Occiput posterior position • พบร้อยละ 15-20 ของการคลอด • มักพบใน pelvis ลักษณะ anthropoid และ

android (Calwell-Moloy classification)

• พบ ROP มากกว่า LOP

กลไกการคลอด • anterior rotation หมุน 135 องศา พบส่วนใหญ่

• posterior rotation หมุน 45 องศา พบร้อยละ 5-7

• Posterior rotation ท าให้เกิด persistent occiput posterior

• การดูแลการคลอด – จุดหมุนแรก คือ หน้าผาก

– จุดหมุนที่สอง คือ ท้ายทอย

Occiput posterior position

• พบร้อยละ 5-10 ของการคลอด • มักพบในการคลอดที่มีการหดรัดตัวมดลูกที่ไม่ดีพอ/แรงเบ่งคลอดน้อย

• หรือพบใน pelvis ลักษณะ pletypelloid และ android (Calwell-Moloy classification)

ROT

การดูแลการคลอด • คอยให้เกิดการหมุนเอง

• เพิ่มแรง – ให้ยากระตุ้นการหดรดัตัวของมดลูก

– ใช้เครื่องดูดสญุญากาศ

• ใช้การช่วยให้เกิดการหมุน

– การใชม้ือหมุน/การใชค้มีช่วยหมุน

Occiput transverse position

Breech presentation • พบร้อยละ 3 ของการคลอด

• เพิ่มอัตราการตายปริก าเนิด 2-3 เท่า

• เพิ่มโอกาสเกดิสายสะดือย้อย (ร้อยละ 10 ใน footling breech,ร้อยละ 5 ในcomplete breech)

• การดูแลการคลอดยาก/ภาวะแทรกซ้อนในแม่และเด็กสูงขึ้น

• เพิ่มการผ่าตัดคลอด

สาเหตุของ Breech presentation • การคลอดกอ่นก าหนด

• การตั้งครรภ์แฝด

• ความพิการของทารก เช่น hydrocephalus

• ความผิดปกติของรก เช่น รกเกาะต่ า

• ความผิดปกติของน้ าคร่ า คือ มากหรือน้อยเกนิไป

• ความผิดปกต/ิเนื้องอกในมดลูกหรืออุ้งเชิงกราน

ชนิดของ Breech presentation • Frank breech พบ ร้อยละ 65-70

• Incomplete breech พบ ร้อยละ 25-30

• Complete breech พบ ร้อยละ 5

การวินิจฉัย Breech presentation • การตรวจหน้าท้อง

– พบ ballotement บริเวณยอดมดลูก

– ฟังหัวใจทารกได้ชัดเจนสงูกว่าสะดือ

• การตรวจภายใน – คล าได้รูทวารหนักและ ischial tuberosities ในแนวเส้นตรง

• การใช้รังสีวินิจฉัย • การใช้คลื่นเสียงความถี่สูง

กลไกการคลอด Breech presentation

• engagement

• descent

• internal rotation

• lateral flexion

• external rotation

• birth;breech,body,head

• พิจารณาข้อห้ามในการคลอดทางช่องคลอดก่อน/หรืออาจพิจารณาข้อบ่งชี้ในการผา่ตัดคลอด – large fetus

– hyperextension head พบรอ้ยละ 5 – footling presentation

– preterm

– severe fetal growth restriction

Breech presentation Recommendation for delivery

• pelvic contraction /CPD

• uterine dysfunction

• poor maternal bearing or maternal disease; PIH

• previous perinatal death/ bad obstetric history

• request for sterilization

Abnormal presentation

Face presentation • พบ 1ใน 500 (ร้อยละ 0.2) ของการคลอด

• พบ LMA มากกว่า RMA (persistent mentoposterior ต้องผ่าตัดคลอด)

• อัตราการตายทารกปริก าเนิดสูงขึ้น

• ภาวะแทรกซ้อน/การฉีกขาดช่องทางคลอดสูงขึ้น

Face presentation

Brow presentation • พบประมาณร้อยละ 0.1

• หากไม่มีการเปลี่ยนแปลงท่าจะคลอดผ่านลง inlet ไม่ได(้ occipitomental diameter ยาว 13.5 เซนติเมตร)

Transverse lie • อุบัติการณ์พบน้อยมาก • หากเกิดการคลอดและปล่อยไว้อาจเกิด obstructed

labor และท าให้มดลูกแตกได ้• หากคลอดได้เองจะมีการพับของล าตัวเป็นรูปตัว V เรียกconduplicato corpare

Tranverse lie

Compound presentation • คือการที่ทารกมีส่วนของแขนหรือขายื่นลงมาพรอ้มกับส่วนน าของทารก

• พบอุบัตกิารณ์ประมาณ 1 ใน 600 (ร้อยละ 0.1-0.2) • โอกาสเกิดสายสะดือย้อยสูงขึ้น • ส่วนใหญ่มักไม่รบกวนการคลอด
