NON AVAILABILITY BIRTH CERTIFICATE – CDMA Commissioner and director of municipal administration, known as CDMA, registers and

maintains the Births and Deaths taken place all over A.P. Non availability of Birth Certificate is required for below reasons.

1. Your birth was never officially recorded. 2. Your birth records have been lost at Dept in natural calamities.

Category Type B

User Charges INR 35/-(1. For Panchayath INR 10/- per Each Copy

2. for Municipality INR 50/- Up To 4 Copies, INR 100/- for 5 to 10 Copies)

SLA 7 Working Days

Documents Required ♦ 1) Application Form* and Any one of the below documents

♦ 2) Ration Card/Any Other Residence Proof

♦ 2) School Bonafide Certificate

♦ 3) Secondary School Certificate(Marks Sheet)

♦ 4) Notarized Affidavit(Non-Judicial Stamp Paper)

Note: The asterisk (*) denotes mandatory requirement of documents.

Apply for Non Availability Birth certificate:

This section contains instructions for the operators of the mee seva web application to apply for

Non Availability Birth certificate.

1) In mee seva home screen, under List of Services, select CDMA Department services as

depicted in Figure 1

2) In The list of CDMA Department services select Non Availability Birth Certificate as depicted

in Figure 2

3) Now Non Availability of Birth screen will be displayed. As depicted in Figure 3

4) Enter Birth Details such as Child Name, Child Sur Name,Relation, Father Name, Father Sur

Name, Mother Name, Mother Sur Name, Date of Birth, Place of Birth(Either

Hospital/Residence/Others), selected location Name, Address1, Address2, Address3, District,

Location(Either Greater Municipality/Municipal Corporation/Municipality/Gram Panchayat) And

Registration Unit Id . As depicted in Figure 4.

5) Enter Informant Details such as Informant name, relation &name, Informant Address1,

Address2, Address3, Mobile No, Aadhaar No, Ration card No, Email ID, Remarks, Pin Code,

Purpose of the certificate and No.of Copies As depicted in Figure 5.

Note: Based on the Citizen request, the delivery option (either through Manual or Speed Post -

Local or Speed Post - Non Local at the Franchisee) has to be selected. As depicted in Figure 6.

The courier charge for Post (Local) is INR. 33/- and for Post to Non Local areas is INR. 46/-.

6) If Delivery type is Speed Post Local/ Non Local, It will Display Postal Details As depicted in

Figure 7.

7) Enter Postal Details such as Door No, Locality/Land Mark, District, Mandal, Village/Ward and

Pin code as depicted in Figure 8.

8) Collect the necessary supporting documents as indicated in the Non Availability of Birth-

CDMA Certificate request service page scan the documents and upload into the system, as

depicted in Figure 9.

9) Click Show Payment. Another window appears requesting to

figure 10.

10) After Show Payment. Confirm Payment will be displayed with uploaded Documents and

payment details as depicted in figure 11.

11) Based on the delivery option chosen by the citizen, the service charges will be calculated and displayed in Show Payment as depicted in figure 12.

12) Collect the amount from the Citizen and click Confirm Payment to submit the request as depicted in figure 13.

13) On confirmation, a receipt will be generated as depicted in the following figure 14. The

receipt will contain the Certificate delivery date.

Note: Ensure that you have sufficient balance with your service provider (SCA), else the system

will not accept the request.

14) On submission, the request will go to respective Department workflow for further

processing.

15) Once the request is approved by Department then, Non availability Certificate will be dispatched through courier to the customer’s address if Delivery Type is Speed Post Local/Non Local.

� If Delivery Type Is Manual, then the citizen need to collect the Non availability Certificate from franchisee where he/she applied for the Certificate.
