will enable us to retain talented new nurses who want to live and work here, to take care of our community. In addition, we have worked creatively to provide affordable hous-ing for our team in opening the Hanover House in partnership with Island Housing Trust and hope to do more in the future.

We are excited by the opportunity the future holds for Martha’s Vineyard Hospital and Windemere. The next phase of our strate-gic plan will require a thorough look at our campus to examine how we can incorporate more services and expand functional space for some of our most-needed outpatient programs like physical therapy.

Your voice is an important part of our suc-cess and we have appreciated your calls, letters, and attendance at our open forums. Our health is your health – for year-rounders and visitors alike. We look forward to sharing this journey now and as we move into this next exciting phase of our hospital’s future.

This past year was a pivotal time at Martha’s Vineyard Hospital and Windemere Nursing & Rehabilitation Center. With the kickoff of our strategic plan, we dug deep into our clin-ical services to assess how we could better meet the needs of our community as we turn our focus to the future.

As part of our plan, we will continue to focus on creating access in primary care with the addition of new providers. We added five new physicians this past year, including a psychiatrist, and have our sights set on recruiting two additional primary care pro-viders over the next two years. The construc-tion of a new primary care wing, expected to be completed in early 2020, will further allow for more capacity with the addition of 16 new exam rooms. A patient call center to be opened soon will provide more efficiency and better customer service in our outpa-tient clinics.

To further fulfill our mission, we recognize and support our employees, who are the backbone of what we do at Martha’s Vine-yard Hospital and Windemere every day. This year, we created a loan forgiveness program for new nurses who struggle to pay back school loans and remain on the island with its higher cost of living. This program

We have community outreach projects sur-rounding substance use and food insecu-rity. We work with the Island Housing Trust to help develop solutions to the current housing crisis. We lead health care train-ing programs for island residents and are working hard to develop a new vision for elder care on the island. We now provide access to primary care for all residents and seasonal visitors. We have a physician- staffed emergency department that is here for all – all of the time.

We cannot be at the heart of our commu-nity without your help and support. The next chapter for the Martha’s Vineyard Hospital is already being written and I hope that you will be a part of it.

Martha’s Vineyard is a special place: a commu-nity of year-round residents, seasonal residents, and short-term visitors who are all fortunate to experience what this beautiful island has to offer. If you’re reading this, you are a member of our community. We share not only an interest in expe-riencing Martha’s Vineyard, whether through work or play, but also in our health. That’s why the mis-sion of the Martha’s Vineyard Hospital resonates with all of us: “to safeguard the health of Martha’s Vineyard residents.”

Without community we would have no mission. And without a mission we wouldn’t persist. We must support our community – and do so in many ways. We are the largest employer on the island and each employee has chosen a career that serves others. They care for visitors, residents, neighbors, family members, and even coworkers. They’re here every hour of every day to keep our community healthy. But it’s our employees’ contributions to the community beyond their hospital work that makes us most proud.

Given the importance of community, we also seek to better understand whom we serve and what services we need to provide. We are engaged in a strategic planning process involving surveys, employees, and patients so that we can plot our future and determine what services we need to provide to keep our community healthy.

It’s been about living our mission “to safeguard the health of Martha’s Vineyard residents.” About listening, engaging, and understanding our neighbors. Most of all, it’s been about rediscovering what it means to belong to our island community.

Because what makes us a community is not just who we are, but what we share. As year-round residents, we all want access to the same goods and services that the people on the mainland take for granted. We all deal with the seasonal ebbs and flows, rising costs, and the lack of affordable housing. We are all concerned about substance use disorders and the opioid epidemic, and we want to do something about it right here on the island. We all care for our neighbors and want everyone to live healthier, more fulfilling lives.

As you’ll see in the stories within this report, our decisions today and what we plan for our future are guided by these things we share. Whether it’s prioritizing primary and preventive care, expanding access to specialists on the island, or opening our facil-ities to our neighbors, we are doing our part to make life better on Martha’s Vineyard. Not just because it’s in our mission statement, but because it’s what it means to belong to a community.

With the assistance of our affiliates at Massachu-setts General Hospital (MGH), Brigham Health, and Boston Medical Center (BMC), we have imple-mented industry-standard medication treatments and trained all emergency department doctors – and most primary care practitioners – in the care of patients with SUDs.

