SAFETY MANAGEMENT AUDIT SYSTEM

1.0 Safety Organization

1.11 Safety Policy (250)

Objective: To establish, maintain and continuously improve the

Organizations occupational health and safety management system.

Does the organization have a written Health and Safety Policy Statement?

a. Does it include statement of commitment, signed and dated by MD?

b. Is the organization for health and safety clearly defined with assigned responsibilities and are they clearly understood by all in the organization?

c. Are all H&S arrangements set out in the policy? (Separate procedure manuals or rule books are acceptable)?

d. Is the policy publicized, known and understood by all employees? Must from part of induction training.

e. Is the policy annually reviewed?

f. Is the policy effectively implemented?

1.12 Appointment of Competent OH & S Assistance: (50)

Objective: To assist in the monitoring, directing, and co-ordination of the OH&S system.

Have competent persons been appointed to assist management with the implementation of H & S measures? Use of external agencies acceptable if measures are taken to ensure competence?

a. Are the duties clearly defined in writing and recorded in the policy?

b. Are they trained to a recognised standard?

c. Is further training provided to meet companys particular needs, e.g. COSHH, Manual Handling, and CDM?

1.13 Responsibilities, Authority and Objectives. (50)

Objective: To clearly identify designated responsibility and

authority throughout the arrangement structure for the achievement of the policy objectives.

a. Has a director/ Senior Manager been designated responsible for health and safety with sufficient authority?

b. Have the roles and the responsibilities been assigned in the policy for; Department Managers; Supervisors, Foreman, are they known and understood?

c. Are safety tours undertaken by senior and department managers, do they endorse pertinent documents (Assessments, reports, minutes 7 records)?

d. Are H & S responsibilities reviewed and assessed as a part of staff appraisal?

e. Are employees responsibilities and duties set out in the safety policy?

1.14 Employee Consultations: (30)

Objective: To ensure employee involvement and consultation arrangements are documented and effective.

a. Are consultation arrangements set out in the safety policy?

b. Is there an agreed procedure for raising H & S issues and discussing inspection reports?

c. Have safety reps been appointed to cover all areas?

d. Are their roles and functions defined?

e. Is health and safety training available for their purpose?

1.15 Safety Committees: (30)

Objective: To have a structure in place which enables OH&S matters to be discussed, actioned and reviewed through the involvement of employees.

a. Has a safety committee been established with an agreed constitution and role, which meets monthly?

b. Is an agenda circulated which includes monitoring the H&S plan, audits and inspections, accident statistics, H&S training, risk assessments and development/review of H&S arrangements, H&S promotion?

c. Are minutes circulated and displayed?

d. Are minutes endorsed by designated senior manager?

1.16 Department H & S Communication: (40)

Objective: To have procedures in place for ensuring that OH&S information is communicated to and from employees and other interested parties.

a. Are monthly H & S briefings held by department managers/supervisors with individuals or groups?

b. Are topics and attendance recorded?

c. Are briefings held prior to the introduction of new equipment, systems or processes?

d. Are method statements discussed prior to undertaking hazardous or non-standard operations?

1.17 First Aid personnel and facilities: (50)

Objective: To ensure that adequate arrangements are in place andmaintained in accordance with the nature and scale of the organization's OH&S risks.

a. Has an assessment of first aid requirements been undertaken and is it reviewed annually?

b. Is the provision of First Aid staff and facilities adequate for the companys activities, including cover for shifts, lone and peripatetic workers?

c. Are first aid facilities easily accessible?

d. Are facilities checked monthly by a nominated person and recorded?

(Checklist and guidance card should be provided in each first aid box).

e. Is a record kept of all cases treated?

1.18 First Aid Training (30)

Objective: To ensure that employees are competent to perform the tasks associated with first aid.

a. Are all first aid personnel trained to approved standard. ( Atleast four full days, six contract hours per day).

b. Refresher training provided within three years (Atleast 2 day refresher)?

c. Special hazard training provided where necessary?

d. Does the company encourage all employees to undertake emergency first aid training, with regular refresher training?

1.19 Health and Safety Promotion: (10)

Objective: To encourage a culture which supports and develops health and safety in the workplace.

a. Is health and safety promotion an item on the safety committee agenda?

b. Is the information from accident statistics audit/inspection reports, national campaigns used to help develop themes for company promotional activities?

c. Are there appropriate means of promotion, e.g bulletins, posters, attitude guides, competitions?

