How do we all access the Care Plan?

CHOC IT Progress• Use of EMIS Clinical Services as

live access to push and pull relevant information from the GP system

• One anticipatory care plan with key things which keep someone out of hospital

• Members of the multi-disciplinary team have access to enter data

• Flows immediately back into the GP EMIS system

• Can be viewed by out of hours staff

• Patient / carers / relatives also have information in printed form

conferencing equipment for moving to more virtual working

Care Plan

Compatible with the data required on the Medical Interoperability Gateway which will eventually connect all systems in east Kent

CHOC Development Progress

• Created with full patient participation, the WaitLess App gives real time data for all the MIUs and A&Es in east Kent.

• Launched in Dec 2016 this innovation was the first of its kind in the UK and the potential impact was unknown.

• Over 15,000 downloads and 130,000 usages to the end of October 2017

• It was anticipated that perhaps 5% of patients attending A&E would be routed to MIU facilities. Overall shift of just over 6%

• Initial results from the east Kent pilot have been independently validated by the University of Greenwich and the Behavioural Insights Team.

WaitLess App has been through national scrutiny, led by a panel, chaired by Sir Bruce Keogh, for the National Innovation Accelerator