These efforts have allowed us to expand the number of practitioners with waivers to prescribe buprenor-phine from three to sixteen in the course of a year. Buprenorphine is a lifesaving medication for people in recovery from an opiate use disorder. When taken properly, it offers relief from cravings and lessens the effects of withdrawal symptoms for people with SUDs. Combined with the emergency use of nalox-one (Suboxone), medication-based approaches have dramatically decreased the risk of overdose death in our community.

Other efforts include a partnership with BMC’s Office Based Addiction Treatment Training and Technical Assistance team to train our primary care and emer-gency department staff to eliminate stigma and pro-vide proper treatment for patients with SUDs. Working with the MGH Emergency Department, MVH has developed a kit for patients who have overdosed or need assistance with their recovery. Having immediate access to medication to stabilize dopamine levels and decrease cravings can have a tremendous impact on an individual’s chances of successfully overcoming their SUD.

Most importantly, patients receive a follow-up with a MVH primary care physician and support from an on-staff psychiatrist within three days of an emergency room visit. The Substance Use Disorder Team provides access to detox beds and crisis support with increased emphasis on opiate use disorders through our part-nerships with Gosnold Treatment Center and Martha’s Vineyard Community Services.

Karen Casper, MD, Medical Director of the Sub-stance Use Disorder Program at MVH, explains,

“The reality of substance use is that it touches all of us at MVH. It is a medical condition which afflicts our patients, friends, and families. We are dedicated to becoming more educated on best practices to help people with substance use disorders manage their illness and reclaim their independence.”

In light of the ongoing opioid crisis across Massa-chusetts, MVH’s efforts to improve treatment for patients with SUDs are poised to make a remark-able impact on the lives of people in our Martha’s Vineyard community.

Using Epic, Partners HealthCare’s system-wide electronic medical record, MVH primary care doctors can request a consult with Massachusetts General Hospital specialists, who review patient charts and provide management medi-cal opinions on the appropriate next steps.

With weeks-long wait times for in-person visits, the ability of eConsults to provide guidance within 72 hours is a major benefit for primary care physicians and patients. Better yet, eConsults do not require schedule coordination between physicians; specialists can get all the information they need directly from the electronic health record, including previous lab tests, specialist visits, and notes from the referring phy-sician. When the specialist determines the appropriate next steps, the medical record is automatically updated, and the physician is notified.

Available to outpatients and for non-urgent cases, eConsults are an important new tool for MVH caregivers. According to Dr. Pieter Pil, MVH Chief Medical Officer, “eConsults streamline care and reduce wait times for patients and their providers to receive specialized care guidance.” In a moment of need, speed and convenience can make all the difference for patients in our community.

improved breast care, close to home

Martha’s Vineyard Hospital and Massachusetts General

Hospital (MGH) are proud to offer ultrasound-guided

core biopsies to women in our island community. With

images taken at MVH and biopsies performed by visiting

MGH breast imaging radiologists, patients are finally

able to get essential diagnoses without leaving the island.

As Natasha Hartmann, supervisor in mammography,

explains, this is made especially important by the dif-

ficulty and cost of traveling to the mainland: “Having

the experience of the Mass General radiologists and the

world-class care they offer at our fingertips allows us to

care for our community locally, with the most advanced

resources available nationally.”

The Mammography Department performed 2,700 mammograms last year.

productive dialogue for better care

On June 4, 2019, Martha’s Vineyard Hospital held its 2nd Annual Public Forum. Speaking to more than 60 community members, President and CEO Denise Schepici, MPH, discussed the progress made in the past year and the challenges facing MVH and Windemere today and in the future. An open Q&A allowed community members to share concerns and get immediate responses from hospital leadership. This kind of frank, ongoing exchange has become integral to the culture at MVH and will help us tai-lor our services to the needs and expectations of the Martha’s Vineyard community in a more transpar-ent and inclusive way.

Susan Jenkinson has been a resident at Windemere since May of 2011. Her daughter and two of three granddaughters have worked at Martha’s Vineyard Hospital for a combined total of almost 50 years. In fact, Susie loves living at Windemere, in large part, because it keeps her close to her family. But, as a life-long Red Sox fan, Susie’s favorite thing about Windemere is the way the caring staff makes sure she is in front of the TV for the first pitch of every game.

