Registration Information

My husband and I have four active kids and at one point or another our entire family has been

involved in Carol Stream Park District activities and volunteering has just always been second nature. I grew up with parents that volunteered and I look at volunteering as a part of being a parent. I volunteer at every school and sport our kids are involved in and at our church. People may think I’m a giver, but the truth is I’m the one receiving the gift by volunteering. I have made life long friends through volunteering and had a lot of fun along the way.

- Annie Taschetta

Annie, a native Minnesotan, moved to the Carol Steam area over 17 years ago with her husband. Annie and her husband are business owners in Wheaton and live in Winfield with their 4 kids, ranging from 8-14 years of age. Annie has an elementary education degree and has taught kindergarten and third grade. She is currently a substitute teacher and a special education coordinator.

Annie loves watching her sons play football and baseball and her daughters compete with the Carol Stream Springers. She volunteers with the Springers Gymnastic Team by creating the team newsletter and bulletin board. She also enjoys reading, traveling and shopping. For more information about volunteering with the park district, visit csparks.org/volunteer or call (630) 784-6100

E-cycle your electronicsGot a new gadget on your holiday wishlist? Don’t junk your old electronics, take them to an electronic recycling center. Visit scarceecoed.org to find local electronic recycling centers.

Transform newspapers to logsRecycle old newspapers by making rolled paper logs for your fireplace. Roll newspaper sheets into a log shape, then soak them in water. Dry the log overnight and use like ordinary wood.

Check your ductsMaximize the efficiency of your heating system by checking your ductwork for leaks. Sealing leaks with duct mastic will stop hot air escaping before it reaches your vents.

This program guide is printed on recycled paper using eco-friendly soy ink. You can help our guides become even greener - when the season’s over, don’t trash them, recycle them!

Earth Flag Awarded to Park DistrictThe Carol Stream Park District was awarded the Earth Flag in September in recognition of its earth-friendly practices.

To achieve the Earth Flag certification the park district had to pass an environmental audit by the SCARCE organization and implement several new green initiatives. These included adding over 100 new recycling bins to the parks, creating new policies to reduce paper, energy and fuel use, and increasing the number of nature and green-based activities for residents.

WAIVERREGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION

The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

WARNING OF RISK

Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety.

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK

Please read this form carefully and be aware that in signing up and participating in the programs listed, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with these programs (including transportation services and vehicle operations, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in these programs against the Carol Stream Park District, including its officials, agents, volunteers and employees. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature. Participation will be denied if the signature of adult participant or parent/guardian and date are not on this waiver.
