It seems like the end of the 2015-16 school year came so fast and the summer just started. Our graduating students of the Class of 2016 are onto their new chapters of their life and I wish them much luck in their future endeavors. We have started the preparations for the 2016-17 school year.

I hope everyone has been enjoying and relaxing this summer and you are ready for the start of the new school year. I have been working on many projects this summer to get ready for the upcoming school year. We have hired three new teachers Ms. Michelle Herrman for Ag; Mr. Ted Kohler for Special Education; and Mrs. Elizabeth Torbenson for MS/HS Guidance Counselor. Many meetings were held to help plan for activities for next year. We have been looking at data, updating the current student handbooks, and reviewing some of our programs as to how the changes that we made last year worked out and where we need to make more improvements. I am currently looking at some of the transcripted credit we offer right here at Bowler as well as our ERVING classes (Distance Learning), Youth Options, Career Options and Youth Apprenticeship opportunities and moving forward to continue to improve opportunities for our students to be career and college ready. The schedule is complete for the 2016-17 school year, and in the near future you will be receiving a letter home and copies of your son/daughter’s schedule for the upcoming school year. Student registration is set up for Thursday, August 18, 2016 from 9:00 AM-6:00 PM. Please make sure to bring your son/daughter in to get registered, pay fees, pick up schedules, student handbook and double check schedules to make sure they are ready for the start of the 2016-17 school year. This would be a great time to meet the new guidance counselor, to bring in school supplies, and if you are a new student or 7th grade student to come and find the classrooms you will be attending. Enjoy your summer; the school year will begin right around the corner. I am positive that our school year will be great and am looking forward to seeing everyone soon!

Hi! I would like to take this opportunity to introduce myself as the 7th-12th grade school counselor for the Bowler School District. I am happy to be joining the supportive staff here, and I look forward to getting to know the students and Bowler community better. As the school counselor, I seek to address the developmental needs of all students through a comprehensive school counseling program in the areas of academic, career, and personal/social development through individual and group counseling, classroom guidance, parent and teacher consultations, referral assistance to other programs and services in the community, and student testing. I believe that every student has the ability to succeed academically, build personal and social skills that enhance their well-being, and understand various career opportunities through an encouraging school environment and school counseling program. I plan to provide a safe and comforting place for students to come to as they need assistance. My focus as a counselor is to help the students succeed academically and gain personal awareness of their strengths and abilities. I believe that knowing how to advocate for oneself in all circumstances brings about success and personal growth, which is an area I am passionate about and will use as a foundation when working with students. I know that collaborating with school staff, parents, and being involved in the community all play integral roles when developing and maintaining a successful school counseling program. I also recognize the necessity of maintaining positive relationships with students, school staff and parents because these relationships are at the heart of the counseling program; as this communal strength is created, students' benefit and the environment becomes conducive to individual growth and learning. If you have any questions, comments and/or concerns, please do not hesitate to call or email me! I am excited for the school year to begin and, again, I look forward to meeting each of you! Sincerely, Liz Torbenson, MA School Counselor

The Bowler High School Chapter National Honor Society is working toward saving 1 MILLION pop tops. When we achieve this great goal, we will be taking them to The Ronald McDonald House in Milwaukee. The tabs will be turned into cash at a recycling center which will provide a “home away from home” for families whose lives have been disrupted as a result of their child’s illness or injury. Currently we have over 301,333 tops and have reached our goal of 300,000 at the end of the school year. Thank you to all who have contributed tops.

FOR SAFETY REASONS, PLEASE MAKE SURE THAT

YOUR CHILD HAS TENNIS SHOES FOR PHYSICAL EDUCATION CLASS.

