BIOACTIVE MATERIALS: A RESTORATIVE REVOLUTION WITH SO MANY NEW MATERIALS AVAILABLE, it can be difficult to decide which one to choose. Many of them have wonderful physical properties and can “fill the hole” in the tooth. However, when lecturing around the country, I tell my audiences that I want materials that actively protect the tooth and enhance therestorative process. For this reason, bioactive materials are the materials of choice in my practice.

I explain to my patients how they benefit from a proactive approach to dental care using bioactive materials. These materials behave favorably in the moist oral environment, neutraliSe acidic conditions that contribute to dental caries production, and help maximiSe the potential for remineraliSation of dentition.

ACTIVA™ BioACTIVE products by Pulpdent are the first dental resin materials with a shock-absorbing, bioactive ionic resin matrix plus reactive ionomer glass fillers that mimic the physical and chemical operties of natural teeth. Interestingly, The US Food & Drug Administration has allowed the claim that ACTIVA™ BioACTIVE products contain a bioactive resin matrix and bioactive fillers, a claim that other companies cannot make.

ACTIVA™ materials are reported to actively participate in the cycles of ionic exchange that regulate the natural chemistry of our teeth and saliva to contribute to the maintenance of tooth structure and oral health. The strength, esthetics and physical properties of ACTIVA are comparable to composites, and it is reported that these materials deliver more fluoride release than glass ionomers1 combining the best attributes of both materials without compromising either one. ACTIVA™ has been shown to chemically bond and seal teeth against bacterial leakage2,3 and to continuously release and recharge of calcium, phosphate and fluoride ions to provide longterm prevention benefits.

Deflection at break (measures toughness and the ability of a strong, hard material to absorb stress without racturing) of ACTIVA is 2-3 times greater than composites and 5-10 times greater than GIs and RMGIs.4,5 And unlike traditional materials that are hydrophobic, repel water, and are designed to be passive, ACTIVA™ is moisture friendly and plays a dynamic role in the mouth. Only moisture friendly materials that are partly water-based or have phases or zones with significant water content can react to changes in the ambient conditions and are capable of this dynamic behavior.6

ACTIVA™ reacts to the continuous pH changes in the oral environment to help fortify and recharge the ionic properties of saliva, teeth and the material itself.1,7,9 For this reason, ACTIVA™ is considered a “smart” material. It contains water, yet the material has extremely low solubility.8 The ionic resin matrix facilitates the diffusion of calcium, phosphate and fluoride ions while still maintaining the excellent physical properties associated with resins and composites.

ACTIVA™ BioACTIVE products contain no Bisphenol A, no Bis-GMA and no BPA derivatives. The ACTIVA™ materials come in two-paste, automix systems with three setting mechanisms: light cure, self-cure resin chemistry, and self-cure glass ionomer reaction.

The recent introduction of fluoride foams has been an extremely successful solution to this problem and as a result, these foams are rapidly gaining popularity amongst dental professionals.

One of the primary reasons patients are more receptive to FLOAM is because the consistency is much less dense than a typical gel formula. When using a gel, patients often feel an unpleasant nauseous sensation when the substance touches the palate. FLOAM has a much lighter, mousse-like texture with bubbles that break down to form a fluoride rich liquid that spreads inter-dentally for optimum surface coverage.

Flavour also greatly impacts a patient’s tolerance to fluoride treatments, which is why FLOAM comes in a natural ‘Strawberry Sensation’ flavour, or a ‘Bubble Gum’ flavour for the younger patients.

GERMIPHENE GEL 7 MINT FLUORIDEA gentle 2% sodium fluoride gel ideal for use with any patient.Gel 7, with a neutral pH, provides an easy four minute fluoride treatment for the dental professional. It may be used either with a tray or “paint on” method. The neutral pH prevents irritation or sensitivity during treatments.

The acid-free gel is indicated for patients intolerant of acidic fluorides, such as those with reduced salivary flow due to chemotherapy, radiation or certain medications causing xerostomia.

The non-irritating formulation is safe for patients with soft tissue irritation, root exposure and titanium implants. Gel 7 will not dull or etch composite restorations, porcelain veneers, glass filled resin sealants and restorations sealed with glass ionomer cements.

Gel 7 is an ideal fluoride therapy for adults as it aids in the protection of recurrent dental decay as well as protects crown margins, preserving crown and bridge work.

SureStop is a sterile, soluble cellulose-based dressing that transforms quickly into a clear, viscous gel upon contact with blood or fluids. SureStop is indicated for the topical management of wounds and to temporarily control bleeding. Appropriate dental indications include extraction sites, dental trauma, biopsy sites, periodontal surgery, and oral surgery. SureStop comes in pre-cut 1.9 cm squares. Each box contains 20 pads, individually sealed in blister packs. SureStop was evaluated by the Dental advisor with 28 consultants in 240 cases This haemostatic dressing received a 93% clinical rating. Product FeaturesConsultants used SureStop primarily after extractions but also in conjunction with other oral surgeries and traumas. It is very effective in achieving haemostasis. The pre-cut squares and individual packaging contribute to its user friendliness. Upon contact with moisture, the woven cellulose becomes a sticky gel, sealing the wound. The biocompatible nature of the cellulose was reported to be appealing to dentists and their patients. SureStop has two unique features – it is bacteriostatic and is very effective in stopping bleeding on patients using anticoagulants.
