PREFACE ThishandbookwasoriginallywrittenbythreemembersoftheClassof’83,withrevisionsalmosteveryyear.Asof2016,thepre-clinicalPresidentoftheSenateholdsthiseditingresponsibility.Thefirsteditionwascompletedwithvaryingdegreesofdistinction.Thesuggestionsandopinionsherewereforthemostpartwrittenfromthestandpointofthe“average”student.Wewrotethishandbooktogiveyoutheadvantageofourexperienceinthehopethatitwilleaseyourpassagethroughthefirstyear. Thetipsonacademicperformance,exams,etc.,thoughapplicablethroughtheentireyear,werewrittenprimarilywiththefirstsemesterinmind.Importanttipsforsecondsemesterclassesarethoroughlyincluded,though.Bywinterbreak,youwillbeoldhands.Althoughyourcurriculumwillnotbeidenticaltothepreviousyears,theadvicehereissufficientlygeneralthatitwillbeapplicabletoyoursituation. Parking,CommutingandSafety

Parking: Forstudentswhocommutetoschool,youhaveseveraloptions:parkinginthegarage,streetparking,takingthebus,biking,orsomecombinationofthese.

Theparkinggarageforstudentsislocatedat620MuhammadAliBlvd.(thecornerofMuhammadAliandClayStreet,entranceonMuhammadAli),adjacenttothenewClinicalandTranslationalResearchBuilding.Thisisabout3blocksfromthemainmedschoolbuilding.ClayStreetistwo-ways,butMuhammadAliisone-way(eastbound).

youearlyonintheyear.Somepeopleusethree-ringbinders,someusefilingcabinets,andsomeusefolders.LaptopusersmayfindthatusingOneNoteishelpfulfororganizing,andallowsthemtogopaper-freebyprintingthematerialtotheircomputers.Developasystem,improveasyougo,anddon’tprocrastinate.You’llbemuchhappierontestday.Feelfreetotalktoyoursecondyeartoseewhatworkedforthem,aswellascommunicatingwithyourpeersabouttheirsystems.

Withtheexceptionofsomepracticalexams,allofyourfirstyeartestquestionsaremultiplechoice.

Thetypesofquestionsthatyouwereaccustomedtoinundergradarefewandfarbetweeninmedicalschool.Inthepast,you’veprobablybeenabletostudyenoughtogetabasicunderstandingofthematerialandthenuselogictofillinthegaps.Inmedicalschool,thequestionsaskedareaboutthegaps,andlogicwon’talwayshelpyou.

Itisnecessarytosetreasonablegoals.Althoughyoumayhavegottenabovea90%onmostofyourexamsinundergrad,thisisrareinmedicalschool.Youshouldgetusedtoalotmorescoresin70’sand80’sthaninthe90’s.Additionally,youshouldexpecttheprofessorstoslightlyalterthedifficultyoftestsbaseduponthepastperformanceofyourclass.Iftheaveragewasveryhighonthepreviousexam,expectthenextonetobemoredifficult.

Generallyspeaking,timelimitsarenotaproblem,exceptonpracticalexams.Onthewrittentestsyouwillhaveplentyoftimetogobackandcheckyouranswers,butyouwon’tonpracticalexamsbecauseyoucan’trevisitastationonceyou’veleftit.ThebestadviceIcangiveyouaboutpracticalsistostaycalm.Ifyoudon’tknowwhatastructureis,don’tpanicandputdownyourbestguessoracluethatmayjogyourmemorylater.Onceyouleavethatstation,putitoutofyourmindandfocusonthenewone.Atreststations,gobackandreviewyourcluestoseeifyoucanmakeaneducatedguess.

NBS-3:NormalBodySystems3

NBS3isthefinalcourseoffirstyearbeginninginmidApril.ThesolefocusofthecourseisNeuroanatomyandNeuralSystems.Lastyear,onlyanatomywastestedonthe1sttestwithonlyonepractical.Thiscoursecanbechallengingtomanysobesuretolookatthelistofpopularresourcesforhelp.

Neuroanatomy

Initiallythecoursefocusesonstructures.StructuresarefirstintroducedthroughSidman&Sidman,aselfinstructionalguide,followedbylecturesandlabsonneuroanatomy.Followingthe1sttest,therearethreeweeksofjustneurosciencelecturesfocusedonteachingfunctionandpathways.

IfyoutakethetimetomasterthematerialinSidman’satthebeginningofthecourse,itwillpayofflaterwhenyouwillneedtoknowthenitty-grittydetailsofthevariouspathwaysandtracks.AllowplentyoftimeforSidmanandSidman’sself-study(DON’TCRAMITIN!).Thetestoverthismaterialisaneasywaytobuildasmallbufferofpointsfromthebeginning.Butbepatientwithit,asitwilloftenfeellikeyouarejumpingaround,memorizingstructuresandfunctionswithoutanyfoundation...andyouare.Justthinkofitasmemorizingalistthatwillhelpwhentheactualclassbegins.Also,itiswisetohangontoyourSidman’sbookincaseyouneedtoreviewanypathways.

