The Family Stability Initiative began in 2011 to help families at risk of homelessness with school aged children to

become self-sufficient. We help families in difficult financial situations to identify and address the problems they face.

We provide six months of intensive case management to help them discover the root cause of their economic crisis,

improve their child’s school attendance and performance, and develop a plan to achieve long-term goals.

SUPPORT SERVICES INCLUDE:

Financial assistance to help families remain in their homes or find affordable housing

Career coaching

Money management and budgeting skills

Referrals to community resources

HOW TO HELP

Child care volunteers are needed while parents attend trainings and job interviews

PROGRAM CRITERIA

Reside in Orange County with at least one school aged child (K-12)

Complete initial family assessment

Meet weekly with case manager

Attend monthly financial education workshops (food and

child care provided)

Follow individualized case plan and family budget

it wasn’t for Shepherd’s House.” -Isabel Gomez, former resident

Shepherd’s House provides safe housing, child care, and a circle of support to homeless single mothers. The

program provides women the opportunity to gain financial stability, parenting support, job training, and life skills to

establish a solid foundation on which to build their future.

Shepherd’s House includes seven duplex apartments that accommodate up to fourteen mothers with children age ten

or younger for six months. The mothers receive case management and life skills training at the nearby

Shepherd’s Common facility.

SUPPORT SERVICES INCLUDE

Safe, secure, fully furnished homes in a positive and

supportive environment

Daily case management

Individual and group counseling

PROGRAM CRITERIA

To qualify for Shepherd’s House, applicants must be a single

homeless woman age 18 or older with up to three children ages

birth to 10 years old. Mothers must be able to work full time and

participate in Life Skills Training.

HOW TO HELP

Sponsor a mother in the program by donating new items (cleaning

products, sheets, kitchen items, household supplies, etc.) or sponsor a

family during the holidays.

Child care volunteers are needed while mothers attend trainings

and job interviews.

Pathways to Independence is a nine-month transitional housing program for homeless single women ages 18 and

above who need emotional support and case management to make the transition to self-sufficiency. It is a

continuation of the care they receive at Pathways to Care, a short-term recuperative care facility for homeless adults

who are recovering from an illness or injury.

Pathways to Independence helps residents find and maintain employment, focus on self-improvement and stability,

learn money management, and locate permanent housing. The goal of the program is to help participants obtain and

remain in permanent housing, increase their skills and/or income, and achieve greater self-sufficiency.

On-site and off-site job training

Assistance in setting and achieving personal goals

Donations of personal hygiene items are appreciated.

St Anthony Garden Courts is a 50-unit affordable housing community for people 55 and over, conveniently located in

St Cloud next to St. Thomas Aquinas Catholic Church. The community consists of one, two and three bedroom

apartments.

Our senior residents are encouraged to keep physically and mentally active by participation in social

development programs. St. Anthony Garden Courts is an equal housing opportunity.

The complex is managed by SPM, LLC. Their professional staff receives extensive training and is dedicated to

making St. Anthony Garden Courts Osceola County’s premiere affordable housing for seniors.

St. Joseph Garden Courts is an affordable housing community with 79 one-bedroom apartments for low income

seniors. The apartments are located next to St. Joseph Catholic Church, Orlando, and many residents appreciate the

convenient access to Mass and other aspects of parish life.

Residents are encouraged to be self-sufficient and to be physically and mentally active. We provide safe and

affordable housing for low-income seniors and social development programs so that seniors are able to have peace

of mind as they age.

St. Thomas Aquinas Free Medical Clinic provides health care to the working poor in Osceola county. Services include

medical care, case management, preventative health education programs, specialist referrals, and referrals to com-

munity social services. The medical clinic is a collaboration between Catholic Charities of Central Florida and St.

Thomas Aquinas Catholic Church.

CLINIC OPERATIONS

Wednesdays 4:00 p.m.—8:00 p.m. (except holidays). The clinic is open one additional night per month for specialty

care (Orthopedic, Urology, Podiatrist, Pediatric, Echocardiograms and Ultrasounds).

The clinic is run by one part-time office manager and a team of dedicated volunteers, including:

medical director, physicians, nurse practitioners, eligibility specialists, and allied health professionals (RN’s/LPN’s).

