up to 12 months (364 days maximum, 12-month option not available in CO, IN, NV or OH)

With additional benefits to help individual stretch their health care dollar, it is the perfect choice for those who are:•Between jobs•Waiting for coverage after they’ve started a new job•A recent college graduate, or no longer covered on their parents’ plan•Unable to qualify for or afford individual coverage or COBRA

choose a deductible and coinsurance from several available options.
• 12-month coverage option (364-day maximum). 12-month option not available in CO, IN, NV or OH.

Plan Benefits• Deductible: Choice of $500; $1,000; $1,500;
$2,500; $5,000; $7,500; or $10,000• Plan Pays: After the Deductible - 80% for
covered expenses; after the $2,000• Out-of-pocket maximum: The plan pays
100% to the lifetime benefit• For deductibles of $5,000 and higher, the
plan pays 100% to the lifetime limit• Lifetime Benefit: $250,000 per insured• Accident Protection Option: 100% of the
first $500 of covered expenses due to a non-work-related accident

STMP Commission & Bonus
Any month in which your commission
statement includes ten (10) months of
commissionable premium for the Short
Term Medical PLUS plan, a bonus of 5% of premium will be paid.