Oncelecturesbegin,utilizethenotesandthelistofitemsinthelabhandbook.Thefacultywillhopefullygiveyouaccesstotheircomprehensiveand(VERY)usefulnotes(theydidn’tprovidethemlastyearforus,butyoucanobtainthemfromupperclassmen).Thequestionswillprettymuchalwayscomestraightfromthese,withtheexceptionofSidmanandSidman.LikeinAnatomy,structuralrelationshipsareusefulsobeabletotracenuclei,tracts,fascicles,andnervesthroughoutthebody,spinalcord,andbrain.

A tutor for the lab sections of this class is also a GREAT idea, even if you are not having difficulty during the scheduled lab sections. Alternatively, towards the end of a scheduled lab session, grab a TA and have them go over everything with you. If you can’t seem to get this done in lab, get the TA to tutor you. It’s a great way to become familiar and confident with the material. Repetition and peer-teaching are key so review with your classmates. When brainstem images (i.e. the rorscharch test blob images) come around (you’ll know what I’m talking about it when you get them!), begin memorizing the images early, just a few a day. If not, it will become overwhelming and a lost cause. Use the powerpoint slide that the professors provide to drill the structures in your head!

Once function comes around, keep up! The information comes fast so you don't want to fall behind. Again, the information you need to answer exam questions will be in the notes and using any old exams will be quite useful.

The course concludes with clinical case presentations. These are great review tools and easy points. They never change the cases, so consult previous classes for useful information. First of all, be sure not to plagiarize. Second, make sure and work through all of the cases on your own so you fully understand them because you will see similar questions on the final. Finally, show up to your cases a few minutes early prepared to present as some expect this from you.

In conclusion, keep up, do your Sidman & Sidman, spend lots of time in lab (with classmates or a tutor), and study the notes for all lectures. While it's hard to focus on neuroscience when summer break is so close, it is a challenging class which requires work. Don’t slack off now! Tips for NBS 3:

1) A popular saying for this course is that everything starts making sense at the very end of the course. In other words, a lot of concepts and pathways will start to make sense when the course is about to finish. With that said, use this as a “comforting tool.” While you may struggle through the first run through a lecture and not understand any of it, you have to trust that it will start to make sense. And trust me, it does!

2) The Brainstem lecture from Dr. Petruska seems at first intimidating and overwhelming. And quite honestly, it is! The fact you have to learn all the different nuclei and tracts for 12 different cranial nerves is surely overwhelming. But take it in small bite sizes. The brainstem lecture sets up the foundation for the pathways you learn for the function component of the course. So do not neglect them. But that means, it doesn’t have to make complete sense when you first learn them, because you will get exposed to them many times along the course.

Nooneisgoingtodenythefactthatmedicalschoolisdifficultandstressful,andthiswillbegintotakeitstolloneachandeverystudentastheyearpasses.Herearesomepointersthatyouwillhopefullyfinduseful:

1. Youarenotalone.Everyoneisstressed,eveniftheydon’tshowitopenly.Itisonlynaturalthatthisdifficultsituationwillaffectyouinsomeway,butdon’tletyourstresstakecontrolofyourlife.

2. Don’tdoubtyourchoiceofcareerjustbecauseyou’resickofbasicsciencecoursesandunderalotofpressure.Yourfuturemedicalcareerwillprobablynotresembleyourfirstyearofmedicalschoolinanyway,sogiveyourselfachance.

3. Chancesare,youwillnolongerbeatthetopofyourclass.Trynottocompareyourselftootherstoomuch,becauseeveryonecomesinwithadifferentbackgroundanddifferentstrengths.Aslongasyoucontinuetopass,youhavenothingtoworryabout.

4. Getenoughsleep!Aminimumof6-8hoursanightisabsolutelynecessary,especiallyaroundtesttime.If

you’remorerested,thenyourstudyingismoreefficientandyou’relesslikelytoseeyourperformancedropoffneartheendofalongdayoftesting.

5. Exerciseandeatwell.Thisdoesn’tmeanyouhavetochangewhoyouare.Justaddadaily30min.walkoryoga,andloaduponhealthyfoods.Thiswilldowondersforyourmindandyourbody.

Priortomedicalschool,mostofyouwhoaremarriedorinsignificantrelationshipsprobablyhadplentyoftimeforwork,school,andyourpersonallives.Unfortunately,thatwillprobablychangeforyouthisyear.Hereisalistoftipsandpointerstohelpyoumangetheproblem:

1) Thefirstthingtorememberistheimportanceofgoodcommunication.Allofthesubsequenttipsareuselesswithoutit.

