Get the Kit Simply visit my.Questforhealth.com to request an At-Home Test Kit. 1. Create an account if you’re a first-time user, or, if your information has changed since your last visit. NJWELL is the registration key. 2. The kit will be mailed to you with everything you need, including instructions and a postage-paid envelope to safely return it to Quest Diagnostics for processing. 3. If you have already downloaded a Physician Results Form or made an appointment with Quest Patient Service Center, but have not yet completed the NJWELL Health Screening, you must first cancel your Physician Results Form “order” or Patient Service Center appointment before selecting the At-Home Test option.