AN OLD PAIR OF TENNIS SHOES THAT THEY CAN KEEP IN THEIR

A Note from your School Nurse Medications: Giving necessary medications to students at school is an important part of what I supervise for your school district. Our staff is happy to work with parents to be sure that medications are given correctly and safely. Here is a sampling of typical medications that may be necessary:

x Daily oral tablets for specific medical conditions x Short term oral tablets, such as antibiotics or pain medications, for specific brief health

conditions x Inhalers or nebulizer treatments for asthma x Eye drops x Topical creams prescribed for specific skin conditions x Insulin injections for diabetes x Epipens (epinephrine injection) for severe allergic reactions x Diastat for prolonged seizures

Keep us informed about medications that your child may need during the school day. We will ask you to complete a consent form, and will assist you to obtain signed medical authorization for medication if it has been prescribed by a doctor or clinic provider. Most medications are kept in a secure location in the school office, with a few exceptions such as asthma inhalers for older students. Immunizations: The Wisconsin Immunization law requirements are included in this edition. Eating & Feeding Restrictions: This form has also been included in this edition. If your child needs alternate foods at school (such as lactose free milk, or substitute foods due to food allergies, etc.) please complete this form and ask your doctor or clinic provider to sign it. This helps us share the information with all necessary staff, and also helps food service staff with the proof needed when alternate foods must be purchased. Questions? CHILDREN WITH DISABILITIES AND SPECIAL DIETARY RESTRICTIONS

A. Rehabilitation Act of 1973 and the Americans with Disabilities Act Under Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act Amendments Act (ADAAA) of 2008, “a person with a disability” means any person who has a physical or mental impairment which substantially limits one or more major life activities or major bodily functions, has a record of such an impairment, or is regarded as having such an impairment. Major life activities include, but are not limited to, caring for oneself, performing manual tasks, seeing, hearing, eating, sleeping, walking, standing, lifting, bending, speaking, breathing, learning, reading, concentrating, thinking, communicating, and working. A major life activity also includes the operation of a major bodily function, including but not limited to, functions of the immune system, normal cell growth, digestive, bowel, bladder, neurological, brain, respiratory, circulatory, endocrine, and reproductive functions.

Please refer to these Acts for more information at http://www.dol.gov/oasam/regs/statutes/sec504.htm and http://www.eeoc.gov/laws/statutes/adaaa.cfm, respectively. B. Individuals with Disabilities Education Act A child with a “disability” under Part B of the Individuals with Disabilities Education Act (IDEA) is described as a child evaluated in accordance with IDEA as having one or more of the recognized thirteen disability categories and who, by reason thereof, needs special education and related services. The IDEA can be found in its entirety at http://nichcy.org/wp-content/uploads/docs/IDEA2004regulations.pdf. The Individualized Education Program (IEP) is a written statement for a child with a disability that is developed, reviewed, and revised in accordance with the IDEA and its implementing regulations. When nutrition services are required under a child's IEP, school officials need to make sure that school food service staff is involved early in decisions regarding special meals. A copy of the completed Dietary Request Form (page 2 of this document) must be kept on file to support any modifications made to students’ meals, even if they are detailed in the IEP. C. Medical Practitioner’s Statement for Children with Disabilities U.S. Department of Agriculture (USDA) regulations 7 CFR Part 15b require substitutions or modifications in school meals for children whose disabilities restrict their diets. A child with a disability must be provided substitutions in foods when that need is supported by a statement signed by a medical practitioner. The practitioner’s statement must identify:

x the child's disability; x an explanation of why the disability restricts the child's diet; x the major life activity operation of a major bodily function affected by the disability; and x the food or foods to be omitted from the child's diet and the food or choice of foods that must be substituted.

This form may be used to obtain the required information from the medical practitioner. “Practitioner” is defined by Wisconsin State Statute 118.29(1) (e): “Practitioner” means any physician, dentist, optometrist, physician assistant, advanced practice nurse prescriber, or podiatrist licensed in any state. Unless the documentation to support the required dietary accommodation has been signed by one of these practitioners, the school is not required to accommodate the request. D. Other Special Dietary Needs School food service staff may make food substitutions, at their discretion, for individual children who do not have a disability. The school should have a completed Dietary Request Form (page 2 of this document) on file from any medical authority certifying the student as having a special medical or dietary need. Such determinations are only made on a case-by-case basis and all accommodations must be made according to USDA’s meal pattern requirements. For more information, please see the USDA guidance: Accommodating Children with Special Needs: Guidance for School Food Service Staff at http://www.fns.usda.gov/cnd/Guidance/special_dietary_needs.pdf.

