HealthCare Chaplaincy – Finding Meaning u Bringing Comfort

From the Chairman and the President & CEO

Thomas Jefferson said, “The greatest service which can be rendered any country is to add a useful plant to its agriculture.”

We view Jefferson’s statement as a metaphor for bringing something new into people’s lives that makes a difference. For fifty years HealthCare Chaplaincy has accomplished exactly that through its leadership in the research, education, and practice of multifaith chaplaincy care.

Now we are cultivating the next new “useful plant” by linking HealthCare Chaplaincy’s established strengths with the growing medical specialty of palliative care, which encompasses the whole person, relieves suffering and improves the quality of life of patients with serious or chronic illness and their families. It matches treatment to the patient’s values and goals and is delivered by a transdisciplinary team of physicians, nurses, professional chaplains, social workers and others.

Today more Americans recognize that we need palliative care to help us meet an immense challenge – providing quality health care for the growing number of Americans with serious and chronic illnesses while reducing costs.

Recent research by the national polling firm Public Opinion Strategies shows that even for those patients who are uninformed about palliative care, once they understand what it is, 92% report they would be highly likely to consider it for themselves or their families if they had a serious illness. The same percentage believe patients should have access to this type of care at hospitals nationwide.

We are engaging in a national conversation on palliative care in a key leadership role for an area that has been underrepresented – the important role of spirituality.

Our innovations in education are enabling chaplains to take a greater role in palliative care teams. In 2011 our faculty and visiting lecturers delivered a series of grand rounds sessions for our staff chaplains to increase their competency in the delivery of effective palliative care in the metropolitan New York institutions where they work. Thanks to grants from the Arthur Vining Davis Foundations and the Retirement Research Foundation, we are developing the first curriculum that will lead to sub-specialty certification in palliative care by the Association of Professional Chaplains.

Also, our online educational webinars are giving chaplains across the country an opportunity to receive continuing education on topics ranging from cultural competency in health care to financial abuse of the elderly.

An additional educational offering, HCC’s twice-monthly online professional journal PlainViews has, under managing editor the Rev. Sue Wintz, become a vehicle that translates knowledge and skills into effective chaplaincy and palliative care. PlainViews gives its readers timely information that enhances their professional practice and is also a forum to communicate with each other. It’s no surprise that it recently crossed the 1,000 paid subscriber mark.

HealthCare Chaplaincy – Finding Meaning u Bringing Comfort

Unlike the fields of medicine, nursing, and social work, professional chaplaincy lacks a body of scientific research that adequately defines and documents the best practices for chaplaincy care and the role of spirituality within palliative care. A significant $3 million grant from the John Templeton Foundation will enable HealthCare Chaplaincy to lead the way to remedy that. Over the next three years we will create a cadre of board certified chaplains skilled and committed to carrying forward the future research agenda. The goal is for chaplaincy to become an evidence-based profession similar to other disciplines. A prestigious advisory group and faculty will play a major role.

We are becoming an authoritative voice on professional multifaith chaplaincy and palliative care through an array of channels that include our website healthcarechaplaincy.org, the twice-monthly e-newsletter HealthCare Chaplaincy Today, the printed bulletin The Beacon, and through frequent posting of informative content in the online social media Twitter, Facebook, and LinkedIn.

Also, this year HealthCare Chaplaincy assumed publication of the respected Soul of Bioethics e-newsletter edited by Dr. Rick Moody, director of academic affairs for AARP.

We are developing the National Center for Palliative Care Innovation that will include a new model of residential care with on site health care and support services that will change for the better how we Americans live the final years of our lives.

In our jubilee year the mantra “Imagine–Innovate–Inspire” has characterized how HealthCare Chaplaincy’s talented staff have taken on the challenges and opportunities that lie ahead for our next 50 years.

T. Michael LongChairman

The Rev. Dr. Walter J. Smith, S.J.President & Chief Executive Officer

“HealthCare Chaplaincy, through its professional chaplains and trained ministers, plays a meaningful role in the recovery and spiritual well-being of patients at Hospital for Special Surgery. Our physicians, nurses, physical therapists, and other members of the HSS family, depend on chaplains to help us improve quality of life for many of our patients. We value our more than thirty-year partnership with HealthCare Chaplaincy, and appreciate their commitment to our mission of providing unsurpassed patient care.”

HealthCare Chaplaincy – Finding Meaning u Bringing Comfort

HealthCare Chaplaincy is an international leader in the research, education and practice of multifaith spiritual care and palliative care.

Palliative care, which relieves suffering and improves the quality of life of patients with serious or chronic illness and their families, is one of the fastest growing fields in medicine.

For 50 years our professional Board Certified Chaplains have helped more than 5 million patients, loved ones and staff find meaning and comfort when coping with life-changing health events – regardless of religion, beliefs, or culture. Professional chaplains are arguably the most cost-effective resource to increase patient satisfaction.

We intend to expand our work into community settings through senior centers, local congregations, and correctional facilities.