Publication of H & S Achievements and statistics:

Objective:

a. Are company safety achievements displayed?

( 5 Star Awards, Sword of Honour)

b. Are accident/incident rates published and displayed?

c. Are road accidents recorded, published and displayed?

Suggestion Scheme:

a. Is there a procedure through which employees may put forward ideas to improve health and safety standards?

b. Are suggestions recorded and acknowledged?

c. Are successes published?

Incident/Accident Recall:

a. Is the incident recall system in use?

b. Are all the talks confidential?

c. Are the work force encouraged to take part and understand why?

d. Is the recall system linked with the toolbox talks?

e. Are the results actioned, monitored and made known?

1.20 Quality System (70)

Objective: There is a direct correlation of quality systems with health and safety. Incorporating the principles will assist in the implementation of health and safety into the workplace.

a. Is the OH&S a part of quality system?

b. Is it periodically reviewed and audited by the qualified auditors (Internal / external)?

c. Is the OH&S principles incorporated and implemented?

1.21 Continuous improvements (30)

Objective: To continuous review the OH&S systems to ensure continuing suitability, adequacy and effectiveness.

a. Is OH&S system reviewed periodically?

b. Are the suggestions / recommendations implemented suitably?

c. Is it reviewed for adequacy and effectiveness?

1.22 Safety Reference Library: (15)

Objective: To ensure that OH&S information is up to date and available to all employees.

a. Is there a library, accessible to staff?

b. Is a list of publications maintained and circulated?

c. Does it contain relevant findings of risk assessments?

d. Is it regularly updated?

1.23 Annual Reports (Loss control achievements): (20)

Objective: To recognize and promote achievements in OH&S.

a. Is an annual report prepared by the Safety Advisor/Departmental Managers for the Directors?

b. Does it include a plan of action for the coming year?

c. Is the report and action plan approved by the directors?

d. Are the company achievements publicised and make known to employees, shareholders and customers?

1.24 Induction competence and job safety training: (130)

Objective: To ensure that employees have the necessary training, education and skills to reduce the level of risk as much as possible. This will also enable employees to identify potential losses and assist in the creation of a health and safety culture.

a.Is induction training provided at the start of employment?

b. Is formal induction undertaken to set lesson plans?

c. Is information, instruction and training provided in the work area covering relevant safety rules and procedures?

d. Is all necessary specific job safety training provided? (e.g. Lift Trucks. Abrasive Wheels, Power Presses, Cranes, Slinging, Hazardous Substances, machine operation, DSE).

Is all training recorded?

1.25 Approved Safety Training: (90)

Objective: To continuously develop the knowledge and skills of employeesthrough structured OH&S education and training.

a. Are training requirements reviewed and assessed atleast annually?

b. Is there a H & S training programme for all managerial and supervisory cadre staff ( including senior mangers)?

c. Is refresher/update training provided?

d. Are employees representatives trained?

e. Are records maintained?

1.26 Occupational Health Programme: (120)

Objective: To ensure the health, safety and welfare of employees and other people so that they are not exposed to risk to their health and safety from the employer's work.

a. Does the company have access to competent Occupational Health Advise ( App. Doctor/OCC. Health Nurse)

b. Are pre-employment medical check up given to prospective employees/pre-employment questionnaire used?

c. Is Health surveillance carried out where required?

d. Is Occupational Health advice sought following serious injury/illness regarding fitness to resume work?

e. Does the company operate a Wellness programme for employees?

f. Do the employees have access to stress counseling?

g. Do you have a programme for dealing with drug and alcohol abuse?

1.27 Recruitment and appointment procedures: (25)

Objective: To ensure the selection process has considered all the aspects with relation to the ability of the person, their health, capabilities and the type of work to be carried out.

a. Are recruitment personnel trained in interviewing techniques?

b. Do interviewers have access to written job descriptions, which identify the physical and mental capabilities required to undertake the work safely?

c. Are records maintained of interviews?

