internal communication and signpostingtowards examples of best practice and innovation.

Case Studies

Supported Living, Doncaster

Here are some examples of how St Anne’s hasmade a difference to the lives of people beingsupported in Doncaster.

J began her support with St Anne’s in June 2013, afterbeing admitted into hospital following safeguardingconcerns at the previous place where she lived. J hadlittle confidence and staff soon became aware thatshe had had very few life experiences. Staff workedhard with J to increase confidence, this began withworking with J to win an appeal that had been ongoing for over 12 months.

From there, the staff team had scope to help Jdevelop her budgeting skills, enabling her toexperience going to the seaside for the first time inher life. J went to Blackpool for the day and was thatoverwhelmed and enjoyed it so much she was thensupported on her first holiday to Scarborough forthe week. J also enjoyed her first ever birthday partyfor her 50th birthday. J invited a number of newfriends that she had made whilst being supportedby St Anne’s.

J then suffered the set back of a heart attack.However, she overcame this with increaseddetermination becoming fitter than she had everbeen before, losing over a stone in weight.

We then spoke to J about increasing everydayskills. J had very little reading and writing skills, sowith staff’s help J started this journey using magickey books. When J knew the fundamentals, sheenrolled into Doncaster College starting with anintroduction to writing.

Since being at college J has further increased notonly her confidence but also her independence andnow goes unsupported to the course, working onher travel training with the intention of travelling toand from college independently. J has put thesenew skills to use, being able to read signs in thecommunity and understand them, completeshopping lists and also enjoy reading socially,becoming a member of the library.

J is in the process of booking her second holiday. Inaddition, she has completed an English course, andshe has enrolled into a maths course where she willbe supported to increase her number skills, workingon how J budgets and manages her finances.

She has also had a holiday and is looking now to goabroad next year.

J’s health as never been better and her relationshipwith her housemate has gone from strength tostrength – a true success story on how greatdetermination and a strong support team can enablepositive changes in a client’s life.

Domiciliary Care, Leeds

Here are two example of clients being supportedin the community to broaden their life experienceswhich have been provided by support workers inour Leeds Domiciliary Care service.

Once a week I support a client to a horse ridingsession at Middleton Equestrian Centre. My clienthas a learning disability and can presentchallenging behaviours which restricts himaccessing daily activities.

Horse riding is a recent change in his support and Ihave seen what a difference this has made to him.

Every aspect of horse management is included in thisactivity from riding the horse to brushing its coat andcleaning the stable. Each week can be a challenge toget him to the centre but once there the environmenthas such a calming and positive influence on him thatit makes the challenge rewarding.

I have also taken this client to a lady who trains andcares for dogs whilst their owners are away. Myclient has taken care of one particular puppyincluding grooming, walking and being involved inits training. This brings so much joy and happinessto him that any other struggles he is dealing with areforgotten whilst being involved in these activities.

We were approached by Adult Social Care tobecome involved in a young man’s life followinghim leaving school and needing support toincrease his social and leisure activities and alsohelp him get ready for college each morning. Thisclient had a lot of change in his life all at oncewhich resulted in him only being able tocommunicate his feeling of being overwhelmed bypresenting challenging behaviours.

These behaviours were such that his relationshipwith a lot of services was strained and his supportwas starting to break down. At one point we wereunsure if we could continue to support this client andneeded to take some action.

We introduced a consistent staff team who are allPBS trained and also have attended intensiveinteraction training. With help from otherprofessionals such as psychologist, occupationaltherapist, specialist nursing team and care managerour small team of support workers have turned thisproject around. The young man has built goodrelationships with the team and is now confidentand happy in their care.

He is now involved in lots of community activities suchas swimming, sensory sessions and sporting groups.

The support continues to move from strength tostrength and we are now able to provide respite inthe home to allow his parents a short break, safe inthe knowledge that he is happy and content withhis carers.

After being informed of thedeath of a long term client,she commented: ‘She’s atpeace now. The family weresinging you and your teamspraises!! Well done! You havea fab team!’

Supported Living, Bradford

Here are the experiences of two people who live inBradford with support from St Anne’s.

M is a client who moved into one of our servicesthree years ago. He had lived with his parents all hislife. They had done everything for him and he didn’tknow how to cook, or even make toast himself. Hewas in receipt of Jobseekers Allowance, which wasraised as a concern, as he isn’t able to hold down afull time job, so was being sanctioned often and leftwith no money. He was a very nervous gentlemanand struggled with establishing relationships withstrangers. He needed support all day every day.

Since moving in, M has been involved in recruitinghis own staff. He wrote the questions for thecandidates, sat on the panel, asked some of thequestions to the candidates and then made hischoice on who he would like to support him. Hisstaff have worked closely with him on developinghis skills and looking at his interests.

In the time he has lived with us, M can now fullycook for himself, he cleans for himself and is veryable. He has a part time job which he enjoys andhas made himself a member of numerous socialclubs. He has established relationships with ourother clients and has built good friendships and hesees these clients regularly.

