SUBJECT AND PURPOSEThe subject of this plan is defining key processes and activities as well as the executors to be engaged in materialization of the Project defined by Tender: Lot 1.1 E-75 (M-22), Final Design and construction of a new motorway section from km 108+000 to km 117+000 called for by "Srbija Putevi" Public Company for State Roads Management, awarded to J.V. "Energoprojekt Niskogradnja" & "Inter-most".Integral Joint Venture Agreement signed by "Energoprojekt Niskogradnja" Joint Stock Co. (as the Leading Partner) and "Inter-most" Joint Stock Co. (as the Partner) defines and regulates their interrelations as well as distribution of works the Partners should execute on the Project E-75 (M-22) Motorway from Novi Sad to Belgrade, Lot 1.1: km 108+000 to km 117+000,The purpose of this plan is clear definition of responsibilities described in the Tender Documentation, which should be fulfilled during the Project execution.The Plan is also assigned to the Resident Engineer as a presentation of the way the Joint Venture should be organized and specification of documents that will be used for the Project execution.2. APPLICATION SCOPEThis Quality Assurance Plan will be applied by organizational parts of the Joint Venture Partners participating in the Project execution and as well at the site.3. NISKOGRADNJA joint ventureMOTORWAY E-75 (M-22) PROJECT BELGRADE - NOVI SAD, LOT 1.1 Section km 108+000 - km 117+000Chief of Surveying Section is responsible for: -taking-over of the geodetic survey data; -recording of "zero state" and survey of the work phases; -transfer of design data to the Site; -provision of data for preparation of as-built drawings -communication with the Resident Engineer.Chief of Laboratory is responsible for: -keeping a record and filing of materials test certificates ; -control of input materials quality -delivering of samples to accredited laboratory for testing, in accordance with the Quality Control Plan; -communication with the Resident Engineer.If some of the managing personnel is temporary absent, the Project Manager shall inform the Chief Resident Engineer about it, and shall appoint an acting person.If General Managers of the Joint Venture members recall some of their managing personnel for the Project, or the Chief Resident Engineer gives an order for his removal from the Project, the General Managers shall appoint adequate new person for the function. Quantity & Cost Control Chief Engineer submits his data to the Chief Resident Engineer.If the Project Manager, as the Contractor's representative is temporary away or recalled (by decision of the General Manager or by the Chief Resident Engineer cancellation of the approval), a new person will be appointed, subject to the Resident Engineer's approval.Organization Chart for the Project is given in Appendix No. 1.4.3.ORGANIZATION OF THE WORKS EXECUTION DESIGNThe Organization of the Works Execution Design includes: -method statements for the Works execution, -temporary traffic regulation design, -list of the construction plants and equipment -Time schedules - physical and financial.Technical Manager is responsible for preparation of the Organization of the Works Execution Design.4.4.QUALITY MANAGEMENT SYSTEMQuality management systems of Energoprojekt Niskogradnja are defined by the quality management system documentation and are in accordance with JUS ISO 9001:2001 standard requirements. Certificates of the Joint Venture members are given in Appendix No. 2.Quality management system on the project is a responsibility of the Project Manager, whereas the Technical Manager and Chief Engineers shall be responsible for its functioning. System functioning shall be analysed on daily and weekly working meetings.11070 BELGRADE, BULEVAR MIHAILA PUPINA 12, P.FAH 20EflBnENERGOFROJEOT Contractor IMTED m OQT HICmNISKOBRAPNIA joint venture IMTCKliJUO MOTORWAY E-75 (M-22) PROJECT BELGRADE - NOVI SAD, LOT 1.1 Section km 108+000 - km 117+0004.5.DOCUMENTATION MANAGEMENTAll documentation shall have a visible evidence of the previous approval given by the appropriate authority.Quantity & Cost Control Chief Engineer records, keeps and files all documentation and distributes necessary copies.Records (unless in free form) shall be completed on forms defined by the Quality System Management documentation of the Contractor, and on forms approved by the Resident Engineer. Responsibility for filling, distribution, keeping and recording of records is on the function mentioned in item 4.2. from whom record proceeds.All changes must be approved by the Resident Engineer, in a way defined by the Contract.4.6.DESIGN MANAGEMENTAll Design documents shall be prepared, reviewed and verified in accordance with applicable laws and the contract conditions.Design documentation shall be prepared by the Designer - CPV, on bases of inputs: the Preliminary Design which forms an integral part of the tender documents, geodetic survey of the "zero state" approved by the Resident Engineer as well as other data base applicable by law.Design documentation shall be prepared by designers with particular license, experience and competence, according to current law regulations and Contract requirements.Six copies of the Final Design shall be delivered to the Resident Engineer, out of which two approved