(EASD) ; June 2012
Criteria for diagnosis
• A1C > 6.5% หร6อ• Fasting plasma glucose > 126 mg/dl (no
calories intake at least 8 hrs) หร6อ• 2-hr plasma glucose > 200 mg/dl during OGTT
หร6อ• Random plasma glucose > 200 with classic
symptoms of hyperglycemia or hyperglycemic crisis
• ควรต่รวจยุ6นยุนผู้ลอ*กคร-งดวยุวธี*เด*ยุวกน
Prediabetes • Individuals with impair fasting glucose (IFG) and/or impair
glucose tolerance test (IGT)• IFG = FPG 100-125 mg/dl หร6อ• IGT = 2hr pg OGTT 140-199 mg/dA1c หร6อ• A1c = 5.7-6.4 %• Relatively high risk for the future development of diabetes• Associated with
– Abdominal obesity– Hypertriglyceridemia– Low HDL– Hypertension
Testing for diabetes in asymptomatic patients
• Asymptomatic patients : – Overweight or obese (BMI > 25 kg/m2)and have
one or more additional risk factor for diabetes– Without risk factor : begin at age 45 years
• ถาปกต่ ใหต่รวจซ้ำ%-าทัก 3 ป&• ต่รวจโดยุใช A1C หร6อ FPG หร6อ 75 g OGTT
Additional risk factors
• Physical inactivity• First degree relateive with diabetes• High risk race (eg. African American, Latino,
Native American, Asian american, Pacific Islander)
• Women who delivered a baby weighting > 9 lb or GDM
• Hypertension(>140/90 or on therapy for hypertension)
Additional risk factors
• HDL level < 35 mg/dl and/or triglyceride > 250 mg/dl
• Polycystic ovarian syndrome• A1C > 5.7 %, IGT or IFG• Clinical associated with insulin resistance
(severe obesity, acanthosis nigricans)• History of CVD
Screening for type 1 DM
• Evidence from several studies suggest measurement of islet autoantibodies in relatives
• Not currently recommend
Prevention of type 2 Diabetes
• IGT, IFG, A1C 5.7-6.4% : weight loss of 7% of body weight, increase physical activity at least 150 min/week of moderate activity
• Metformin for prevention of type 2 DM : BMI>35 kg/m2, age<60, women with prior GDM
• Annual monitoring
Metformin *
• Metformin less effictive than lifestyle modification
• Cost-saving over a 10-year period• Recommended for very high risk individual :
history of GDM, very obese, and/or severe or progressive hyperglycemia
Glycemic control
• Glucose monitoring– Self-monitoring of blood glucose: effective therapy– Continuous glucose monitoring (CGM) : • age > 25 year, type 1 DM
• A1C – At least 2 times a year in who are meeting
treatment goal (and stable glycemic control)– Quarterly in whose not meeting the glycemic goal
A1C limitation
• Conditions that affect erythrocyte turnover (hemolysis, blood loss) and hemoglobin varients
• Not provide a measure of glycemic variability or hypoglycemia
Glycemic recommendation
• A1C < 7.0%*• Preprandial capillary plasma glucose : 70-130
mg/dL*• Peak postprandial capillary plasma glucose <
180 mg/dL*• More or less stringent glycemic goal may be
appropriate for individual patients
Glycemic recommendation
• More stringent A1C goal (<6.5%) for selected individual patients , without significant hypoglycemia or other adverse effects of treatment

Method of Development of Evidence Base Clinical Practice Guidelines
• We recommend : strong recommendation– More good than harm
• We suggest : less strong recommendation– More careful consideration of the person’s
circumstances, value, and preference to determine the best course of action
Preconception care of women with diabetes
• Lower rate of congenital anomalies and spontaneous abortion
• Multidisciplinary team – Glycemic control– Smoking cessation
Preconception glycemic control
• Blood sugar and HbA1C level close to normal• Anomaly risk of 3.9%-5.0% with
periconception HbA1C of up to 10.4%• Risk then climbing to 10.9% if HbA1C 10.4% or
higher
Insulin therapy
• We suggest– Change to insulin regimen– Treated with rapid acting insulin analog therapy
(Lispro, Aspart)– Diabetes women using the long acting insulin
analogs insulin Detemir or Glargine preconceptionally may continue with this therapy before and during pregnancy
Insulin therapy
• Rapid acting insulin analog less likely to cause hypoglycemia– Lispro and Aspart : approved by U.S. Food and
Drug Administration (FDA)– Glulisine : not approved by FDA
• Long acting insulin : lower rate of nocturnal hypoglycemia (compared with NPH)– Insulin Detemir : approved by FDA during
pregnancy.– Insulin Glargine : not approved by FDA
Folic acid supplementation
• 3 months before withdrawing contraceptive• Reduce risk of neural tube defects• Suggest a daily dose of 5 mg
Ocular care
• We recommended– All women : ocular assessment– If retinopathy : worsening during pregnancy– Deferring conception until the retinopathy has