The clinic is supported by donations and collaboration with local Catholic churches, the

Osceola Council on Aging, faith-based community partners, private individuals, grants from

local hospital foundations and businesses, in-kind donations of medical and office equipment,

and local fundraising efforts. Volunteers are the major contributors to the success of the clinic.

Pathways to Care is a 40-bed not-for-profit recuperative care facility that provides health care, housing, and hope to

Central Florida’s vulnerable homeless population in need of a safe place to heal after a serious illness or injury.

Residents receive medical care, counseling and resources to develop a social recovery plan to address the root

causes of their illness and the financial struggles that have led them to becoming homeless and unemployed.

On average, 85% of Pathways to Care residents relocate with family or other independent living options after leaving

the program.

Nutritious meals

Access to mental health counseling

Community organizations including, Catholic and Protestant churches,

homeless agencies, and hospitals partner with us to care for the

most vulnerable members of our community.

community since 1988. Many immigrants need assistance in navigating a very complex and ever-changing U.S.

The Lazarus Free Medical Clinic, in partnership with San Pedro de Jesus Maldonado Mission, provides caring, quality

medical care, referral services, and preventive health education to disadvantaged migrant workers and working poor

families. More than 1,000 sick and hurting patients receive medical attention each year.

The clinic is open Tuesdays and Thursdays from 4:00 p.m. to 8:00 p.m. (except holidays) and is operated by one

part-time office manager and a dedicated team of volunteers, including: medical director, physicians, nurse

practitioner, registered nurses, certified health educator, and administrative specialists.

HOW TO HELP

Donate: Medical supplies (including pediatric supplies) and silent auction items. Please call ahead to determine

need. Financial contributions are appreciated.

Volunteer: Volunteers are the major contributors to the success of the clinic. Opportunities include: medical

personnel, computer entry, clerical support, eligibility, community outreach, fundraising event support, and grant

writing. Days and hours are flexible. Bilingual in English and Spanish a plus.

The clinic complies with all State, Federal and HIPAA regulatory requirements.

The Western Region of Catholic Charities of Central Florida provides vital services to people in need in Polk County at two

offices in Lakeland and Winter Haven. Serving as the charitable arm of the Diocese of Orlando, Catholic Charities of

Central Florida Western Region provides hope and opportunity to the poor and vulnerable.

The Lake Morton Senior Center is a special place where seniors spend their day participating in programs that

promote health and wellness through fellowship, activities, and education. Through a coordination of programs and

services, seniors who participate at Lake Morton Senior Center find the physical and mental stimulation they need to

remain independent longer.

HOW TO HELP

Donate: In addition to support from the United Way of Central

Florida and the Diocese of Orlando, Lake Morton Senior Center

operates a thrift store on site to help offset the costs of the

program. The Thrift Store is always in need of donations

such as: household items, small appliances in working

order, new and used adult clothing in good condition.

Financial donations are also appreciated.

Volunteer: Dedicated instructional volunteers

needed on a weekly basis to teach classes such as:

exercise, arts and crafts, health and wellness,

computers and technology, and music.

“Agape means love – God’s love for all humankind, and humankind’s love for God. It’s just

the perfect word for a food bank that helps feed children and adults in the community who

Agape Food Bank, a program of Catholic Charities of Central Florida, has been

serving the hungry in Polk, Hardee, and Highlands counties since 1983. The food bank

distributes more than 6 million pounds of food each year to over 250 soup kitchens,

churches, senior centers and other not-for-profits.

This food comes from businesses, clubs, schools, parishes and individuals who sponsor

food drives, along with donated food from manufacturers and distributers. Agape is the only USDA commodities

provider in the region and the only agent in Polk County for Feeding America, the largest hunger-relief program I

n the nation.

AGAPE FOOD BANK PROGRAMS

Mobile Pantries: Agape’s outreach efforts include “mobile pantries,” which takes food to the hardest hit areas of our

community with the highest poverty rates.

SmilePak: SmilePak provides backpacks filled with food to children in great need who otherwise would not have

enough to eat over the weekend when school lunches are unavailable.

Volunteers are an essential and vital arm of Agape, providing

tremendous contributions, such as:

Sorting and boxing donated food at the warehouse

Sponsoring food drives

Distributing food from the mobile food pantry

Providing office assistance
