I temporarily relocated to Niagara Falls where my family resides. I currently work for Niagara Falls Memorial Medical Center (NFMMC) as an Electronic Health Records Specialist, gaining valuable experience while I conduct my Central Florida-based career campaign. NFMMC is part of the largest health system in Western New York that includes Kaleida Health, Catholic Health and Rochester Health. My goal is to secure a HIT leadership position that is commensurate with my vast background and progressive experience! I have a superb track record in the WNY and FL multi-specialty physician, primary care physician, hospital, post-acute care, IDN, IPA, GPO, MSO, PHO, ACO, TPA, ACLF, LTC, mobile health, home health, drug & alcohol treatment centers, primary and behavioral health care integration, managed care, government RFPs, retail pharmacy, and corporate arenas. All of these providers would highly benefit from latest HIT platforms. Health Information Technology is a key enabler and potential accelerator of the successful adoption of personalized medicine throughout the entire healthcare continuum. Through my involvement in Integrated Delivery Networks, I have seen first-hand that it does take teamwork to transform healthcare. I LIVE IT EACH DAY! I am dedicated to building connected communities of PATIENT- CENTRIC health with Population Health Management Solutions (PHMS) and Professional Business Services (PBS) as the vehicles to improve the delivery and increase access to cost-effective, value-based healthcare. The Western New York and Florida provider communities are gradually adopting specific PHMS and PBS. The providers have implemented programs such as PCMHs, Health Homes, Integrated Primary Care & Mental Health Services, and Medication Therapy Management that have significantly reduced hospital stay and readmissions as well as the number of visits to costly ERs and convenient care facilities. It is crucial that providers have access to real-time, user-defined algorithm support tools that will help identify and stratify high-risk populations through the seamless and rapid flow of digital information, including genomic, clinical outcome, and claims data. In turn, both markets are ripe for Accreon’s innovative technologies that will maximize Triple AIM Roadmaps for Healthcare in order to comply with and financially benefit from federal and state incentive programs. In addition, NFMMC, Kaleida Health, Catholic Health, and numerous other providers are interfaced with the RHIO (HEALTHeLINK). The goal is to achieve true Connected Communities of Health Interoperability through plug-in models that enable seamless

integration with practice workflow that will accommodate future HIT, mobile and telemedicine applications. Through Patient & Consumer Engagement Programs and Outcomes Management and continuous quality improvement, the PCPs are the hub of all relevant activity and responsible for the coordination of care throughout the entire healthcare continuum from “Womb to Tomb.” In WNY and Florida we are truly transforming healthcare through collaboration within our connected communities! Poised for advanced challenges, I look forward to contributing to an organization’s

success through my outstanding understanding of Integrated Delivery Networks and

Health Information Technology’s role in our ever-evolving healthcare environment. I

am seeking progressive challenges and responsibilities that will enable me to

proactively utilize my vast wealth of knowledge and experience. Most importantly, I

want to be a driving force that will enable a company to execute its Mission through its

Roadmap strategies and objectives!
