Salt Lake City Accessibility Council Membership Application Information Purpose: The Salt Lake City Accessibility Council (“The Council”) is a citizen advisory group formed to help the city understand and address issues surrounding disability. The Council provides an ongoing opportunity to advise the Mayor and the City ADA Coordinator on issues related to facilities, services, equipment, plans and programs to ensure non-discrimination for all citizens. Membership Representation: The Council is comprised of at least fifteen (15) members appointed by the Mayor. The Council seeks advice from representatives of all disability and age groups, citizens of Salt Lake City, individuals who work in Salt Lake City, and representatives of agencies and entities that provide services or support to individuals with disabilities within Salt Lake City. The Council strives to represent the full range of disability types including but not limited to: • Blind or visually impaired • Chronic conditions • Cognitive or learning disabilities • Combinations of multiple disabling conditions • Deaf or Hard of Hearing • Hidden disabilities • Mental illness • Mobility impairments or physical disabilities • Seniors (65 years & older) Individuals representing themselves must be residents of Salt Lake City or work in Salt Lake City. Salt Lake City Accessibility CouncilMembership Application Information

Purpose:

The Salt Lake City Accessibility Council (“The Council”) is a citizen advisory group formed to help the city understand and address issues surrounding disability. The Council provides an ongoing opportunity to advise the Mayor and the City ADA Coordinator on issues related to facilities, services, equipment, plans and programs to ensure non-discrimination for all citizens.

Membership Representation:

The Council is comprised of at least fifteen (15) members appointed by the Mayor. The Council seeks advice from representatives of all disability and age groups, citizens of Salt Lake City, individuals who work in Salt Lake City, and representatives of agencies and entities that provide services or support to individuals with disabilities within Salt Lake City. The Council strives to represent the full range of disability types including but not limited to:

Individuals representing themselves must be residents of Salt Lake City or work in Salt Lake City.

Membership Term:

Service on The Council will usually begin in January and follow the calendar year. Membership terms run for (3) three consecutive years. You may re-apply for additional (3) three year terms.

Meetings:

Council meetings are usually held on the fourth Tuesday of each month from 3:00 to 4:30 PM. at the Salt Lake City and County Building. Each member is required to serve on a working subcommittee that meets at least once a month. Attendance and participation at these (2) two monthly meetings are very important. An attendance policy is strictly followed. Any member with four (4) absences from Council meetings in one calendar year shall forfeit membership on The Council.

How Do I Apply for Council Membership?

Interested persons should complete and submit the attached application to the ADA Coordinator. If you have questions or need assistance completing the application, please contact the ADA Coordinator.

Applications are accepted throughout the year but to be considered during the end of year annual membership process, applications are due by November 30. If you are not selected to serve at the start of a membership year, your application will be held in the “applicant pool” and considered with future vacancies.