copies shall be returned back to the Contractor: one to be filed and the other to be used during the execution of the Works.All amendments in design documents shall be submitted to the Engineer for the approval in accordance with applicable procedure.The As-built Drawing, prepared on the basis of completed work items, shall show all changes and discrepancies (if any) in relation to the Final Design. It is submitted first to the Resident Engineer and then to the Engineer, to acquire the Certificate of Occupation .4.7.PURCHASE MANAGEMENT4.7.1. Suppliers SelectionManagers of Departments/Chiefs of Commercial Sections shall, before the beginning of the Project execution and on the basis of their list of assessed suppliers, address their proposal to the Project Manager. Selection of potential suppliers is made by the Contractor's commission which shall forward to the Resident Engineer the following documents for his approval. -proposition of suppliers and supply sources (for the required works) -documented information demonstrating that the purchased material will be in compliance with the requirements; -documented information demonstrating that the proposed sources can meet the demands, -key material purchasing schedule.11070 BELGRADE, BULEVAR MIHAILA PUPINA 12, P.FAH 207~~TelT(011)310-8340,310-8330,310-8320 * FAX(011)311-24938Tel.: (Oil)310-8340,310-8330,310-8320 * FAX(011)311-24938Tel.: (Oil)310-8340,310-8330,310-8320 * FAX(011)311-2493ENERGOPROIEKT Contractor: NISKOGRADNJA joint ventureMOTORWAY E-75 (M-22) PROJECT BELGRADE - NOVI SAD, LOT 1.1 Section km 108+000 - km 117+0004.7.2.Material and Plant PurchaseBefore the commencement of the works, on bases of prepared time schedules, the Technical Manager, in cooperation with the Quantity & Cost Control Chief Engineer, prepares a purchase schedule of material with defined monthly quantities of bulk material that will be brought to the Site and, if necessary, stocks that will be maintained as well as the schedule for procuring specific items. This plan is submitted to the Resident Engineer for the approval within 28 days after the commencement of the works.Once the Works are commenced, the Technical Manager and the Chief Engineers shall, on the basis of time schedules, prepare major materials purchase list, specifying the quantity that shall be required monthly, i.e. the purchase list for certain items. These lists shall be submitted to the Resident Engineer for his inspection and approval, 28 days after signing the Contract at the latest.On the basis of prepared monthly material purchase lists, the Technical Manager/Chief engineers shall submit, through their requisitions, the specified quantities and the required quality of materials to the Manager of Department/Chief of Commercial section who shall, on the basis the requisition plan and the list of suppliers approved by the Engineer, organize the purchase of required material, manufactured articles and/or plant.4.7.3.Receiving of Material and PlantQuantitative receiving inspection of purchased material, manufactured articles and plant is performed by warehouse storekeeper. Testers perform qualitative assessment, take samples of particular material and deliver samples to the certified Testing Laboratory for testing, in accordance with appropriate procedures.Receiving, storing, keeping and distribution of purchased material in the Site is responsibility of warehouse store keepers.Resident Engineer's personnel is entitled to examine, inspect, measure and test the material and to check the progress of construction works and production and manufacture of material.4.8. RESOURCE MANAGEMENT 4.8.1. Human ResourcesGeneral Manager of the Leading partner appoints the Project Manager, as a responsible person for the Project execution and asks the Engineer for the approval. The Project Manager submits to the Engineer names, CVs, and duties of his key personnel.On bases of prepared time schedules, the Technical Manager and Chief Engineers define necessary human resources. The Project Manager shall not remove from the Site any major item of equipment without the approval of the Resident Engineer.4.8.3.The InfrastructureAn infrastructure needed for the Project execution is defined in the Organization of the Works Execution Design. Provision and maintaining of the infrastructure is responsibility of the Project Manager.4.9. SUBCONTRACTORS MANAGEMENTBefore the beginning of the project execution, on bases of lists of assessed subcontractors, Marketing / Quantity, Cost Control and Contracting Department Managers give suggestion to the Project Manager who makes selection of potential subcontractors and submits their names to the Engineer for the approval, with data demonstrating that the suggested subcontractors has references, financial, personnel and physical equipment to satisfy needs of the works execution.The Technical Manager gives the Engineer not less than 28 days notice of the intended date of the commencement of each Subcontractor's work.Subcontractors Works monitoring, control and acceptance, during the execution of the project is responsibility of the Technical Manager in accordance with defined procedure.4.10 QUALITY CONTROL OF MATERIAL AND WORKMANSHIPQuality control of material and workmanship is prescribed in details in the "Quality Control Plan", that is a component part of this plan (Appendix No. 3). QA & QC Manager, in cooperation with the Quality Control Manager prepares and controls realization of the Quality Control Plan.The Employer's stuff has a right to examine, inspect, measure and test the used material and the executed works.4.11. EQUIPMENT CONTROLControl and measuring equipment is in the authority of the Project Manager, and is regularly controlled and calibrated. CONTROL OF ACCIDENT PREVENTION AND ENVIRONMENTAL PROTECTION MEASURESAccident prevention and environmental protection measures are given in details in parts 5.2.3 "Definition of all particular safety arrangements for traffic management including the transportation of equipment and materials" and 5.2.4 "Definition of all particular arrangements for the protection of the environment" of the Tender. Responsibility for preparation of this Proposal is of the Health and Safety Manager that prepares it in cooperation with the Safety at Work Engineers of the Joint Venture members.Compliance with the measures given in the Survey is responsibility of the Technical Manager and Chief Engineers.4.13. EXECUTION OF WORKS4.13.1. Preparation and Submission of Necessary Programs to the EngineerWithin 28 days after commencement of the works, the Technical Manager, in cooperation with Chief Engineers, completes and delivers to the Engineer a detailed time programme that includes. -a sequence of the works execution -a material purchase schedule, -an equipment engagement schedule, -a personnel engagement schedule, -a schedule of subcontractors engagement, -a traffic management plan, -a detailed projection of the cash flow.On the basis of time schedules given in the detailed time programme, weekly and monthly time schedules are prepared. The Technical Manager organizes schedules preparation, and the Quantity & Cost Control Chief Engineer prepares them on bases of data received from the Chief Engineers. Necessary material, personnel and equipment resources are defined and provided on the basis of weekly and monthly time schedules approved by the Engineer.4.13.2. Compliance with Time ScheduleThe works are executed in accordance with accepted Time Schedule defined by The Organization of the Works Execution Project. Organization of Works ExecutionThe Technical Manager is responsible for organization and coordination of the execution of works on the site, in accordance with the time programme.Chief Engineers are responsible for execution of particular group of the works and for providing information for Construction Journal. The Construction Journal is verified by the Project Manager or Technical Manager with his sign and a stamp.The Technical Manager organizes daily work meetings where pages from the book of daily reports for that day are submitted, daily effects and respecting of prescribed activity methods and quality system functioning are analysed, jobs that will be conducted next day are defined, necessary instructions are given to the executors and finally, orders are given for corrective actions in case of disrespecting of prescribed activity methods and nonconforming practice. Beside the Technical Manager, at daily meetings are also present: -Chief Engineers, -Site Agents / Section Chiefs and Chiefs of Equipment, -Subcontractor's Managers.Notes from the work meetings are taken in The Works Meetings Notebook by the Quantity & Cost Control Chief Engineer.The Resident Engineer organizes weekly site meetings where analysis of obligations given by the previous work meeting note are performed, as well as an analysis and acceptance of the report for the last week, distribution, analysis and acceptance of the next week schedule, and necessary instructions are given to the Contractor.Beside the Resident Engineer, the Project Manager, the Technical Manager, the Chief Engineers and Quantity & Cost Control Chief Engineer are also present on meetings. Quantity & Cost Control Chief Engineer is responsible for keeping notes from the meetings.If it was shown that the actual works progress is too slow, or has fallen behind the current programme, up on the Resident Engineer request, the Technical Manager, in cooperation with the Chief Engineers, organizes preparation of a revised programme and supporting report describing the revised methods that will be adopted in order to expedite progress and complete within the time for completion. The revised programme is submitted to the Resident Engineer for the approval.4.14. MONITORING, REVIEW AND ACCEPTANCE OF THE WORKSTime schedules execution monitoring and control of the executed works are the responsibility of the Technical Manager and the Chief Engineers.4.14.1. Monthly Reports include: -Charts and detailed descriptions of the works progress for each item; -Contractor's documentation related to the works execution by items; -Photographs showing the status of manufacture and of the works progress in the site; -Details showing the presence of Contractor's personnel and equipment the on the site including replacements, if any; -Copies of quality assurance documents, test results and certificates of materials; -List of Variations, notices about eventual Employer's claims, or about the Contractor's claims; -Safety statistics including data of any hazardous