2) Rememberthatyourstatusasamedicalstudentputspressurenotjustonyou,butoneveryonethatisclosetoyou.Don’tthinkthatyou’retheonlyonethatisaffected.Makesureyourlovedonesunderstandtheamountofcommitmentmedicalschoolrequires,butdon’texpectthemtosufferinsilence.Takethetimetoaskyourpartnerhowtheyfeel,listenclosely,andshowhimorheryoucare.

3) Justbecauseyou’reinmedicalschooldoesn’tmeanthatallofthehouseholdchoresshouldgetpushedoff

onyourpartner.Therearetimes(likearoundfinals)whenyoureallydon’thavetimetodothedishesortakeoutthetrash,buttrytomakeaneffort.

4) Planningisessential.Althoughitdoesn’tsoundveryromantic,youmayhavetoscheduletimetobewith

yourpartner.Thequantityoftimethatyouwillhavetospendwithyourpartnermaydecrease,butyoucanoffsetthisbyimprovingthequalityofthetimeyouhavetogether.Leaveyourscholasticissuesatthedoor,andfocusyourfullattentionontheonesyoulove.

5) Duringthesummerbetweenfirstandsecondyear,youwillhavealotoffreetime.Itmaybeagoodideato

It’seasytogetcaughtupwithclassesandeverythinggoingonthefirstyearandforgetwhyyouarereallyhere:tobeadoctor!Careerplanningisanincreasinglyimportantpartofyourmedicaleducationasresidencyprogramsbecomemoreandmorecompetitive.Thereareanumberofclubsthatcanprovideinformationaboutspecificcareers(InternalMedicineClub,etc),andthefollowingaresomeresourcesthatcanprovideyouwithmentorswhoarefamiliarwithspecificfields.

PALSProgram:ThisprogramisthroughtheGreaterLouisvilleMedicalSocietyandpairsfirstyearstudentswithphysicianmembersoftheGLMS.Studentsmeetwithmentorsonceorsoasemester.