1.28 Safety Inspections: (30)

Objective: To confirm that all safety measures are in place and when deviations are found, they are dealt with quickly.

a. Are safety inspections carried out monthly by safety representatives?

b. Are all areas of the company premises inspected, including external and common service areas?

c. Is inspection check list used and specific to each area?

d. Is the inspection report signed for by and discussed with a designated manager?

e. Is remedial action initiated by the manager and recorded?

1.29 Safety Audits: (40)

Objective: To appraise the effectiveness in the workplace of OH&S management system.

a. Are all areas of the company audited annually?

b. Is the audit comprehensive (cover all elements of the Five Star programme) of documented safety standards?

c. Are auditors trained?

d. Is audit report submitted to and considered by senior management?

e. Are the results published?

f. Are audit results used to assist in the development of the safety action plan?

1.30 Procedure for the procurement of new plant and equipment: (50)

Objective: To have in place effective health and safety systems for the procurement of plant, equipment and materials to eliminate or reduce the risks to the organization and employees.

a. Are safety standards established for the purchase of all new plant and equipment?

b. Are nominated competent persons assigned to authroise purchases?

c. Are Safety advisors involved in establishing standards?

d. Are precommissioning checks undertaken?

e. Is a maintenance programme established and log book provided for new plant and eqauipment.

1.31 Product / Service Safety: (40)

Objective: To ensure that the products and services meet legal requirements and minimize the risks to the customer and company.

a. Is there a product Quality System in operation?

b. Is the product investigated and researched for its safe use?

c. Are records of tests maintained?

d. Can the product present a danger when discarded?

e. Has a procedure been installed for product recall?

1.32 Appointment and Control of Contractors: (100)

Objective: To ensure that there are systems and controls in place to reduce the exposure of risk to find parties and organizations assets and employees.

a. Is there a procedure of assessing contractors H&S standards?

b. Are Contractors required to submit copies of their Safety Policy, accident statistics, risk assessments and methods statements?

c. Are prework meetings held to establish safe working procedures?

d. Are contractors made aware of hazards and measures required to control (induction training or safety rules provided)

e. Are visitors/contractors made aware of emergency procedures?

f. Is a record kept of all visitors/contractors on the premises?

1.33 Risk Assessments: (80)

Objective: To identify hazards and asses risk to workers and others who may be affected by the undertaking of the company.

a. Is trained staff nominated to undertake risk assessments in all departments?

b. Are appropriate risk assessments been undertaken in all areas, and have significant results been recorded or published?

c. Are risk assessments used to develop the H & S plan?

d. Have written H&S arrangements/rules been established to control risks?

e. Are employees aware of rules and arrangements and are they effectively implemented.

1.34 Monitoring and review of Assessments and safe working practices: (40)

Objective: To have in place adequate controls and systems to ensure that all risk assessments are monitored and, where necessary, reviewed.

a. Is there a programme to regularly review risk assessments?

b. Do supervisors undertake planned job observations to ensure arrangements are effectively applied and remain relevant?

c. Are the results recorded?

d. Are actions taken to correct deviations and update procedures?

1.35 Work Permits: (60)

Objective: To identify areas of work which require a higher duty of care due to the potential nature of the risk.

a. Have risk assessments identified areas where permits- to- work are required?

b. Have appropriate procedures and documentation been developed?

c. Are designated persons nominated to issue permits?

d. Are procedures effectively implemented and records maintained?

e. Are permits displayed at the work site?

f. Are permits only valid for specified period ( does not exceed one shift period)?

1.36 Off the Job Safety: (20)

Objective: To reduce losses through the reduction of accidents and incidents occurring outside of normal working hours.

a. Absentee records used to determine time lost and cost of accidents occurring outside of the workplace.

b. Promotion programme developed and implemented by the safety committee. (Off the job safety)

2.0 Management Control System

2.1.1 Work Equipment: (100)

Objective: To ensure that systems and procedures are in place for

the provision and safe use of work equipment.

a. Is there a programme of routine and planned preventative maintenance for work equipments?

b. Are maintenance records kept?

c. Is the maintenance/repair of equipment restricted to trained personnel?

d. Have operators received training/instruction?

e. Is use/operation of equipment restricted to authorised persons?

f. Has the equipment been subjected to risk assessment?

g. Have controls been established to protect against contact with dangerous parts and/or other specified hazards?

h. Is work equipment suitable for environment stable, securely fixed where necessary?