incidents and activities relating to environmental protection and public relations aspects; and -Comparisons of actual and planned progress, with details of any events or circumstances which may jeopardize the completion in accordance with the Contract, and the corrective actions being (or to be) adopted to overcome delays.After verification by the Project Manager, the Quantity & Cost Chief Engineer submits Reports to the Engineer in six copies, within 7 days after the last day of the period to which it relates.The Project Manager presents monthly reports on the monthly meetings organized by the Resident Engineer.4.14.2. The Works AcceptanceThe Resident Engineer or delegated Deputy Resident Engineer confirms the works acceptance with his verification in the Construction Journal.On bases of data for the Measurement Book prepared by the Site Agents / Section Chiefs, Quantity & Cost Control Chief Engineer prepares Accounting Sheets of the Measurement Book. On bases of the Accounting Sheets, the Resident Engineer verifies the Measurement Book and certifies amounts of the executed works.Measurements for payment shall be carried out by the Quantity, Cost Control and Contracting Department, once a month, on the basis of the works that have been approved by the Resident Engineer. The Project Manager takes actions to eliminate the nonconformity, and informs the Resident Engineer about it.If nonconformity is minor, the Technical Manager, in cooperation with the Chief Engineers defines the way and the dead line for solving of the nonconformity, and inscribes it in the Daily Works Progress Reports Book and the Construction Journal. He organizes the nonconformity's removing by the appropriate Site Agent / Section Chief / Subcontractor's Manager, if the Resident Engineer approved it.If nonconformity is large, the Project Manager stops the Works process. The Technical Manager, in cooperation with the Chief Engineers gives proposition for solution of the nonconformity that assumes that product with the nonconforming quality can be. -accepted with a large repair, -detached for another purpose, -rejected.Proposition is submitted by the Project Manager to the Engineer for the approval, and final decision is distributed to the Technical Manager who organizes, over the Chief Engineers, removing of the nonconformity.After the nonconformity has been removed, the Technical Manager and the Engineer verify the product in the Working Diary. In case of continual disrespect of practice, the Technical Manager and the Chief Engineers identify the cause and responsibility for the appearance, suggest the way for its removal and inform the Project Manager about it.The Project Manager considers the suggestion, verifies it and submits to the Engineer for the approval. Final decision is distributed to the Technical manager who takes actions by the Chief Engineers for nonconformity removal.Description of the nonconformity and performed repairs are recorded as part of the documentation for the Technical Review, to show the right state.The Chief Engineers are obliged for documented monitoring of all inputs considering the costs for nonconformity removal, and to send data about it to the Quantity, Cost Control and Contracting Department.The Quantity, Cost Control and Contracting Department makes an estimation of the costs and informs by a document the Project Manager about it.4.16.EMPLOYER'S TAKING-OVER4.16.1. During the technical acceptance, the Project Manager delivers to the Employer all documentation prescribed by the Contract.If the Commission has approved issue of the Taking-Over Certificate to the Contractor, the Engineer issues the Taking-over Certificate on a prescribed way. If the application has been rejected, the Project Manager organizes, via the Technical Manager and the Chief Engineers performing of the necessary works, specified in the Commission's Report, and repeats the procedure.The Engineer may issue the Taking-Over Certificate for Parts of the Works, in a prescribed way. 4.17. CONTRACT DOCUMENTATION CHANGES MANAGEMENTIf, during the Project execution, a need for contract documentation changes arises, one Side that approves the Contract proposes a request for preparation of an Annex to the Contract. Each change can include: -change of amount for any works item included in the Contract, -change of quality and other characteristics for any works item, -changes in levels, items and/or dimensions of any part of the works, -omission of some parts, except when those works are done by someone else, -any additional work, equipment, material or services necessary for the regular works, including investigating and similar works and additional tests, -changes in sequence or time sequence of the works execution.Contract documentation change can be proposed to the Project Manager by: -the Technical Manager and the Chief Engineers responsible for construction organization, -the Quantity & Cost Control Chief Engineer, responsible for the Contract realization monitoring; -Subcontractor's Managers.On bases of their propositions, the Project Manager sends a request for the contract documentation change to the Quantity, Cost Control and Contracting Department of the Leading partner, who's taking further action, in accordance with clauses of the Contract.