HealthCare Chaplaincy is the only organization that does all three:

• Clinical practice – We manage and staff professional chaplaincy care departments in major metropolitan New York hospitals and long-term care facilities. Our board certified chaplains are integral members of the professional health care team.

• Clinical and continuing professional education – We are a leadingprovider of chaplaincy education in the United States, and now affiliated with Columbia University Teachers College. With support from the Arthur Vining Davis Foundations and the Retirement Research Foundation, we are developing the first curriculum that will lead to sub-specialty certification in palliative care by the Association of Professional Chaplains.

• Research – Our Spears Research Institute is a leader in rigorous scientific research on the relationship of spiritual care and health. With support from the John Templeton Foundation, over the next three years we will create a cadre of board certified chaplains skilled and committed to carrying forward the future research agenda for chaplaincy to become an evidence-based profession similar to other disciplines.

HealthCare Chaplaincy is developing a National Center for Palliative Care Innovation on a riverfront campus in Manhattan, the first comprehensive project of its kind in the world.

This residential, research and education center will integrate palliative care practices within an enhanced assisted living residence and provide continuous care for those living with serious, progressive illnesses. It will demonstrate that better care depends on the quality, not the quantity of care.

The Center will also house a geriatric and palliative care medical home practice, which will offer “whole person” multidisciplinary and integrated care not only to our residents, but, on an outpatient basis, to people who live in the neighboring communities. The Center will also be the new permanent home for all of our educational and research programs, our library and instructional spaces as well as for our executives and program administrators. This building has been designed to meet eligibility requirements for LEED (Leadership in Energy and Environmental Design) Platinum “green” certification, and will also include a completely automated parking garage.

HealthCare Chaplaincy – Finding Meaning u Bringing Comfort

From the Executive Vice President & COO

HealthCare Chaplaincy’s reengineering program has added the fourth word “Integration” to the mantra of “Imagine–Innovate–Inspire.” While internal departments operate in their own areas of specialty, we have shattered artificial silo walls. The result is collaboration and creativity that have lifted effectiveness and have created an organizational culture that crackles with positive energy.

Clinical Practice, Education and Research

As health care reform is requiring greater integration within our partner institutions, HealthCare Chaplaincy is demonstrating ways that

chaplains help to improve the patient experience, teach staff to become more competent when dealing with diversity, help staff reduce burnout, and facilitate coordination among the members of the health care team.

HealthCare Chaplaincy now has 14 clinical partnerships with the addition of Steven and Alexandra Cohen Children’s Medical Center of New York, a part of the North Shore-Long Island Jewish system, and the Mount Sinai Medical Center in Manhattan. At both, HealthCare Chaplaincy provides the professional chaplaincy services through board certified chaplains; and at Mount Sinai, an educational program gives chaplaincy student interns clinical experience.

At all 14 partner institutions our chaplains work closely with local clergy to help

patients transition from acute care settings to the community. HealthCare Chaplaincy’s educational programs have expanded beyond the foundational clinical pastoral education. We have built a solid program in Supervisory Education – the “teacher training” program for educators of professional chaplains. Each year HealthCare Chaplaincy, through a highly selective process, recruits Supervisory Education Students who enter a rigorous two year teaching program to earn the credentials that permit them to apply for certification as Chaplaincy Supervisors.

In addition to the research projects cited in the letter from the chairman and president & CEO, HealthCare Chaplaincy’s research department is contributing to Quality Improvement projects that our clinical staff are developing in order to adequately evaluate them. This represents a shift to focusing on outcomes evaluation rather than theoretical analyses.

Policy Formulation

At our request, the four national chaplaincy associations have agreed that HealthCare Chaplaincy will serve as the collective chaplaincy representative within an important coalition – the National Coalition for Hospice and Palliative Care – which is educating legislators and key health policy makers about the importance of hospice and palliative care.

HealthCare Chaplaincy this year delivered testimony to the Medicaid Redesign Task Force assembled by New York’s governor Andrew Cuomo. We showed that the enhanced assisted living model could, if used for even 1,000 persons within the last year of life, save tens of millions of federal and state dollars, while providing substantially higher quality and more coordinated care than the current “ping ponging” between nursing homes, home care and hospitals. Currently, the U.S. spends more money per capita than any other developed country but ranks ninth in terms of quality of care.

National Center for Palliative Care Innovation

The transition to old age and our inevitable mortality is what Americans fear most.

HealthCare Chaplaincy – Finding Meaning u Bringing Comfort

HealthCare Chaplaincy is developing the National Center for Palliative Care Innovation to demonstrate how this unavoidable transition can be a time of meaning and fulfillment rather than one of failing health and loneliness.

This enterprise recognizes that for a person in this stage of life, spiritual needs – those things that seek to put together the whole meaning of one’s life – are at the core, and that the American health care system gives them insufficient attention and focus, because it focuses on management of medical issues.