2.1.2 Procedures for isolation of equipment: (40)

Objective: To ensure that equipment is made safe under particular circumstances, such as when maintenance is being carried out or when an unsafe condition develops.

a. Are means available to isolate equipment from all sources of energy?

b. Is there a lock out procedure, written and applied?

c. Is use made of padlocks and multi locks?

d. Are individual keys under the control of the person undertaking the work?

2.1.3 Equipment Controls: (30)

Objective: To ensure equipment has adequate controls to ensure

its operation.

a. Is all work equipment fitted with suitable control for starting, stopping and adjustment?

b. Are emergency stops controls provided and are they easily accessible?

c. Are interlocks and sensing controls provided where necessary?

d. Are all controls clearly and uniformly marked?

e. Is the operation of controls, interlocks, sensing devices and guards regularly checked and recorded?

2.1.4 Hand Tools: (20)

Objective: To ensure that hand tools are of a high quality and that

there is an inspection system in place for defects.

a. Are all hand tools regularly inspected for condition, whether company owned or private?

b. Are the provision and use of work equipment regulations applied?

c. Are explosive power tools limited to use by trained authorized persons.

2.1.5 Access Equipment: (50)

Objective: To ensure that adequate inspection and examination

system are in place, so that defects, damage or wear can be deteedcted and remedied before they result in an unacceptable risk.

a. Is all scaffolding erected and dismantled by trained persons?

b. Is inspected regularly (weekly and after alternation or being subject to adverse conditions)?

c. Is register maintained?

d. Are all ladders, steps and access platforms identified and registered?

e. Are rules established and applied for the safe use of ladders?

f. Are all items regularly inspected by a competent person?

g. Is the inspection of fixed ladders and access equipment included in the maintenance programme?

h. Are scaffolds provided at all access steps off points?

2.1.6 Lifting Equipment and Records: (90)

Objective: To ensure that system and procedures are in place for

the provision and safe use of lifting equipment and its operation.

a. Are all lifting equipments registered and test certificate issued?

b. Is all lifting equipment inspected before use and subject to regular thorough examination?

c. Is the SWL marked on all equipment and on a notice where the equipment is stored?

d. Is the equipment stored correctly when not in use?

e. Is there an appointed person to take charge of lifts?

f. Are users trained, including use of standard hand signals?

g. Is there safe access and egress to and from overhead cranes and other equipment?

2.1.7 Pressure Systems and Portable Gas Containers: (50)

Objective: To ensure that there are written schemes in place for

the examination, inspection, maintenance and safe operation of all pressurised plant and equipment.

a. Is there a programme of inspection and maintenance for all pressure systems and written schemes of examinations where necessary?

b. Are all valves uniformly marked?

c. Is there a written procedure for system isolation and applied rigorously?

d. Are receivers thoroughly examined and are records maintained?

e. Are employees given instruction/information on dangers of pressurized gases?

f. Are all portable gas containers correctly stores and secured?

g. Is area clearly identified and kept free of combustible material?

h. Is portable equipment correctly inspected and maintained and are

records kept?

i. Are there suitable arrangements for transportation of cylinders?

j. Is the use of systems restricted to trained authorized persons?

2.1.8 Mobile Motorised Equipment: (90)

Objective: To ensure that there are written schemes in place for

the examination, inspection, maintenance and safe operation of all motorised plant and equipment.

a. Is there a maintenance programme for all vehicles?

b. Are drivers trained (including refresher) and authorized in writing?

c. Are licenses checked at least annually?

d. Are FLTs subjected to thorough examination and a daily log maintained?

e. Are charging areas for electric FLTs clearly designated, signs displayed and free of combustible material?

2.1.9 Hazardous Substances Control: (100)

Objective: To control the exposure of persons and the

environment to substances hazardous to health to prevent injury, illness or damage.

a. Is there a competent person appointed in writing to coordinate COSHH assessment and control.

b. Has a survey been undertaken and a list compiled of all substances used and/or produced, safety data sheets obtained?

c. Has a suitable and sufficient assessment of all substance/operations been undertaken and results recorded (assessment to cover use, storage, handling, disposal and spill)?

d. Are adequate controls implemented and maintained? (Estimation, substitution, control at source PPE)

e. Have all staff been trained/instructed in use of controls, and training recorded?

f. Is there comprehensive information available to employees/occupational health/first aider?

g. Are there controls on procurement to ensure quantity is not exceeded and no substance is introduced without assessment?