Meningococcal Disease: Protect Your Child

Public health authorities recommend that teenagers and college-bound students be immunized against a potentially fatal bacterial infection called meningococcal disease, a type of meningitis. Meningococcal disease is a rare but potentially fatal bacterial infection that can cause severe swelling of the brain and spinal cord (meningitis) or a serious blood infection (meningococcemia). Meningococcal disease strikes up to 3,000 Americans each year; nearly 30 percent of these cases are among teenagers and college students. Up to 83 percent of all cases among teens and college students may potentially be prevented through immunization, the most effective way to prevent this disease. A meningococcal vaccine is available that protects against four out of five strains of bacterium that cause meningococcal disease in the U.S. The Centers for Disease Control and Prevention (CDC) and other leading medical organizations recommends that all 11-12 years olds should be vaccinated with meningococcal conjugate vaccine (MCV4). A booster shot is recommended for teens at age 16 to continue providing protection when their risk for meningococcal disease is highest. Teens who received MCV4 for the first time at age 13 through 15 years will need a one-time booster dose at 16 through18 years of age. If a teenager missed getting the vaccine altogether, they should ask the doctor about getting it now, especially if they are about to move into a college dorm or military barracks.

About Meningococcal Disease Meningococcal disease is often misdiagnosed as something less serious because early symptoms are similar to common viral illnesses. Symptoms of meningococcal disease may include high fever, severe headache, stiff neck, nausea, vomiting, sensitivity to light, confusion, exhaustion and/or a rash. Teenagers and college students are at increased risk for meningococcal disease compared to the general population, accounting for nearly 30 percent of all U.S. cases every year. Meningococcal disease can be misdiagnosed as something less serious, because early symptoms like high fever, severe headache, nausea, vomiting and stiff neck, are similar to those of common viral illnesses. The disease can progress rapidly and can cause death or permanent disability within 48 hours of initial symptoms. Meningococcal disease is spread through direct contact with respiratory and/or oral secretions from infected persons (for example, kissing or sharing drinking containers). It can develop and spread quickly throughout the body, so early diagnosis and treatment are very important. Even with immediate treatment, the disease can kill an otherwise healthy young person within hours of first symptoms. Of those who survive, up to 20 percent may endure permanent disabilities, including brain damage, deafness and limb amputations. Lifestyle factors common among teenagers, college students and military personnel are believed to put them at increased risk of contracting meningococcal disease. These lifestyle factors include crowded living situations (for example, dormitories, sleep-away camps), active or passive smoking and irregular sleeping habits. Teens should avoid sharing eating utensils and drinking out of the same container, since infections may spread through this type of close contact. STEP 2: DO ANY HOUSEHOLD MEMBERS (INCLUDING YOU) CURRENTLY PARTICIPATE IN ONE OR MORE OF THE FOLLOWING ASSISTANCE PROGRAMS: FOODSHARE, W-2 CASH BENEFITS, OR FDPIR?

A) IF NO ONE IN YOUR HOUSEHOLD PARTICIPATES IN ANY OF THE ABOVE LISTED PROGRAMS:

x Circle ‘NO’ and skip to STEP 3 on these instructions and STEP 3 on your application.

x Leave STEP 2 blank. B) IF ANYONE IN YOUR HOUSEHOLD PARTICIPATES IN ANY OF THE ABOVE LISTED PROGRAMS:

x Circle ‘YES’ and provide a case number for FoodShare, W-2 Cash Benefits, or FDPIR. You only need to write one case number. Please note: A BadgerCare case number cannot be used on this application, only the programs listed above.

x Skip to STEP 4, leave STEP 3 blank.

STEP 3: REPORT INCOME FOR ALL HOUSEHOLD MEMBERS A) Report all income earned by children. Refer to the chart titled “Sources of Income for Children” in these instructions and report the combined gross income for ALL children listed in Step 1 in your household in the box marked “Child Income.” Only count foster children’s income if you are applying for them together with the rest of your household. It is optional for the household to list foster children living with them as part of the household.