Theboardsareathree-stepprocessofbecominglicensedtopracticemedicine.StepIandStepIIaretakenduringmedicalschoolandapassinggradeonbothisrequiredforgraduating.Inresidency,you’lltakeaStepIII.TheboardsaretogethercalledtheUSMLEStepI-IIIexams(U.S.MedicalLicensingExam). StepIistakeninMayorJuneaftersecondyear,beforethirdyearbegins.Thetestiscomputerformattedandlastseightconsecutivehours,includingonehourofbreaktime.Apassingscoreof192isestimatedtobeabouta55-65%score.Studentstendtobeespeciallynervousaboutboards.You’llhearallofthetimeabouthowimportanttheyareforgettingintoresidencies,especiallyifyouwanttobecompetitive.Anditdoesn’thelpthatthetestcancoveranypossibledetailyoulearnedovertwomaterial-packedyearsofmedicalschool. Astudyshowedthat70%ofstudentscomeoutofboardsthinkingthattheyfailedthem.Fortunately,therealfailratesarearound4-5%.In2005,theaveragepassrateforamedicalschoolwas93%withascoreaverageof217amongstallfirsttimeUSandCanadiantesttakers.AtUofLthatyear,thepassratewas94%andtheaveragescorewasa219.IfyouarecuriousnowaboutwhattheStepIaveragesareforeachresidency,thewebsitetofindthatinformationishttp://www.nrmp.org/data/index.html. ItiswellnotedatUofLthatalthoughMCATscoresofmatriculatesarehoveringaroundaveragenationally,theretendstobebetterthanaveragescoresonStepI,whencomparednationally.Block-styleexamsandproblem-andteam-basedlearninghavelikelybeenacontributoryfactoroverthepasthalf-decade.Blockexamsprepareyoutocramalotofmaterialintoyourheadinashortperiodoftime,andhelpyoubuildupyourintegrityforsittingthrougha7-hourtest.Thecurrentadministrationhasanintentfocusonpreparingstudentsforboardexamsbetterthanever.Justinthepastcoupleofyears,the2ndyearcurriculumhasbeenentirelyrefacedtofacilitateboardprep.Allinall,theschoolismovingclosertoasystemsbasedteachingstyleandrecentclassesboardscoreshaveshownsignificantimprovementovernationalaverages. Sohowshouldyouprepare?Ifyouarereadingthisasanincomingorcurrentfirst-year,don’tworryaboutitnow.ThebestthingyoucandountilSpringofyoursecondyearcomesaroundistostudyhardforallyour1stand2ndyearclasses.Youwillfindthatalotofrecommendedbooksforyourclassesareactuallyboardstudybooks,anyway.FirstyearmaterialisactuallylowyieldforStep1.ThereisEmbryology,Anatomy,Neuroanatomy,Physiology,andBiochemonstep1,butmostquestionsthatrelatetothesealsoexpectyoutoknowsomePharmacologyorPathology,whichis2ndyearmaterial.2ndyeariscumulativeenoughthat1styearmaterialwillnotseemthatdistantbythetimeboardscomearound. Whatyoushouldplanondoingisbuying“FirstAidfortheBoards,”touseasastudyaid2ndyear.Itmayintimidateyouthatitistheboardstudybook,butithasamazinglists,charts,mnemonics,andfigurestohelpyoulearnessentialsfor2ndyearcourses.Ifyouuseanybooks2ndyear(it’snotverynecessary),useFirstAid.Anoldeditionwillnotbebad,andyoucangetthenewoneforlessthan$30whenitcomesoutintheSpring. Onemore“boardprep”resourceyoucouldbenefitinusingearlyisUSMLEWORLD.Youwillfindthatin2ndyear,coursepracticequestionsareveryscarcecomparedtowhatyougetusedtoin1styear.WORLDisthesolutiontothatdilemma,andthequestionsareincrediblysimilartorealstep1questions(manyconsideritharder).Theonlydownsideisthattouseitfortheentireyear,youhavetocashoutbigtime($400),ascomparedtothe$135toonlyuseitforafewmonthsofboardprep.WORLDisapopularstudysourceforStepIIprepand3rdyeartoo. Secondyearendsthe2ndtolastweekofApril,givingyouover2monthsforboardprepandbreaktime.Manystudentsstudy4-7weeks,with8-12hourdays(andoftenadayortwooffperweek).Yousetupaschedule,makeastudyplan,pickyourresourcesandgoatit.Fortunately,thelastblockofschoolistypicallythemostrelaxedof2ndyear,givingyousometimetopsychyourselfup.Manyboardprepcourseoptionswillbethrownatyouduring2ndyear.Thosewhoarebiglecturefansareprobablythemostlikelytoendupusingthese. StepIIofboardsistakenfollowingthirdyear,typicallybetweenJulyandDecember.Thecontentfocusesonthirdyearmaterial.StresslevelstendtobelowerforStepII,likelybecauseofthecumulativenatureofworkingintheclinics.Also,residencyprogramstendtofocusmoreonStepIscores.Oftentimes,moreemphasiswillbeputonStepIIifyouneedsharpenupaCVwithalessthanperfectStepIscore. You’vespenttwoyearsdroningthroughanunhealthybalanceofhardcorestudyingandpost-blockcelebrating.Finallyyou’rereadytoplaydoctor.I’llgoaheadandpointitoutnow-youwillstillfeellikeyouknownothing,andfromtimetotime,it’llseemliketheresidentsandattendingarehappytodrillthatrealizationbackonyou.Nothingcantotallyprepareyouforthestartofthirdyear.Youjusthavetotrustinyourabilitytoapplyyourknowledgeandrememberthatbecomingandbeingadoctorinvolveslife-longlearning. ThebestpieceofadviceI’veheardregardingclinicalyearsisto“takeownershipofyourpatient.”Youwillbeassignedpatients,anditwillbeuptoyoutorevieweverythingyouneedtoknowregardingtheircondition,andtobereadytopresentthemtotheresidentsandattending.Youshouldtrytobeapersonalizedexpertoneachpatientyouareassigned. Onatypicaldayonanyhospital-basedrotation,yourschedulewillbeasfollows:roundandwritenotesonthepatientsyouarefollowing,andthenroundwiththeteam.Theafternoonisspentwritingorders,checkinguponlabvaluesorimaging,andadmittingnewpatients. Likeresidents,medicalstudentstakecall,whichwillvarybyrotation.Ingeneral,youwillfollowthecallscheduleofyourteam,oroftheresidentwithwhomyouarepaired.Dependingontherotationandthehospital,youmayberequiredtospendthenightinthehospital,inwhichacallroomwillbeprovidedforyou. Duringthefamilymedicinerotation,youarerequiredtospendsometimeatacliniclocatedinaruralKYsetting(AHEC).Thefamilymedicinecoordinatorwillplaceyouatafacilityandprovideaplacetoliveduringthattime.Itisauniqueexperiencetohaveindividualattentionwithaphysician,somakethemostoutofit!ThosewithspecialcircumstancesaretheonlyoneswhogetprioritytoworkinJeffersoncounty(planonhavingakid<6moold). Standardizedpatientexams:Somerotationshavearequiredstandardizedpatientencounterattheendoftherotation.ThesearesimilartoencountersduringICM-2,andyourpatientencountersduringeachclerkshipwillprepareyousufficiently.