2.2.0 Radiation Sources: (80)

Objective: To have in place systems to contain and control all

sources of radiation.

a. Are radiation advisor and supervisor appointed in writing?

b. Is a register of sources maintained?

c. Are their written storage, handling and disposal procedures?

d. Have employees been trained in procedures?

e. Is Health Surveillance undertaken and records kept?

f. Are warning and information signs displayed?

2.2.1Non-Ionizing Radiation: (30)

Objective: To control the exposure of persons to radiation to

prevent injury, or illness.

a. Have risks been identified and assessed? (UV, IR, M/W, R/F & LASERS)

b. Are control measures in place?

Electrical Safety:

2.2.2Portable and Transportable electrical equipment: (40)

Objective: To have systems and controls in place to prevent the

risk of electrocution.

a. Has all portable and transportable equipment been identified and entered on a register?

b. Is there a suitable programme for test/inspection of equipment, including where appropriate, user checks?

c. Are protective measures implemented for portable hand tools ( use of battery operated tools, L.V equipment, RCDs).

2.2.3 General Electrical Installations: (90)

Objective: To have in place management controls and systems to

ensure that all electrical systems are constructed and maintained to avoid the risk of injury and damage.

a. Are fixed electrical installations regularly inspected/tested and reports held?

b. Is there a plan of the electrical system, is it kept up to date?

c. Are there written procedures for the secure isolation of electrical systems?

d. Are switches and distribution boards clearly marked?

e. Is access to HV switchgear restricted to authorized persons?

f. Are written safe working procedures including permit to work system available for HV operations and live working?

2.2.4RCDs- Electrical Protection Devices: (40)

Objective: To ensure that adequate equipment and protection is in

place and maintained.

a. Have areas where RCD protection is required been identified and devices installed?

b. Are RCDs are checked six monthly by users?

c. Are RCDs regularly inspected/tested by a competent person?

d. Are all tools and test equipment used on electrical systems insulated?

e. Is protective equipment (gloves, probes, mats etc.) regularly inspected/tested and records maintained?

Personal Protection Equipment:

2.2.5 Assessment and Controls (Issue, use and maintenance of PPE): (50)

Objective: To ensure that adequate controls are in place for the

correct provision and use of personal protective equipment.

a. Has an assessment been carried out as the requirements for and suitability of PPE?

b. Is record maintained of equipment issued and commitment obtained from user?

c. Have employees received instruction and training on use, care and storage of PPE?

d. Is equipment checked regularly by supervisors?

2.2.6 Head Protection: (15)

Objective: To assist in the prevention of injury to the head.

a. Are areas where use is required identified and defined?

b. Is equipment provided and worn?

2.2.7 Eye and Face Protection: (15)

Objective: To assist in the prevention of injury to the eyes and

face.

a. Are areas where use is required identified and defined?

b. Is equipment provided and worn?

2.2.8 Footwear: (15)

Objective: To assist in the prevention of injury to the feet.

a. Area areas where use is required identified and defined?

b. Is equipment provided and worn?

2.2.9 Protective Clothing: (15)

Objective: To assist in the prevention of injury to the body.

a. Are requirements for protection clothing identified, item issued and worn?

b. Are there arrangements for the cleaning, storage and drying of clothing?

2.3.0 Hand and Arm Protection: (15)

Objective: To assist in the prevention of injury to arm and hands.

a. Have areas where protection is required been identified?

b. Is suitable protection available and used ( gloves, barrier creams)?

c. Have employees been given information on the risks and instructed on requirement for personal hygiene?

2.3.1 Respiratory Equipment: (50)

Objective: To assist in the prevention of ill health.

a. Are areas where respiratory equipment is required identified?

b. Has the correct type of equipment been determined and issued?

c. Have employees been trained in use of equipment?

d. Is the equipment used as required and regularly inspected?

e. Is there permit procedure for use of SCBA and use restricted to authorised persons?

