EMPLOYER CONTRIBUTION PLAN

To provide the best benefits at the best value,Abacus has developed a program where youincrease your group’s health benefits deductiblesand co-pays and free up dollars to help pay forthe Abacus Medical Support Insurance Planavailable through Madison National LifeInsurance Company. It provides more first-dollarbenefits at a lesser cost – a win for both you andyour employees.

Or, you may choose to leave your current benefitplans in place and still provide the Abacus Medical Support Insurance Plan as an addedbenefit for your employees.

VOLUNTARY BENEFITS PLAN

As the cost of health insurance continues toescalate, you may think your only alternative tohigher premiums is shifting some of the addedcosts to your employees – through increaseddeductibles and plan co-pays. The AbacusMedical Support Insurance Plan providesemployees with the option to purchase avoluntary, supplemental plan that may helpreduce the out-of-pocket hospital deductibleswith a First Day in Hospital benefit.

No one plans to get sick or injured –but your employees can be prepared if ithappens to them.

Plan highlights for employees and employers alike include:

• Guaranteed Issue

• Employer contribution plan: two enrolled employees;no pre-existing condition limitation

• Minimum voluntary group size: two enrolledemployees; 12/12 waiting period on pre-existingconditions for Critical Care

• Composite rates: one rate regardless of age or gender

• Easy claims filing: telephonic claims initiation

• Benefit payment: pays directly to the Insured inaddition to any other plans

• No physician or hospital restrictions: you chooseyour own provider

First Hospital ConfinementPays the benefit amount for an insured’s first hospital confinement up to 3 days at $1500 perday. Limited to the First Hospital Confinement each Calendar Year for each Insured and willnot exceed:

Day-1 - $1500 Day 2 - $1500 Day 3 - $1500

Critical CareMakes a one-time payment of the benefit amount if one of the following specified healthevents occurs for the first time in the insured person’s lifetime while the Rider is in force:Heart Attack, Stroke, Life Threatening Internal Cancer, End Stage Renal Failure, CoronaryArtery Bypass, Major Organ, or Permanent Paralysis. Subject to a 30-day waiting period. Pre-Existing Condition limitations may apply.- Requires diagnosis by a Doctor. Once the maximum benefit has been paid, coverage underthis Rider terminates for the insured person.

Accident ExpensePays the lesser of the benefit amount or actual expenses for covered charges incurred by aninsured person as a result of a covered Accidental Bodily Injury which occurs whilecoverage is inforce under this Rider.- Initial charges must be incurred within 90 days of the covered Injury and while coverage iscontinuously inforce.

Summary / OverviewThrough your employer, you are presently covered under your group medical plan, with benefits that are mandated through the “PPACA”Patient Protection and Affordable Care Act, which was passed into law by Congress. Based on your specific plan benefits, your coveragehas deductibles and co-pays that may require you to pay out of pocket expenses for as much as $6,350.00 on an individual and $12,750.00 onthe family. You can’t stop medical cost from rising! You can’t prevent gaps in coverage from occurring! However, you can help manage the cost youwill incur through deductibles and co-pays by purchasing the Abacus Affordable Supplemental Plan.
