Brent Adult Social Care Occupational Therapists

Brent Adult Social Care is committed to developing an integrated Occupational Therapy service provision within Brent’s Adult Social Care Customer Journey as Occupational Therapists are recognised as being key in delivering a high quality multidisciplinary service that effectively meets the needs of our customers.

Occupational Therapy posts have been created in Adult Social Care Teams, with the aim of ensuring customer focussed and timely Occupational Therapy services that deliver and promote independence and improved outcomes for customers, through assessment and intervention in moving and handling, major adaptations, complex equipment provision and housing assessment needs.

Occupational Therapists are encourage to develop new skills using the wide range of training available in Brent Adult Social Care and professional development through professional supervision and annual appraisal. Brent Adult Social Care Occupational Therapists have created a lively and supportive peer group to enabling all Occupational Therapists to take an active role in their own continuing professional development.

As part of our positive working relationship with LNWUHCT we have developed a rotational post which allows Occupational Therapist to experience both Health & Social Care.

Adult Social Care OT Role

To share professional expertise and train qualified and unqualified staff to be able to address needs for minor adaptations, simple aids to daily living equipment and fast track showers.

Undertake OT assessments for new & existing customers to social care where the presenting needs are in relation to major adaptations, complex equipment or moving and handling risk assessment.

Undertake moving and handling re-assessments/reviews where care packages and or carers are no longer managing or where safe guarding concerns have been raised to ensure compliance with health and safety legislation and regulations

Assessments for Housing department in respect of requests for housing needs assessments where the DMO or Housing Officer has requested a functional assessment in respect of housing needs.

Void viewings for Housing Department where properties have been offered to customers where there are known disability needs in respect of the type and suitability of a property and then to assess for and raise major adaptations or DFG referrals and OT specifications as appropriate.

Provide specialist professional project consultation on new build designs for Housing Associations, advising on design and accessibility, amendments and specifications for adaptations , allocation of tenants and individual / bespoke adaptations.

Professional consultancy on internal housing schemes – eg extra care sheltered housing scheme design

To undertake complex equipment and adaptation reviews where there is a change in needs and arrange for equipment provision and referrals and specifications for major adaptations/DFGs as appropriate

Job Purpose:

To provide a specialist Occupational Therapy assessment and Adult Social Care service provision for Brent adult residents with substantial physical disabilities in accordance with relevant legislation and department eligibility criteria. In conjunction with the service user formulate a plan to enhance their independence through provision of equipment or through home adaptations. The needs of the carer are also to be carefully considered, assessed and where eligible addressed.

Principal Accountabilities and Responsibilities:

1. To directly manage in accordance with department procedures a caseload of service users as allocated by the Deputy Team Manager and or Team Manager.

2. To visit people with substantial disabilities in their own homes and carry out needs led assessments and reviews of their functional needs and environmental difficulties, taking into account the needs of carers and other family members.

3. To arrange for the provision of equipment and minor adaptations that will meet the service users needs and give instructions on use of the equipment to both user and carers.

4. To prepare and revise care plans in accordance with departmental procedures.

5. To make referrals as appropriate for carers assessments and to other specialist agencies both in-house and externally.

6. To keep accurate records and case notes of all work carried out, using the department’s

policy, procedure and standards on case work recording.

7. To ensure confidentiality of service users’ information.

8. To ensure that all Health & Safety requirements are met throughout the implementation of any work plan.

9. To undertake moving and handling assessments, arrange delivery and demonstration of equipment along with written reports and recommendations to ensure the safety of service users and carers.

10. To assess for and make written recommendations to the housing department to support service users’ applications for Disabled Facilities Grants, housing adaptations and re-housing needs.

11. To make written recommendations to Housing Associations for adaptations.

12. To work with surveyors from housing departments, housing associations and external Architects to design major adaptations and monitor the adaptation installation to ensure that the works meet service users needs.

13. To produce reports on the functional needs of service users where appropriate e.g. for re-housing, employment.

14. To respond to and where possible to resolve complaints within the Borough’s complaint procedure.

15. To work with other statutory agencies and voluntary and private sector to facilitate an effective service delivery.

16. To access appropriate assistance where language difficulties present problems in care management.

17. To advise Team Manager about any trends or emergency issues that may impact upon the delivery of service.

18. To directly manage in accordance with department procedures a caseload of service users a proportion of which as determined with the Deputy Team Manager and or Team Manager, to be complex cases.

19. To assist with in-house and external training for staff involved in the provision of care for disabled people.

20. To participate in the training and supervision of Care Assessors and OT students.

21. To undertake the role of Clinical Supervisor and management Care Assessors, ensuring regular supervision, appraisal and learning and development.

22. Carry out duties with due regard to the Council’s Customer Care, Equal Opportunities, Information Governance, Data Protection and Health and Safety policies and procedures.

23. Undertake any other duties commensurate with the general level of responsibility of this post.

Job Knowledge, Skills & Experience:Specify the qualifications, experience, skills and abilities required.

Knowledge and Qualifications:

Degree /Diploma from the College of Occupational Therapist or equivalent and OT registered with the Health Care Professionals Council

Evidence of recent and relevant Continuing Professional Development Evidence of knowledge and understanding of relevant Social Care legislative and policy

framework including tests to determine eligibility for services Knowledge of the needs of elderly people, people with physical disabilities

Experience:

Experience of working with older people and adults with physical disabilities; older people with dementia and mental health needs; adults with learning disabilities

Experience of undertaking comprehensive OT assessments and in particular those in relation to moving and handling and or major adaptations

Experience of working in a multi-disciplinary team Experience of leading or participating in the delivery of training for OT Students and other staff

Hours of Work36 hours per week

Whilst your hours will normally be worked during Monday to Friday, your actual working week runs from Monday to Sunday and you may be required to work on any day in this period.

Pay IntervalsMonthly on the 15th of each month into your bank account.

Salary progressionAll appointments are made at the bottom of the salary scale for the post unless by exception. Within the salary range there a number of salary points. Subject to satisfactory service your salary will rise incrementally each year through these points to the maximum.

Annual leaveAnnual leave entitlement starts at 26 days plus leave on all recognised public and bank holidays rising to a maximum of 32 days depending on length of service. Leave is pro rata for staffs who work part time or job share.

Probation periodThe appointment is subject to completion of a satisfactory probation period.