2.3.2 Hearing Conservation: (40)

Objective: To assist in the protection of hearing.

a. Have noise problems been identified as part of risk assessment?

b. Have noise surveys been undertaken and information recorded?

c. Have measures been taken to eliminate or reduce noise levels (reduction at source, enclosure, removal or restriction of employee access)?

d. Have noisy areas been clearly defined?

e. Is hearing protection provided and worn?

f. Are employees exposed to noise given regular hearing acuity tests?

2.3.3 Safety Harness: (40)

Objective: To provide a means of restraint of falling.

a. Have areas where harnesses are required been identified (including emergency escape) and is correct equipment issued)?

b. Is equipment checked and inspected regularly?

c. Are anchorage points suitable and regularly tested?

2.3.4 Manual Handling: (100)

Objective: To have in place management controls and systems to

ensure that manual handling issues are identified and eleminated or reduced to an acceptable level.

a. Have manual handling risks been identified?

b. Have steps been taken to eliminate the problem of automation, mechanisation and by mechanical aids?

c. Have manual handling assessments been carried out and results recorded?

d. Have steps been taken to reduce the risk?

e. Have employees been trained in use of correct techniques and is it recorded?

2.3.5 Ergonomics: (60)

Objective: To incorporate ergonomic principles in all aspects of

the working environment.

a. Has Ergonomic survey been undertaken at all work operations?

b. Has a checklist been used covering suitability of seating, work base height, space standards, position of controls and switches, position and movement of work places?

2.3.6 Display Screen Equipment: (50)

Objective: To have in place management controls and systems to

ensure that workstations are designed to fit the individual.

a. Have all users/operators been identified?

b. Have workstation assessments been completed and identified improvements implemented?

c. Are assessments reviewed annually?

d. Have arrangements for eye screening/testing been implemented and staff advised?

e. Have all users/operators been made aware of risks and given information/training on how to avoid them?

3.0 Fire Control System

3.11 Fire Safety Protection Co-coordinator: (40)

Objective: To identify the person nominated to advise, manage

and control the risk from fire within an organization.

a. Is there a properly appointed fire prevention and protection coordinators?

b. Has the appointed person been properly trained?

c. Are his duties and responsibilities set out formally?

d. Does he appraise management at regular intervals and as the need arises?

3.12 Fire Risk Assessments and Equipment: (80)

Objective: To formally identify, evaluate and control the risk from

fire within an organization.

a. Have fire risk assessments been completed for each department, recorded and where required actioned?

b. Does the fire equipment comply with the demands of the fire certificate?

c. Is all the equipment registered and regularly inspected (Monthly)?

d. Is annual maintenance progamme in place?

3.13 Fire Equipment and Call Point Identification: (30)

Objective: To ensure that adequate means exist for fighting

equipment, fixed and portable.

a. Are all alarm call points marked, identified and kept clear?

b. Are all the fire equipment points identified, clearly marked and maintained clear of obstructions?

3.14 Alarm System and Emergency Lighting: (50)

Objective: To ensure that adequate means exist for the amnual

and automatic detection of fire and the audible warning of those who may be endangered by that fire.

a. Is the alarm system tested on a weekly basis?

b. Is there a separate independent back up system?

c. Can the alarm be heard in all areas of the premises?

d. Are all employees aware of the evacuation signal?

e. Are all automatic detection systems regularly tested?

f. Is there a permanent telephone line to the outside at all hours?

g. Is emergency lighting provided and maintained?

3.15 Fire certificate and Log Book: (40)

Objective: To ensure that adequate procedures and systems exist

for the safe evacuation of people while on the premises.

a. Is there a fire certificate for the premises? Has one been applied for?

b. Are any changes notified to the authorities?

c. Is the logbook maintained?

3.16 Drill and Instruction: (90)

Objective: To train all people in emergency and evacuation

procedures and to carry out exercises to test their effectiveness.

a. Are there written procedures for fire evacuation?

b. Are fire evacuation procedure notices displayed?

c. Do drills take place biannually?

d. Are all staff instructed annually with wardens and marshals trained?

3.17 Fire Escape Routs & Exits: (100)

Objective: To ensure that adequate means of escape is provided

and maintained for the safe evacuation of people in the event of fire.

a. Are all escape routes clean, adequate and safe?

b. Are all fire doors properly rated for their application?