Sources of Income for Children Sources of Child Income Example(s)

x Earnings from work x A child has a job where they earn a salary or wages.

x Social Security o Disability Payments o Survivor’s Benefits

x A child is blind or disabled and receives Social Security benefits.

x A parent is disabled, retired, or deceased, and their child receives social security benefits.

x Income from any other source x A child receives income from a private pension fund, annuity, or trust.

What is Child Income?

Child income is money received from outside your household that is paid directly to your children. Many households do not have any child income. Use the chart below to determine if your household has child income to report.

If anyone in your household participates in the assistance programs listed below, your children are eligible for free school meals:

x The Supplemental Nutrition Assistance Program (SNAP) or FoodShare x Temporary Assistance for Needy Families (TANF) or W-2 Cash Benefits x The Food Distribution Program on Indian Reservations (FDPIR)

3 How to Apply for School Meal Benefits| Application Instructions

FOR EACH ADULT HOUSEHOLD MEMBER:

B) List Adult Household member’s name. Print the name of each household member in the boxes marked “Names of Adult Household Members (First and Last).” Do not list any household members you listed in STEP 1. If a child listed in STEP 1 has income, follow the instructions in STEP 3, part A. C) Report earnings from work. Refer to the chart titled “Sources of Income for Adults” in these instructions and report all income from work in the “Earnings from Work” field on the application. This is usually the money received from working at jobs. If you are a self-employed business or farm owner, you will report your net income.

D) Report income from Public Assistance/Child Support/Alimony/SSI/VA Benefits. Refer to the chart titled “Sources of Income for Adults” in these instructions and report all income that applies in the “Public Assistance/Child Support/Alimony/SSI/VA Benefits” field on the application. Do not report the value of any cash value public assistance benefits NOT listed on the chart. If income is received from child support or alimony, only court-ordered payments should be reported here. Informal but regular payments should be reported as “other” income in the next part.

What if I am self-employed?

If you are self-employed, report income from that work as a net amount. This is calculated by subtracting the total operating expenses of your business from its gross receipts or revenue.

Who should I list here?

When filling out this section, please include all members in your household who are: x Living with you and share income and expenses, even if not related and even if they do not

receive income of their own.

Do not include people who: x Live with you but are not supported by your household’s income and do not contribute

income to your household. x Children and students already listed in Step 1.

How do I fill in the income amount and source?

FOR EACH TYPE OF INCOME: x Use the charts in this section to determine if your household has income to report.

x Report all amounts in gross income ONLY. Report all income in whole dollars. Do not

include cents. o Gross income is the total income received before taxes or deductions. o Many people think of income as the amount they “take home” and not the total,

“gross” amount. Make sure that the income you report on this application has NOT been reduced to pay for taxes, insurance premiums, or any other amounts taken from your pay.

x Write a “0” in any fields where there is no income to report. Any income fields left empty or blank will be counted as zeroes. If you write ‘0’ or leave any fields blank, you are certifying (promising) that there is no income to report. If local officials have known or available information that your household income was reported incorrectly, your application will be verified for cause.

x Mark how often each type of income is received using the check boxes to the right of each field.

E) Report income from Pensions/Retirement/Social Security/All other income. Refer to the chart titled “Sources of Income for Adults” in these instructions and report all income that applies in the “Pensions/Retirement/Social Security, Other Income” field on the application. F) Special Situations. For seasonal workers and others whose income fluctuates and usually earn more money in some months than others. In these situations, project the annual rate of income and report that. This includes workers with annual employment contracts but may choose to have salaries paid over a shorter period of time. This includes school employees. G) Report total household size. Enter the total number of household members in the field “Total Household Members (Children and Adults).” This number MUST be equal to the number of household members listed in STEP 1 and STEP 3. If there are any members of your household that you have not listed on the application, go back and add them. It is very important to list all household members, as the size of your household determines your income cutoff for free and reduced price meals.

H) Provide the last four digits of your Social Security Number. The household’s primary wage earner or another adult household member must enter the last four digits of their Social Security Number in the space provided. You are eligible to apply for benefits even if you do not have a Social Security Number. If no adult household members have a Social Security Number, leave this space blank and mark the box to the right labeled “Check if no SSN.”

