Dear Friends of Community Health Alliance: How else to describe the changes to our organization in 2012- 2013 except as momentous! On July 1st, 2012 Saint Mary’s Regional Medical Center came under new management and spun off their Mission Outreach programs. All of these valuable health care programs served the same population of patients as did HAWC-those patients with little or no insur-ance who otherwise would not receive basic health care. Thanks to the leadership of mul-tiple parties from both HAWC and Saint Mary’s, all of the programs merged and became Community Health Alliance (CHA), complete with its own separate non-profit Community Health Alliance Foundation.

CHA now operates five fixed sites of primary health care-Wells Avenue, Neil Road, Sun Valley, Outreach (Community Assistance Center at 4th and Record Street) and Virginia City. CHA is also responsible for mobile dental sealant/restorative vans, a medical van and the Hospital Dentistry Program for disabled children. The Northern Nevada Dental Health Program and CHA are now closely associated and have recently added the Adopt A Vet Dental Program. The WIC program and Ronald McDonald van are also under CHA. Immunize Nevada and the Nevada Cancer Coalition were administered by CHA for much of the year, but have recently become separate entities. All told we now have 10 sites, 185 employees and, over the past year, we have provided over 120,000 patient visits!

CHA has expanded the definition of primary health care in an effort to help our patients achieve the WHO definition of “health”- state of complete physical, mental and social well-being and not merely the absence of disease or infirmity. We have integrated mental health, behavioral health and substance abuse into our health center sites. The University of Nevada School of Medicine and UNR School of Psychology have been of tremendous help with this initiative, with advanced students and faculty co-located in our health centers. We offer access to legal services through our agreement with Washoe Legal Services. We are actively involved in the Bridges Out Of Poverty initiative. All of these “wrap around services” are necessary to address the social determinants of disease.

As our services expand so too does the need for support from our community. We have not received any more Federal dollars despite the merger and expansion of services: Federal support amounts to no more than 15% of our total budget. We need you, our community partners, to help fund our expanding services for those who need them most. I am grateful to all the wonderful, generous donors who continue to believe in and support our mission. You are the reason why CHA can thrive and grow to meet community needs. Began providing integrated behavioral health services for children and adults. Mental health services are provided at the time of a primary care visit.

Modernized all of our health centers and programs with electronic medical records. Sought recognition as a Patient Centered Medical Home and strengthened Quality Improvement processes.

Created partnerships with a wide range of community organizations to ensure that patients receive all of the services they need. Partnerships provide access to mental health care, legal services, literacy services, financial counseling.

M ore northern Nevadans received high quality health care because Community Health Alliance was formed, increasing access.

Community Health Alliance was created July 1, 2012 with the merger of Saint Mary’s Mission Outreach and HAWC Community Health Centers. The organizations had similar missions, and shared the commitment to provide high quality health care services to underserved populations in northern Nevada. Combined, the organization provides care through five fixed-site health centers, four mobile units, and numerous WIC offices. Cooperative agree-ments brought together additional providers, such as the Northern Nevada Dental Health Program and Adopt a Vet Dental Program.

Community Health Alliance is a 501c(3) nonprofit entity and a Federally Qualified Health Center (FQHC). More than 120,000 patient visits were provided during the period of July 1, 2012 through June 30, 2013. Patients may use all public and private insurance products to cover their care. Uninsured patients are charged based on a sliding fee scale that is adjusted according to income and family size. Community Health Alliance medical and dental programs serve persons of all ages. During the 2012/2013 fiscal year, Community Health Alliance worked towards integration of all services to support holistic care for our patients. With the help of community partners, such as the University of Nevada School of Medicine, UNR, and other nonprofit entities, we provided a robust mix of primary medical care, behavioral health care, dental care, nutrition counseling, and many other critical services. To support these services, the Community Health Alliance Foundation was created July 1, 2012. Governed by a separate Board of Directors, the Foundation is responsible for raising funds to support Community Health Alliance programs and services.

I have been blessed that for almost forty years I had full access to the very best medical care with top-flight health insurance coverage. In 2009, that came to an end. Though I am new to HAWC, I am astounded at the work done here. The physical environment is as clean and bright as any primary

care physician’s office. The staff are gracious, client-centric, compassionate (even when it is hard to be) and competent at a level equal to any I have experienced elsewhere, and better than most. Thank you for the work you do, your energy and passion, and the blessing you are to our community.

Our Mission Community Health Alliance's mission is to provide quality, afford-

able, comprehensive primary health care services for those in need.

I just wanted to say how much I appreciated every-one who helped us today. Everyone was so helpful and kind. This was my first touch with the HAWC Clinic with my niece and sister-in-law. Thank you all for your immense help and kindness.

Community Health Centers—Some History

Health centers owe their existence to a few determined community health and civil rights activists working in low‐income communities during the 1960s. Millions of Americans, living in inner‐city neighborhoods and rural areas throughout the country suffered from deep poverty and a desperate need for health care. Among those who were determined to change those conditions was H. Jack Geiger, then a young doctor and civil rights activist. In the 1960s, as the “War on Poverty” began to ripple through America, funding was approved for the first two neighborhood health center demonstration projects, one in Boston, Massachusetts, and the other in Mound Bayou, Mississippi.

But it was the passage of the Economic Opportunity Act of 1964 that marked the birth of America’s Community Health Centers. The health center model that emerged targeted the roots of poverty by combining the resources of local communities with federal funds to establish neighborhood clinics in both rural and urban areas. It was a formula that not only empowered communities, but also generated compelling proof that affordable and accessible health care produced compounding benefits. Studies showed that the health center model reduced health disparities, lowered infant mortality rates, and reduced chronic disease. Health centers created jobs and other investments in hard pressed communities, and produced cost‐savings for the health care system by reducing the need for acute care at hospital emergency rooms.

Community Health Centers have stayed true to their roots, continuing the model of integrated health and social services to combat the health effects of poverty. This rich history has influenced Community Health Alliance in developing a robust mix of programs to serve northern Nevada’s most vulnerable patients.
