New Medical Plan Added • The CareFirst Catastrophic Medical Plan—A new

medical plan that provides basic coverage for big medical expenses, rather than coverage for everyday, routine medical services. This plan is not for everyone, so be sure you compare it to the other CareFirst medical plans

More Access To Services For CareFirst Members • Free mental health visits through CareFirst Video Visit—

See a mental health provider virtually through CareFirst Video Visit at no cost. In fact, all services through Video Visit are now free!

• Free virtual physical therapy—The new Hinge Health program, available to CareFirst medical plan participants, ofers free virtual physical therapy exercises that you can do in your home on your schedule

• More free procedures through Carrum Health—The Carrum Health no-cost surgery benefts have expanded to include more types of surgeries and new cancer support

Changes to what you pay for medical benefts With healthcare costs continuing to rise nationwide, there will be an increase in 2022 medical rates and some changes to the CareFirst medical plan benefts. Changes to other medical options’ rates vary. What doesn’t change is that Sodexo will still pay for the majority of medical costs.

EyeMed Vision plan, get better benefts when you use vision providers who are in the Eye360 network. Also, see the enhancements to benefts for progressive lenses

• Increased company-paid life insurance for full-time hourly employees—Under Free Basic Life, the beneft for hourly employees will increase from $10,000 to $25,000

• Dental rates go down for the third year in a row—We reduced our dental rates while keeping our dental plan benefts in the top quartile for our industry

• Most rates will stay the same for 2022 with the exception of medical coverage

To get the app, log into the Sodexo Benefts Center website and add your phone number under your profle. Then, download the app and search for Sodexo. Follow the prompts to get set up. Health Take Care & Stay Well

Medical You have three national medical plan options administered by CareFirst BlueCross BlueShield—the Health Reimbursement Account (HRA), Preferred Provider Organization (PPO) and Catastrophic Plan. Each plan has a Blue Rewards Account for incentives you earn to help pay for eligible healthcare expenses. See page 14 for details.

Depending on where you live, you may be eligible for one of the regional medical plans, which include the Kaiser Permanente HMO, HMSA PPO and HMO, and Triple-S Salud.1

1 If you live in California, Colorado, Georgia, Hawaii or the Mid-Atlantic region (DC/MD/VA), you have Kaiser Permanente HMO as an additional medical plan option. HMSA plans are available in Hawaii, and Triple-S Salud is only available in Puerto Rico. The CareFirst HRA and PPO Plans cover the same eligible services, allow you to see any provider you want (you’ll pay less with in-network providers) and include prescription drug coverage (see page 9).

The HRA Plan comes with a Health Reimbursement Account (HRA) that Sodexo contributes money to. You can use it to help pay for eligible $0 (no deductible) health expenses until you meet your deductible.

The CareFirst Catastrophic Medical Plan provides coverage if you experience a catastrophic illness or injury, such as from a car accident or heart attack. Potentially life-threatening events like these are costly and might cause you to meet your deductible and out-of-pocket maximum.

Under this medical plan, you pay all medical expenses until you meet your deductible and out-of-pocket maximum (which are combined into one amount). This means you will pay 100% of the cost of regular medical services—such as doctor visits, prescriptions, specialist visits and elective surgeries—until you have met your deductible and out-of-pocket maximum. Once you meet these, the plan will pay 100% toward covered services for the remainder of the calendar year.

Surgery can be complicated and costly. Also, not all medical providers deliver the same quality of care and sometimes surgery can even be avoided altogether. That’s why Sodexo ofers a no-cost surgery beneft through Carrum Health for employees and their dependents enrolled in a CareFirst medical plan.

Through this beneft, you and your enrolled family members (age 18+) can access high-quality surgeons and Centers of Excellence (COE) hospitals. All of the surgery costs and most travel expenses, if necessary, are covered. Plus, you’ll have a dedicated patient Care Specialist to help you throughout the entire process. Covered surgeries under this Carrum Health beneft include knee, hip, shoulder, back, neck and elbow. For 2022, weight loss and heart surgery will also be included. New

Important Benefits Details As you choose or update your 2022 benefts, there will be a few things to keep in mind.

If you do nothing during Annual Enrollment You’ll remain in the same plans you currently have at the same levels of coverage. You will not have an HCSA or FCSA, because you must re-enroll in these accounts each year.

Nicotine surcharge If you’re enrolling in medical or group term life insurance for the frst time, you must disclose any nicotine use. If you answer “yes,” you’ll be charged an annual $1,200 nicotine surcharge ($23.98 per week).1 If you use nicotine products and wish to quit, consider enrolling in a smoking cessation program. Most Sodexo medical options ofer free programs to help you quit. Check your medical plan or contact LifeWorks for more information.

Adding new dependents If you add dependents to your coverage, you’ll need to provide proof of eligibility. This can be done with a marriage certifcate, domestic partner afdavit, adoption papers or birth certifcates.

New ID cards ID cards will be sent to you if you enroll in the medical, prescription drug and vision plans. If you enroll in the HRA Plan, you’ll also receive an HRA prescription debit card. If you enroll in the HCSA and choose the debit card payment option, a debit card will be mailed to you.

Life events If you experience a qualifying life event during the plan year, such as marriage, birth, adoption, divorce, etc., you may be eligible to enroll for the frst time or change your enrollment. You must request the change within 30 days of the event. You can schedule an appointment with a customer service representative at SodexoBeneftsCenter.com.

Employees in Hawaii If you cancel or decline Sodexo medical coverage, you must complete a Hawaii State Waiver Form and return it to the Sodexo Benefts Center. You can call the Sodexo Benefts Center at 855-668-5040 to request the form, or it will be sent to you if you do not make an election for medical coverage.

If the form is not completed and received by the Sodexo Benefts Center by the date your benefts would normally begin, you will be automatically enrolled in the Kaiser Permanente HMO with employee-only level of coverage, and you will be responsible for paying the cost of that coverage. If you are automatically enrolled, you will not be able to cancel your coverage until the next Annual Enrollment unless you experience a qualifying event.

If you lose coverage If you and your dependents have health coverage through another employer and lose it, you may be eligible to enroll in coverage through Sodexo. You must request enrollment within 30 days afer your other coverage ends or afer the other employer stops contributing. You can schedule an enrollment appointment with a customer service representative at SodexoBeneftsCenter.com.
