From our early beginnings in 1903 as the Henry Boyle home to our extended St. Francis Home campus of today, many dramatic changes have occurred. What has not changed is our commitment to provide you with a caring, Christian environment. Under the sponsorship of the Congregation of Sisters of St. Agnes, we strive to serve each resident with respect, dignity and a sense of individual worth.

Our continuum of care offers you three options tailored to fit your needs:

• Independent Living. St. Clare Terrace offers a safe, secure environment where you can enjoy a fulfilling lifestyle through independent community living.

• Assisted Living. St. Francis Terrace offers limited care while still allowing you a strong sense of independence.

• Skilled nursing care. St. Francis Home is a 107-bed skilled care nursing facility specializing in long-term care and offering a special household designed especially for residents with Alzheimer’s disease.

Assisted LivingSince opening in 1998, St. Francis Terrace has offered residents assistance with the activities of daily living in a comfortable, supportive home to make life more enjoyable. Our uncommon blend of high-quality services and premier housing gives you the privacy, dignity and independence you want, and the personal care and assistance you may need.

Skilled Nursing CareSt. Francis Home is the heart of our care campus. This 107-bed skilled care nursing facility specializes in long-term care for the elderly. We provide the latest in nursing home care, create a home-like atmosphere for our residents and have highly-trained staff in Alzheimer’s disease and dementia.

The home is Medicare and Medicaid certified. There are a number of activities going on throughout the month to engage our residents.

Connected to Agnesian HealthCare’s St. Agnes Hospital and Fond du Lac Regional Clinic, the St. Francis Home campus is ideally suited to meet the growing healthcare needs of its elderly population.

Independent LivingAt St. Clare Terrace, you can make the most of your retirement in the privacy and security of your spacious one or two bedroom apartment. You furnish your apartment the way you like it, because you are, after all, creating your place to call home.

You may choose to eat daily meals restaurant-style in our dining room, or to cook in your own kitchen. There are also plenty of group activities in which to participate.