The National Center for Palliative Care Innovation will be housed in a new multi-function building that will include 120 units of enhanced assisted living for persons with serious progressive illnesses, a geriatric/palliative care outpatient medical center, the educational, research, clinical practice and administrative offices of HealthCare Chaplaincy, and a parking facility. It will be a highly “green” building on Manhattan’s Lower East Side along the FDR Drive.

The National Center for Palliative Care Innovation will address the important issues required in managing day to day life for the residents as well their health care needs.

For the first, HealthCare Chaplaincy has engaged the management of Carnegie East House, a 100 unit, nonprofit enriched housing residence as the management agent for the new residence. Carnegie East House will oversee the temporal initiatives, while a chaplain co-director will further the residence’s spiritual and programmatic vision.

For the second, Beth Israel Medical Center, part of Continuum Health Partners, has signed a letter of agreement to provide multi-disciplinary comprehensive geriatric and palliative outpatient care to the residents and the broader community. A licensed nursing and hospice operator will provide skilled nursing care and hospice care.

Support for families and loved ones will be a priority to help families address their own feelings as they experience transitions, and help them to deal with their losses.

This Center is set to move to the construction phase in 2012 as HCC completes the financing package, including raising the necessary equity, obtains the necessary approvals, and breaks ground on what will be a national demonstration model for palliative care.

Claire H. AltmanExecutive Vice President and Chief Operating Officer

“Winthrop-University Hospital’s Board of Directors, administration and staff have come to appreciate our well-developed partnership with HealthCare Chaplaincy as an essential element to the growth and development of Pastoral Care Services at our organization. The Chaplaincy’s expertise, level of professionalism, and commitment in providing chaplains of the highest quality has been invaluable in maintaining our standards for outstanding patient care.”

HealthCare Chaplaincy – Finding Meaning u Bringing Comfort

Despite the uncertain economic times, HCC’s net assets increased $2.6 million bringing HCC’s total assets to $26.2 million. Total support and revenue, excluding investment income, increased $0.6 million to $5.8 million. HCC benefited from the bequest of $959,000 from Harriet Huber, of which $370,000 has been designated by the Board in October 2011 to provide support for scholarships for chaplaincy students. HCC’s re-engineering efforts produced important efficiencies in fiscal 2011. These resulted in operating expenses of $7.0 million in 2011 as compared with $7.3 million in 2010 (a 4% reduction in spending).

The solid performance of HCC’s investment portfolio of 25.6% will likely not be matched in 2012 given the turbulence of the global economy. To protect against such volatility, HCC’s Board of Trustees adopted a new investment policy in 2011 which called for significant diversification of HCC’s investment pool. This diversification was achieved by June of 2011 and is an important safeguard in these uncertain economic times.

As HealthCare Chaplaincy pursues its bold vision to lead in the field of spirit centered palliative care with the development of the National Center for Palliative Care Innovation, it proceeds on a sound financial footing.

“I wanted to write a quick note about the Rabbi who is the Chaplain here at our hospital. Yesterday and extending into today, I had a devastating but difficult case. The family and patient are Russian Jews who required spiritual guidance from the Rabbi to alleviate their anxiety. He went above and beyond and assisted the Management team by becoming the liaison. His calm demeanor and kindness was a Godsend to us and we could not have accomplished as much without him. We are grateful to have him as our Chaplain and wanted his actions and assistance to not go unnoticed.

2010 Wholeness of Life Awards DinnerA GATHERING OF FRIENDS

“For almost 20 years our patients, families and staff at Lawrence Hospital Center have enjoyed a rich partnership in pastoral care with HealthCare Chaplaincy! One of the seasonal highpoints of our hospital year is our Wholeness of Life award ceremony when we spotlight our honoree for excellence in caring for the whole person—body, mind, and spirit. I know that this profound and deeply felt award lifts the spirits and refreshes the energies of our whole organization as we all strive to reach for excellence on every level and continue to serve the health care needs of our community in our second century.”

What People Say About Us

“My husband had his surgery and is recovering nicely at home. He had a rocky time in the hospital that gave both of us much stress and anguish. Tom is lapsed religiously and chose not to have any ministry, but I certainly needed it! You were kind enough to listen and say a prayer for us and I felt so much better afterwards. God sent you to me when I was wilting. I send you all my best wishes and say a prayer for you. God bless you!”

What People Say About Us

Sent to the president of Hospital for Special Surgery:

“I am a graduate nurse, and as such, I am very much aware of the quality of care. My husband was scheduled for knee replacement. We arrived at your hospital with surgery scheduled for that day. While at the drop-off point, our car was hit by another driver. Immediately your people were there to help us. I had to stay to await the police, and my husband had to make his way up to the Same Day Surgery Area. I was very concerned about him, as I had always taken him by wheelchair…but not to worry. Along came your chaplain who helped in every way possible. She became our Guardian Angel, not only throughout this very upsetting day but also throughout my husband’s stay. We will never forget our experience at the hospital and will always recommend your facility for any and all.”
