AccidentAdvance offers employers maximum flexibility

Available as either off-the-job only or 24-hour coverage, AccidentAdvance is not your ordinary accident insur-ance. This group voluntary policy is designed with three base coverage options, which employers can offer in varying amounts to suit the needs of their workforce: accident emergency treatment, follow-up visits and physi-cal therapy, and initial accident hospitalization. Optional riders are listed below, and individual and family cover-age is available. Refer to the Producer Guide, CHI01A for additional details.

Employers choose base coverage levels and optional riders

Employers customize their plan design by selecting off-the-job or 24-hour coverage, then by offering from 1 to 12 units of coverage for each of the base coverage options:

• Accident Emergency Treatment: physician or hospital treatment, including X-rays and scans, following an accident

• Follow-up Visits and Physical Therapy: coverage for follow-up treatment beginning within a specified period of time after the accident

• Initial Accident Hospitalization: initial admission coverage within 96 hours of the accident• Employers also select from the following menu of optional riders (availability varies by state):• Accidental Death and Dismemberment: for death resulting from and occurring within 90 days of the accident • Accident Hospital and ICU Income Rider: for hospital confinement resulting from and beginning within 30 days of

the accident; pays for up to a year for each accident• Expanded Benefits Rider: payments available per a list of scheduled injuries and treatments• Wellness Benefit Rider: covers annual health screening according to a schedule• Disability Income Riders: Available for accident-only, sickness-only, and spouse off-the-job accident-only

Keeping Things Simple: The Benefits of Web Enrollment

Transamerica has developed a unique web based enrollment service that can greatly reduce the amount of time to enroll each employee. Because AccidentAdvance is available on a group platform, we can streamline enrollment for employ-ees, for employers, and for you, integrating with other HR systems already in place.

Underwriting and Participation Guidelines

Minimum monthly premium is $10, or $2.31 per week excluding disability income riders.

At least 5 insured employees are required to establish and maintain an employer group, subject to state minimums. Other group types may require higher participation. Coverage is offered on a Guaranteed Issue basis as long as the eligi-bility requirements are met.

Who can participate:• Employees and spouses ages 18 and 64. • Eligible children from birth through age 25.

Coverage is portable, subject to a change in premium rate, should employees leave their company.
