Before you submit:
1. The Academy has drafted a template letter for you to use as you draft your letter. 2. All letters must include your name and the file code, CMS-1631-FC. The template letter includes
the file code in the appropriate location.3. You can always go to regulations.gov to access a webpage devoted to the final rule we are
working on. Go to regulations.gov and run a search for the rule’s Federal Register Number: 2015-28005. The rule’s name is “Medicare Program: Revisions to Payment Policies under the Physician fee Schedule and Other Revisions to Part B for CY 2016.”
4. Once you’ve written your letter, you are ready to submit it.
To submit your letter online:
1. Follow this link to go to the correct submissions form: http://1.usa.gov/1NXFo7i 2. You can either
A. Cut and paste your letter into the comment box (note there is a 5,000-character limit which is approximately 1.5 typed pages).
OR B. Upload your letter by clicking “choose files” and following the instructions (there is no
character limit for an uploaded letter but we do not recommend that these letters go over 2 typed pages).
3. Scroll down the screen and provide your first and last name in the appropriate fields, check “I want to provide my contact information” and fill out the online form when it appears. Deselect “I am submitting on behalf of a third party.”
4. Click on the “Category” drop down menu and scroll down to select “Health Professional – A0007”
5. Click on the blue “Continue” arrow in the bottom right hand corner6. Verify that your information is correct
Click on the edit button if there is incorrect information7. Check the “I read and understand the statement above” and click “Submit Comment”8. A screen will appear with a confirmation number for your comment. Copy the number down so
that you can use it to ensure your comments have been received.9. Email your comment letter to the Academy, [email protected], so that AGS and the
Academy can maintain our records.