1. WHO CAN GET FREE OR REDUCED PRICE MEALS? x All children in households receiving benefits from FoodShare, The Food Distribution Program on Indian

Reservations (FDPIR)], or W-2 cash benefits are eligible for free meals. x Foster children that are under the legal responsibility of a foster care agency or court are eligible for free meals. x Children participating in their school’s Head Start program are eligible for free meals. x Children who meet the definition of homeless, runaway, or migrant are eligible for free meals. x Children may receive free or reduced price meals if your household’s income is within the limits on the Federal

Income Eligibility Guidelines. Your children may qualify for free or reduced price meals if your household income falls at or below the limits on this chart.

2. HOW DO I KNOW IF MY CHILDREN QUALIFY AS HOMELESS, MIGRANT, OR RUNAWAY? Do the members of your household lack a permanent address? Are you staying together in a shelter, hotel, or other temporary housing arrangement? Does your family relocate on a seasonal basis? Are any children living with you who have chosen to leave their prior family or household? If you believe children in your household meet these descriptions and have not been told your children will get free meals, please call or e-mail Wade Turner, Homeless Liaison at 715-793-4101 ext. 5302 or email [email protected]

3. DO I NEED TO FILL OUT AN APPLICATION FOR EACH CHILD? No. Use one Free and Reduced Price School Meals Application

for all students in your household. We cannot approve an application that is not complete, so be sure to fill out all required information. Return the completed application to: Linda Brend, District Accountant, 500 S. Almon St., Bowler, WI 54416.

4. SHOULD I FILL OUT AN APPLICATION IF I RECEIVED A LETTER THIS SCHOOL YEAR SAYING MY CHILDREN ARE ALREADY APPROVED FOR FREE MEALS? If any children in your household were missing from your eligibility notification, contact Linda Brend, District Accountant, 715-793-4101 or email [email protected] immediately.

application. However, this information is necessary for other programs and may be used to determine if your household is eligible for additional benefits.

6. MY CHILD’S APPLICATION WAS APPROVED LAST YEAR. DO I NEED TO FILL OUT A NEW ONE? Yes. Your child’s application is only good for that school year. You must send in a new application unless the school told you that your child is eligible for the new school year. If you do not send in a new application that is approved by the school or you have not been notified that your child is eligible for free meals, your child will be charged the full price for meals.

7. I GET WIC. CAN MY CHILDREN GET FREE MEALS? Children in households participating in WIC may be eligible for free or reduced price meals. Please send in an application.

8. WILL THE INFORMATION I GIVE BE CHECKED? Yes. We may also ask you to send written proof of the household income you report.

9. IF I DON’T QUALIFY NOW, MAY I APPLY LATER? Yes, you may apply at any time during the school year. For example, children with a parent or guardian who becomes unemployed may become eligible for free and reduced price meals if the household income drops below the income limit.

10. WHAT IF I DISAGREE WITH THE SCHOOL’S DECISION ABOUT MY APPLICATION? You should talk to school officials. You also may ask for a hearing by calling or writing to: Faith M. Gagnon, 500 S. Almon St. Bowler, WI 54416, 715-793-4101

11. MAY I APPLY IF SOMEONE IN MY HOUSEHOLD IS NOT A U.S. CITIZEN? Yes. You, your children, or other household members do not have to be U.S. citizens to apply for free or reduced price meals.

12. WHAT IF MY INCOME IS NOT ALWAYS THE SAME? List the amount that you normally receive. For example, if you normally make $1000 each month, but you missed some work last month and only made $900, put down that you made $1000 per month. If you normally get overtime, include it, but do not include it if you only work overtime sometimes. If you have lost a job or had your hours or wages reduced, use your current income.

13. WHAT IF SOME HOUSEHOLD MEMBERS HAVE NO INCOME TO REPORT? Household members may not receive some types of income we ask you to report on the application, or may not receive income at all. Whenever this happens, please write a 0 in the field. However, if any income fields are left empty or blank, those will also be counted as zeroes. Please be careful when leaving income fields blank, as we will assume you meant to do so.

