Centennial Sump Line Permanent Repair Work
Scope of Work
Westfield Public Works is requesting a quote to perform permanent sump line repairs within the
Centennial neighborhood. The sump lines in this neighborhood have been impacted by roots growing
within the 6” SSD under the curbs. In order to repair the issues these are the procedures we are
following.
1. Locate each sump line stub at the corresponding address.
2. Expose sump line stub.
3. Directionally drill or pipe burst new 2” – 4” Poly pipe to nearby structure through the existing 6”
SSD and appropriately core into the nearby curb drain structure.
a. Pipe size will vary depending of how many sump lines will be tied into the new pipe.
4. Each sump line at the addresses will need to be tied in by fusing them into the new main sump
line.
5. All holes will be required to have a flowable fill backfill.
6. Any removed curbs that are necessary to be removed for repairs will be required to be poured
back in place.
a. Removal of curbs will vary, but initially will likely not be necessary. Decisions to remove
curbs will need to be discussed with Westfield Inspector prior to approval.
7. Any removed asphalt or roadway removed in order to perform work will need to be flow fill
back filled and concrete capped flush with pavement.

8. Full restoration with proper top soil (no dig spoils) and seed plus straw will be required.
Additional Notes:
1. An Encroachment Permit will need to be submitted and approved prior to mobilization, but all
fees will be waived. This is for our records of work performed within the neighborhood and
ROW.
2. Any open excavations will have to be properly Snow Fenced and Caution Tapped when leaving
that location over night or during the day.
3. All traffic signage and lane closures will be required.
4. You will be working in large subdivision with many residents. There will be times where you are
approached by the public. Our expectation is cordial attitudes and if a question cannot be
answered please defer to the Westfield Inspector.

charge, plus reasonable attorney fees necessary to collect past due invoice. If using credit card to pay for invoice there will be a
minimum convenience charge of 4%. This serves as notice of intent to file a lien for slow or nonpayment.
Quality workmanship and professionalism are standard. Material if provided by DDD will be as specified. Any alteration or
deviation from above specification involving extra cost will be executed only upon written orders, and will become and extra
charge over and above the estimate. This was given under normal ground conditions. The price will be adjusted if abnormal
conditions exist (i.e. water, sand, rock, under ground utilities or any other obstructions)
Acceptances of Proposal - The signature below indicates acceptance of stated terms, conditions and specifications by both
parties. This document may be executed in multiple counterparts and by facsimile signatures.
