Definity/UHC Consumer-Driven Health (CDH) Plan – How does it work?
CACI funds a Health Reimbursement Account (HRA) Amount funded is based on covered family size You decide how to spend or save these dollars Unused HRA $ rolls over to next year
After the HRA is used, and your deductible has been met, covered expenses are paid at 90%: Save money by using network doctors, hospitals, labs, etc. All pharmacies are treated as “in-network” Deductible, co-insurance, out-of-pocket maximum

Medical Plan EnhancementsPreventive Care and Health Screenings Covered at

The CIGNA and Definity/UHC medical plans cover recommended mammograms, Pap smears, and prostate cancer tests, as well as other screenings at 100%* (in- network only for plans with networks).
If you are in the CIGNA Network or OAP Plan, you may need to tell your provider that preventive care is not subject to a copay.
New Hearing Aid Benefit
Participants in the CIGNA and Definity/UHC CDH Plan will have a new hearing aid benefit. Beginning on July 1, 2009, hearing aids are a covered expense, paid at 80%, up to $5,000 every three years.
* The CIGNA OAP Plan caps the Preventive Care Benefit at $1,000 per calendar year.

If you suffer from a chronic medical condition, such as high blood pressure, asthma, or diabetes, CACI offers you a special program to help you manage your illness. Through a partnership with Optimal Health, you have access to a registered nurse health coach and a team of health experts who can help you stay on track with your treatments.
This program is voluntary, confidential, and free – no one at CACI will know if you participate. You may be eligible to participate in this program if you suffer from one or more of the following conditions:
Asthma Congestive heart failure
Cancer Chronic obstructive pulmonary disease
Back pain Diabetes
Coronary artery disease End-stage renal disease

New Prescription Drug AdministratorBeginning on July 1, 2009, prescription drug benefits for participants in CACI’s medical options provided through CIGNA (except the CIGNA International Plan) and Definity/UHC will be administered through Medco Health Solutions, Inc. (Medco).
Prescription Drug Benefits — CIGNA Plans
When you enroll in any of the domestic CIGNA medical plans, your retail and mail-order prescription drug coverage is administered through Medco. See the chart below for the amount you’ll pay for prescriptions, based on the three tiers.

Prescription Drug Benefits — Definity/UHC CDH Plan
For the Definity/UHC CDH plan, prescription drug benefits are currently administered through Medco and this will continue into the future, with some changes, including a new prescription drug card. You pay for your prescriptions in full up to the plan deductible (with the help of your HRA); then, once you have satisfied the deductible, you will pay 10% for prescription drugs through retail pharmacies or Medco By Mail.

NEW Medical and Prescription Drug ID Cards
Starting on July 1, 2009, participants in the CIGNA Network, OAP, and Indemnity Plans, as well as the Definity/UHC CDH Plan will have two separate ID cards for medical and prescription drug benefits.
Please show your new prescription drug ID card to your pharmacist to ensure you receive the correct benefits. If you do not receive a new medical and/or prescription drug ID card by July 1, contact the appropriate vendor:

Life/AD&D Benefits are 100% Paid by CACI CACI Group Term Life (GTL) Benefits
This policy pays one times your annual salary to a maximum benefit of $300,000. Basic Group Life is the default coverage. Employees may elect to reduce their basic group term life coverage to $50,000 (if employees earn more than $50,000 per year) in order to avoid income taxes levied on amounts of CACI-paid insurance in excess of $50,000. (If employees reduce their life coverage they will receive a small premium credit.)
CACI AD&D Benefits CACI automatically provides AD&D coverage at three
times your annual salary up to $500,000, or pays benefits for certain injuries. Loss must be the direct result of an accident. If the loss is the result of war or an act of war, the policy pays six times your annual salary to a maximum benefit of $500,000.

Supplemental Life Insurance: Employees have the option to purchase group rated portable term insurance for themselves and their dependents. The maximum amount of coverage employees may purchase is as follows: The lesser of 5 times employees’ annual salary or $1,000,000
for employee; The lesser of 5 times employees’ annual salary or $250,000
for employees’ spouse; $20,000 each for employees’ dependent children.

New! Voluntary Critical IllnessCritical IllnessThe new Critical Illness plan provides you and your family with additional financial relief if diagnosed with a critical illness such as:

Certain limitations*, exclusions, and waiting periods apply.
You may elect coverage for yourself in the amount of $10,000 or $20,000. Coverage is portable after 6 months of coverage.
Rates are locked in at time of election and will not increase as you get older.
Coverage for your spouse is available in the amounts of $5,000 or $10,000; however, your spouse’s coverage cannot be more than 50% of your coverage. If you elect coverage, your eligible children are automatically covered for 10% of your coverage at no additional cost to you.
If you enroll in this plan, you and your covered spouse are eligible for a $50 benefit each year for health screenings, such as mammography, colonoscopy, chest x-rays, stress testing, and certain blood tests. (Dependent children are not eligible for this benefit.)