The Jenkinsons cherish events hosted by the staff at Windemere like the Harvest Festival and Summer BBQ, which bring family members together with Susie to share enjoy-able day-long activities. Both Susie and her family laugh as they recall seeing their grand-son, Kayden, act as “side entertainment” with his exuberant dance moves. Susie loves riding around in her Red Sox-adorned scooter for meals and watering the flower and vegetable garden, at ease with the knowledge that she lives with staff and residents who care about her, and that her beloved family is never too far away.

We continue to strengthen our relationship with Mar-tha’s Vineyard Community Services. This year, we part-nered with them to transform our Red House property into a Recovery Support Center that will provide well-ness classes, life skills training, and professional sup-port to enhance and promote recovery. The Recovery Support Center, led by the recovery community, will be a valuable part of the island’s continuum of care for those seeking a path to recovery from drugs or alcohol.

We see these and other partnerships as one of our core values as health care stewards in the Martha’s Vineyard community. There is, however, much work to be done, and we look forward to developing new and productive relationships with other island organizations in the years to come to fulfill our mission.

Health and wellness, however, often involve more than medical equipment and examination rooms. These partnerships have addressed important community challenges such as affordable housing, end-of-life care, and the recovery process from substance use disorders.

The lack of affordable housing has a particular impact on our ability to keep crucial staff members of Martha’s Vineyard Hospital and Windemere on the island year-round. In fact, over the years we have lost some of our best team members to opportunities off island, where living costs are lower. That’s why, starting in 2018, we partnered with Island Housing Trust (IHT), a local non-profit that works to secure affordable housing for qualified islanders. IHT purchased the Hanover House, a bed and breakfast with 15 private living spaces in Vineyard Haven, with MVH as a long-term lease holder. This arrangement allows us to offer housing to qualified employees of MVH and Windemere – taking care of our employees to ensure care for our community. Last year we forged a new partnership with Healthy Aging Martha’s Vineyard to raise awareness in the com-munity about advanced directives for end-of-life care. Staff training has been expanded to include cultural dif-ferences in end-of-life care, starting conversations about advanced directives, and putting principles into action. Further initiatives will allow us to better communicate with our patients about health care proxies. This ongo-ing work is driven by a core insight: when someone you love is nearing their end of life, whether it be expected or not, knowing their wishes can bring clarity and comfort to a difficult time.

As long-time visitors who came to call the Vineyard “home,” the Hutchinsons understood the impor-tance of bringing great primary care to the island. As daughters Diane Hutchinson Reilly and Alexan-dra Hutchinson Kline explain, “We saw the need for primary care was growing and knowing how important it is to the island community our par-ents loved, investing in the new primary care wing to expand access felt right. We are honored and fortunate to have the opportunity to direct how the funds be used to benefit MVH. The progress CEO Denise Schepici and the organization have made recently inspired us to want to give back in a way that would provide the most beneficial impact on the community.”

Growth plans for primary care include the addition of two new providers – one in the coming year and another in 2021. These expanded resources will allow MVH to care for the entire year-round population of 17,000. And a new primary care wing opening in early 2020 will provide 16 new exam rooms, doubling our capacity and making a signifi-cant impact on our ability to provide the best care for the entire Vineyard community.

The new primary care wing will create 16 new exam rooms in addition to the 20 that are already existing.

At the rate primary care is growing and knowing how important it is to the island community our parents loved, investing in the new primary care wing to expand access felt right.

Since 2018, we have added four physicians to our primary care team and, for the first time, embedded a psychiatrist in the department, Dr. Ruth Barron. Bringing a psychiatrist onto our team is part of our accreditation as a Patient Centered Medical Home, a health care model focused on the whole patient – mind and body.

This expanded primary care team has allowed us to serve over 2,000 new patients since 2018. In fact, there is no longer a waitlist to see a physician. We have also improved access by adding drop-in hours for pediatric patients in need of a last-minute appointment.

Combined with the forthcoming expansion of the primary care space by early 2020, these efforts are helping us meet the need for primary and preventative care for our entire community.
