2

Catheters are used for inappropriate reasons

Asepsis/infection control is breached during insertion

Asepsis/infection control is breached post-insertion

Inappropriate urine specimen collection

Clinician awareness of catheter is limited

Catheters are left in situ for too long

Lessons are not learnt from previous CAUTIs

events

Catheters are inserted for

wrong indications

Catheters are inserted for

nursing convenience

Catheters are inserted for

specimen collection

Catheter insertion is not documented

in the healthcare record

Catheter is not secured to

body

Catheter is not routinely

checked for breaches

Poor understanding of

catheterisation risks

Catheters are forgotten until

discharge

Catheter is not covered during

ED or shift handover

Patient contaminates

catheter or bag

No catheter removal/review

plan is in place

Inappropriate catheter

choice

Aseptic technique is

not used

Inadequate cleaning pre-insertion

No investigation of

previous events

Not a mandatory HAI

clinical indicator

Insertion competency varies

between clinicians

No removal order is documented

in the healthcare record

Don’t know the appropriate indications

for urine specimen collection

Catheter information is not