14. WE ARE IN THE MILITARY. DO WE REPORT OUR INCOME DIFFERENTLY? Your basic pay and cash bonuses must be reported as income. If you get any cash value allowances for off-base housing, food, or clothing, it must also be included as income. However, if your housing is part of the Military Housing Privatization Initiative, do not include your housing allowance as income. Do not include any combat pay resulting from deployment as income.

15. WHAT IF THERE IS NOT ENOUGH SPACE ON THE APPLICATION FOR MY FAMILY? List any additional household members on a separate piece of paper and attach it to your application. Contact Linda Brend, District Accountant, 500 S. Almon St., Bowler, WI 54416 or 715-793-4101 or email [email protected] to receive a second application.

16. MY FAMILY NEEDS MORE HELP. ARE THERE OTHER PROGRAMS WE MIGHT APPLY FOR? To find out how to apply for FoodShare or other assistance benefits, contact your local assistance office or call 1-800-362-3002.

If you have other questions or need help, call 715-793-4101 ext. 6312.

Sincerely,

Linda Brend, District Accountant School District of Bowler 500 South Almon Street Bowler, WI 54416

PUBLIC RELEASE NATIONAL SCHOOL LUNCH AND BREAKFAST PROGRAMS, SPECIAL MILK PROGRAM

RELEASE STATEMENT

The School District of Bowler today announced its policy for children unable to pay the full price of meals served under the National School Lunch Program and School Breakfast Program or milk for split-session students served under the Special Milk Program. Each school office and the central office has a copy of the policy, which may be reviewed by any interested party.

The following household size and income criteria will be used for determining eligibility. Application forms are being sent to all homes with a notice to parents or guardians. To apply for free or reduced price meals or free milk, households must fill out the application and return it to the school (unless notified at the start of the school year that children are eligible through direct certification). Additional copies are available at the office in each school. The information provided on the application will be used for the purpose of determining eligibility and may be verified at any time during the school year by agency or other program officials. Applications may be submitted at any time during the year. To obtain free or reduced price meals or free milk for children in a household where one or more household members receive FoodShare, FDPIR, or Wisconsin Works (W-2) cash benefits, list the household member and the FoodShare, FDPIR or W-2 case number, list the names of all school children, sign the application, and return it to the school office. For the school officials to determine eligibility for free or reduced price meals or free milk of households not receiving FoodShare, FDPIR or W-2 cash benefits, the household must provide the following information requested on the application: names of all household members and the adult signing the application form must also list the last four digits of his or her Social Security Number or mark the box to the right of “Check if no SSN”. Also, the income received by each household member must be provided by amount and source (wages, welfare, child support, etc.). Under the provisions of the free and reduced price meal and free milk policy Linda Brend, District Accountant, will review applications and determine eligibility. If a parent or guardian is dissatisfied with the ruling of the official, he/she may wish to discuss the decision with the determining official on an informal basis. If the parent/guardian wishes to make a formal appeal, he/she may make a request either orally or in writing to: Faith Gagnon, District Administrator, 500 S. Almon St. Bowler, WI 54416, 715-793-4101. If a hearing is needed to appeal the decision, the policy contains an outline of the hearing procedure. If a household member becomes unemployed or if the household size changes, the family should contact the school. Such changes may make the household eligible for reduced price meals or free meals or free milk if the household income falls at or below the levels shown above, and they may reapply at that time. Children formally placed in foster care are also eligible for free meal benefits. Foster children may be certified as eligible without a household application. Households with foster children and non-foster children may choose to include the foster child as a household member, as well as any personal income available to the foster child, on the same application that includes their non-foster children. The information provided by the household on the application is confidential. Public Law 103-448 limits the release of student free and reduced price school meal eligibility status to persons directly connected with the administration and enforcement of federal or state educational programs. Consent of the parent/guardian is needed for other purposes such as waiver of text book fees. Non-discrimination Statement: In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA. Persons with disabilities who require alternative means of communication for program information (e.g. Braille, large print, audiotape, American Sign Language, etc.), should contact the Agency (State or local) where they applied for benefits. Individuals who are deaf, hard of hearing or have speech disabilities may contact USDA through the Federal Relay Service at (800) 877-8339.
