CCR Vision:  All children live with a committed, permanent and nurturing  family.  

Services and supports are tailored to meet the needs of the individual child and family being served with the ultimate goal of maintaining the family or when this isn’t possible, transitioning the child or youth to a permanent family and/or preparing the youth for a successful transition into adulthood.

When needed, group home care is a short‐term, specialized and intensive intervention that is just one part of a continuum of care available for children, youth and young adults.

Principles Wraparound

• Families have a high level of decision-making power at every level of the Wraparound process.

• Team members are persevering in the commitment to the child and family. • Wraparound efforts are based in the community and encourage the family’s use

of their natural supports and resources. • The Wraparound approach is a team-driven process involving the child, family,

natural supports, agencies, and community services working together to develop, implement, and evaluate the individualized services plan.

• Services and supports are individualized, built on strengths, and meet the needs of the child and family across the life domains to promote success, safety, and permanency in home, school, and the community

• The process is culturally component, building on the unique values, preferences, and strengths of the child, families, and their communities.

• The plan is develop and implemented based on an interagency, community/neighborhood collaborative process.

• Wraparound plans include a balance of formal services and informal community and family resources, with eventually greater reliance on informal services.

• Wraparound teams have adequate and flexible funding. • Outcomes are determined and measured for the system, for the program, and for

the individual child and family.

Core Practice Model • Children are first and foremost protected from abuse and neglect, and

maintained safely in their own homes • Services are needs driven, strength-based, & family focused from the first

conversation with or about the family • Services are individualized and tailored to the strengths and needs of each child

& family • Services are delivered through a multi-agency collaborative approach that is

grounded in a strong community base. • Parent/Family voice, choice, & preference are assured throughout the process • Services incorporate a blend of formal & informal resources designed to assist

families with successful transitions that ensure long-term success. • Services are culturally competent and respectful of the culture of children & their

families • Services and supports are provided in the child & family’s community • Children have permanency & stability in their living situation.

Continuum of Care Reform (CCR) In order to support the belief that all children live with a committed, permanent, and nurturing family this work will ensure that services and supports are tailored to meet the needs of the individual child and family by:

• Parent/Family voice, choice, & preference are assured throughout the process. • Evidenced based assessment process should occur, and include a teaming

model and youth voice and choice • Families have a high level of decision-making power at every level of the
