Proposed project 1: Mobile Health (r)Evolution, Patients‘ Empowerment and Adherence

DIGITAL TRANSFORMATION - Digital health and care solutions

BETTER HEALTH AND CARE - Contribution to the Call on Digital transformation: Innovative digital solutions for people

Content: Prevention, disease management and medication management of key chronic diseases. Its aim is to develop & implement social innovations - new patient/HC providers interaction/approaches and implement them in primary health care by employment of advanced ICT solutions.

Targets: Improvement of patients empowerment and adherence by 20% as well as optimization of healthcare capacities and costs for EUR 50 mil./mil. population per year.

Partners sought: Regions, innovation hubs/ICT experts and other key partners able to manage:

• Local project with 30-50 health teams (GPs, enrolling app. 300 patients each) for pilot implementation of mobile health management, followed by scale up.

• Best HC practice as well as knowledge sharing from medical psychology, health anthropology and health marketing by interregional multidisciplinary collaboration.

Funding: Combined local/international (EU) funding.

Proposed project 2: Pharmacist supervised chronic disease self-management platform and business model

DIGITAL TRANSFORMATION - Digital health and care solutions

Content: Transformation of pharmacies from medicines dispensing business to high added value healthcare service business, supported by ICT tools for connected /integrated health management.

Targets: Increase the HC capacities by activation of underused competences of pharmacists by clinical pharmacy services implementation: clinical pharmacotherapeutic review, medicines use review, personal medication card, personal health and disease management, pharmaceutical care, and others for prevention and treatment of chronic diseases.

Partners sought: SME partners (ICT experts or similar) with access to/collaborating with pharmacies / chains

Funding: Combined EU SME 2nd level / private funding.