M recently attended a review of his support with hissocial workers and has dramatically reduced hishours, so that he can do as he pleases, as he is nowmuch more able. M also asked his mum torelinquish her appointeeship and M has control ofhis own finances. We are very proud of him.

The second example is client A. Client A has alwayslived with two other clients in a three person service.One client moved into independent living threeyears back, leaving a void in this service. A wasadamant he didn’t want anyone to move in and wasvery anxious when another client was identified tomove in.

A did very well and accepted this gentleman into hishome, but then the gentleman also moved out tolive independently. So this put the service back to atwo person service with a void. To complicate thingseven more, his housemate formed a relationshipwith a lady friend and now spends very little time atthe service. This meant that A was spending a lot oftime at home alone and his support ends at 9pm,meaning he was totally alone.

He is a very able gentleman, so his needs mean thathe does not need more support, but his wishes arethat he has support for company. We had issueswith him making suicide threats and a variety ofdifferent threats to himself and property. After lots ofwork with A, he has started to do really well. He isliving pretty much alone at the minute, with support,and a new client has been identified to move insoon. This would have been a huge problem with Aat one point, as he didn’t want anyone new to movein, he wanted his housemate to stay home more.

He has massively surprised us and has been veryreceptive to the new client, asking when he ismoving in etc. planning Christmas with him and haseven expressed wishes to live independently in aflat of his own one day and we are working towardsthis. He settles when staff leave and is fine whenthey come back and he has really shown maturity.

We are very proud of him and his progress. We aredefinitely looking into him having his own place andwill support him through every step of this.

Here are some examples of how St Anne’s hasmade a difference to the lives of people beingsupported in Doncaster.

We support a young man whose confidence wasrock bottom when he came to live with us, he’s nowtravel training to go to a local Resource Centre eachday. This involves two buses each way and thelatest report is that he is doing ‘brilliantly’.

Another gentleman was found to have diabetes andhe has lost nearly two stones in weight. He has afixed bicycle in his conservatory which he “cycles” onfor 30 minutes each night. He has also securedvoluntary employment which he loves.

M expressed a wish to go on holiday to Spain. Wewrote him an itinerary as he wanted to visitMonserrat, Barcelona. He did all these things andmuch much more, and he is already planning fornext year with a different itinerary.

W wanted to get out into the community more, butdue to his inability to travel on public transport, thiswas proving problematic. We applied for a PIP forhim, but this was refused. We appealed and he isnow the owner of a shiny new car on the drive.

We support P who was extremely scared of going outinto public places. We felt that her vision wasn’t quiteright and the reason she had lost confidence wasthat she couldn’t see properly. We had her eyeschecked and she was found to have cataracts in botheyes. These have now been removed and she cansee a whole new world.

C had always dreamed of going to Florida, due to thedistance we spent a great deal of time planning this.We applied to her Trust for the money and luckily thiswas granted. She has now fulfilled one of her dreamsand has lots of lovely pictures as memories.

Julie joined St Anne’s in 1992 when she transferred from the thenCalderdale Health Authority to take up the post of nursing homemanager. Since that time Julie has held a number of posts at variouslevels within the organisation.

The Financial Statements for the year ended 31 March2015 on which the Auditors issued an unqualified auditreport were approved by the Board on 29 July 2015. TheFinancial Statements have been submitted to the CharityCommission and delivered to the Registrar of Companies.

Statement of the Auditors to the Membersof St Anne’s Community Services

We have examined the summary financial statement ofSt Anne’s Community Services for the year ended 31March 2015 which comprises the Income andExpenditure Account and the Balance Sheet set out onthe previous page.

This report is made solely to the association’s membersas a body in accordance with section 427 of theCompanies Act 2006 and the regulations madethereunder. Our work has been undertaken so that wemight state to the association's members those matterswe are required to state to them in an auditors’ report andfor no other purpose. To the fullest extent permitted bylaw, we do not accept or assume responsibility to anyoneother than the association and the association’s membersas a body, for our audit work, for this statement, or for theopinions we have formed.

Respective responsibilities of directorsand auditors

The directors are responsible for preparing the 2015Annual Review in accordance with United Kingdom law.Our responsibility is to report to you our opinion on theconsistency of the summary financial statement withinthe 2015 Annual Review with the full annual accountsand its compliance with the relevant requirements ofSection 427 of the Companies Act 2006 and theregulations made thereunder.

We also read the other information contained in the2015 Annual Review and consider the implications forour report if we become aware of any apparentmisstatements or material inconsistencies with thesummary financial statement.

Basis of opinion

We conducted our work in accordance with Bulletin2008/3 ‘The Auditors’ Statement on the SummaryFinancial Statement in the United Kingdom, issued bythe Auditing Practices Board for use in the UnitedKingdom. Our report on the association’s full annualfinancial statements describes the basis of our auditopinion on those financial statements.

Opinion

In our opinion the summary financial statement isconsistent with the full annual accounts for the yearended 31 March 2015 and complies with the applicablerequirements of Section 427 of the Companies Act 2006and the regulations thereunder.

We have not considered the effects of any eventsbetween the dates on which we signed our report on thefull annual financial statements, 18 September 2015, andthe date of this statement.
