    NH Program Activities/Description

    The Action Learning Collaborative team will begin to

    identify the communication that is needed by families of

    Latino CYSHCN to avoid lapses in coverage as major

    changes are made to the NH Medicaid and CHIP programs. The Team will create the materials needed (taking into

    effect factors of health literacy), arrange for appropriate

    translation and plan dissemination of the materials.

    Community forums will be developed and held as well to

    meet families where they are at.

    Barriers to ease of use of services for Latino families

    Nationally, 21.5% of children overall have public health insurance coveragealmost twice that percentage, 38.3%, of children of Hispanic ethnicity are

    covered by public insurance.

    An even higher percentage of NH CSHCN of Hispanic ethnicity rely on public

    insurance for their healthcare needs: 43.9% have only public insurance and an

    additional 20.9% have public and private health insurance. Reinforcing the needfor ensuring and maintaining uninterrupted access to public insurance.

    In the area served by CHS, the percentage of families with children under 18

    who live below poverty level has doubled in 15 years. In 2010, 717 parents had

    a country of origin other than the United States. Spanish speaking families

    comprise the largest group (Mexico, Central America and the Caribbean).Currently, it is anticipated that 102 CSHCN of Latino families are directly

    connected to Child Health Services.

    Evaluation methodsThe Action Learning Collaborative Team will evaluate success as follows:Existing tools facilitating ease of use for NH families of Latino CSHCN will

    be detailed and inventoried.

    New tools that are developed will be added to the inventory.

    Participation in the Team meetings will be tracked and recorded.

    A spectrum of resource materials will be developed and disseminated the distribution of these materials will be recorded and reported.

    An evaluation tool will be developed and all Community Forum

    participants will be asked to rate the impact of the forums and their level of

    understanding of services based on participation in the Forums.

    When surveyed community providers and other family advocates willresult in their reporting

    an increased level of confidence and comfort in answering families

    questions about the changes to Medicaid and CHIP.