been treated – Women with retinopathy be seen by eye specialist
every trimester, then within 3 months of delivering
Ocular care
• We suggest– Diabetes women not known to have retinopathy :
ocular assessment soon after conception and then periodically as indication
Renal function
• We suggest : all women– Urine albumin to creatinine ratio– Serum creatinine– Estimated GFR– Monitored renal function regularly during
pregnancy (renal dysfunction)
Management of hypertension
• We recommend – BP control < 130/80 mmHg– Discontinue ACEI or ARB before withdrawing
contraceptive measures or trying to conceive– ACEI or ARB should be withdrawn immediately
upon the confirmation of pregnancy
Management of hypertension
• Safe drugs for treating hypertension– Methyldopa– Labetalol– Diltiazem– Clonidine– Prazosin
Management of dyslipidemia
• We recommend against : – Statin : X– Fibrate : C– Niacin : C
• We suggest bile acid binding resin
Gestational diabetes
• Current definition : any degree of glucose intolerance with onset or first definition during pregnancy
• Screening using the protocol and threshold value of the International Association of Diabetes and Pregnancy Study Group (IADPSG) and American Diabetes Association (ADA)
Gestational diabetes
• Testing for overt diabetes in early pregnancy– We recommend universal testing : FPG, HbA1C,
random plasma glucose (before 13 weeks gestation, or as soon as possible thereafter)
– Second test to confirm the diagnosis
Gestational diabetes
• Testing for gestational diabetes – At 24-28 weeks of gestation– In pregnant women not previously identified– 2 hour, 75 g OGTT– International Association of Diabetes and
Pregnancy Study Group (IADPSG) criteria– Hyperglycemic and Adverse Pregnancy Outcome
study (HAPO Study)
Diagnostic criteria
Diagnosis Fasting plasma glucose (mg/dL)
Random plasma glucose (mg/dL)
HbA1C (%)
Overt diabetes
> 126 > 200 > 6.5
Gestational diabetes
92-125 Not applicable
Not applicable
2 hour 75 g OGTT • At 24-28 weeks gestation
Diagnosis Fasting Plasma Glucose (mg/dL)
1-h value (mg/dL)
2-h value (mg/dL)
Overt diabetes
> 126 Not applicable
> 200
Gestational diabetes
92-125 > 180 150-199
Management of elevated blood glucose
• Recommend – Target blood glucose level as close to normal as
possible– Initial treatment : medical nutrition therapy and
daily moderate exercise for > 30 min– Pharmacological therapy : if lifestyle therapy is
insufficient
Gestational diabetes
• Postpartum care– Recommend • 24-72 hour after delivery : fasting plasma glucose or
fasting self monitoring blood glucose• 6-12 weeks after delivery : 2 hour, 75 g OGTT• All women : lifestyle
Gestational diabetes
• Postpartum care– Suggest : • blood glucose lowering agent should be discontinue
immediately after delivery
Glycemic targets
Target value (mg/dL)
Preprandial blood glucose < 95
1 h after the start of a meal < 140
2 h after the start of a meal < 120
Pharmacological therapy
• Suggest : – Long acting insulin analog detemir may be
initiated– Insulin glargine may continue during pregnancy– Lispro and Aspart : be used in preference to
regular insulin
Long acting insulin
• Detemir : theoretical advantage over glargine• Glargine : higher affinity for the IGF 1
receptor increase mitogenic activity• Glargine : unlikely cross placenta and animal
studies not show Glargine to be embryo toxic• Insulin Detemir is now approved (Category B)
by the FDA
Rapid acting insulin
• Compared with regular insulin• Greater lifestyle flexibility, improve quality of
life, better postprandial blood glucose control and HbA1C reduction
• Similar rate of prematurity, cesarean delivery, worsening of retinopathy
• Glulisine : not FDA approved
Noninsulin antihyperglycemic agent
• Suggest– Glibenclamide : alternate to insulin therapy• Except :
– GDM before 25 weeks– FPG > 110 mg/dL
– Metformin : only who refuse or can not use insulin or glibenclamide ; not use in first trimester
Glibenclamide
• Undetected umbilical cord glibenclamide concentration
• Effective in controlling blood glucose• Not FDA approved
Metformin
• No difference in maternal glycemic control• No increase risk of neonatal abnormality• Category B• Cross freely through the placenta• Long term follow up : not available• Increase rate of preterm birth
Lactation
– Recommend breast feeding– Continue metformin and glipizide– Concentration of metformin in breast milk are low– Glibenclamide : not detect in breast milk
