STEP 1

Call Employee Occupational Health Services (EOHS) to schedule

pre-placement health assessment

STEP 2

Visit: New team member “Launch Pad” after EOHS

Launch Pad Location: 1633 N Capital Avenue (Suite 730)

Hours: 7:30 a.m.-4 p.m. (Monday-Wedns & Friday)

7:30 a.m.-12p.m. (Thursday)

Bring the “Provisional Employee Badge Form” with you if

received during the pre– placement health assessment

Parking on the day of New Team Member Orientation (1st day of employment)

Free Parking at Lot 404 (North Campus Lot: 1200 Indiana Ave) & 403 (North campus Lot: 1302 Indiana Ave. Vehicles will

not be ticketed. Ride IUPUI Brown Line shuttle to the Riley Outpatient Center (ROC) stop. Allot time to park & shuttle in.

Download the Doublemap app for shuttle times/locations (select system: IUPUI). Should you wish to park closer (Riley

Garage) you can do so at your own cost

Bring with you: I-9 Verification Documentation, if applicable (examples: driver’s license and social security card or passport, etc.)

Dress for orientation: Business Casual (Example: Khakis, Polo Shirt)

Do NOT wear: Jeans, shorts, Non IU Health scrubs, sleeveless tops, sweats, midriff shirts, sandals, flip flops

Bring the following items with you

To EOHS & “Launch Pad” visits

All records/ documents requested for pre-placement

assessment.

Verification of eligibility to work I the US (I-9 documentation)

Year, Make, Model & License Plate number (for obtaining

your work parking permit)

Uniform Size (if applicable)

Emergency contact info, direct deposit routing/account

numbers (if applicable)

Unable to visit the New Team Member “Launch Pad”?

Be sure to complete ALL the following prior to attending orientation

Visit EOHS & complete your pre placement health assessment (contact info listed above)

Safety & Security office to have your badge photo taken (bring your “Provisional Employee Badge Authorization Form”
