Introduction The Improving Access to Psychological Therapies (IAPT) programme has one principal aim, to support Mental Health Trusts implementing National Institute for Health and Clinical Excellence (NICE) guidelines for people suffering from depression and anxiety disorders. At present, only a quarter of the 6 million people in the UK with these conditions are in treatment, with debilitating effects on society. (http://www.iapt.nhs.uk)

The way the IAPT programme has been embraced by the NHS across England is inspiring. Primary Care Trusts (PCTs) have established these new services faster than planned and, as a result, more people are getting access to the help they need. This is the NHS working at its best, with local systems responding to local needs and innovating to provide high quality services. Two types of psychological therapy practitioners are required:

High Intensity therapists trained in cognitive behavioural therapy for people with moderate and severe depression and anxiety disorders

Low intensity therapy workers trained in cognitive behavioural approaches for people with mild to moderate anxiety and depression. These approaches include guided self help and delivering psycho-educational groups. Services will also have administrative staff, employment advisors, a GP advisor and links with other services such as housing, drugs advice and benefits

The service also has counsellors and nurses who provide high intensity therapies.

It is important that people have a say in what kind of treatment they receive. This helps ensure the best health outcome for them. Clinicians should explain which treatment they are recommending and why they think it is suitable for the patient. The team and their patients should have clear information about local services and the treatment choices available. IAPT services routinely measure people’s health outcome. This charts their progress and has therapeutic benefit. It is part of ongoing, collaborative service evaluation too, providing feedback on elements of treatment that are helpful or unhelpful. The results evaluation results are available in Appendix 1, below in summary is a snapshot of the results.

The IAPT Service in Chesterfield & North East Derbyshire commenced in October 2009 when Lincolnshire Partnership NHS Foundation Trust won the tender to manage the service. The IAPT service has now moved into their permanent accommodation at Spire Walk, Chesterfield. This accommodation has five consulting rooms and a large group room. Not all staff see their clients at Spire Walk due to the huge geographical area that Chesterfield covers, however all staff have to come into Spire Walk to input their data onto the information system.

Summary of Results It was agreed to hand out the IAPT questionnaires at the patients’ penultimate appointment or near the end of their treatment and patients were assured that their anonymity would be guaranteed. There were 148 completed questionnaires received from the beginning of July 2012 to the end of September 2012, this is a decrease of 3 returns compared to the 151 received in Quarter 1. A summary of results is shown in a performance dashboard on page 3. The comments from patients are listed in categories in Appendix 2.

Spire Walk

My therapist was excellent I was at ease from the first appointment and she got the best out of me. I don’t think I would have come so far in so short a time without the help I have received

Caslte St

Did online workbooks which were very good, however, it may have helped to be able to contact someone in between appointments with any questions, comments, general support etc

Shirebrook

a letter/information pack would be helpful before the first appointment to help you understand CBT and what the service is and how your treatment is set out, although I understand this is difficult as each person is different

Shirebrook

It has been amazing and has helped me make the right choices I need to be happy thank you Shires Surgery

The techniques I have learned have become an important part of day to day life, to help overcome any issues Spire Walk

My only dissatisfaction was the length of time it took to get the service which meant the major part of y crisis was over before therapy started Spire Walk

** WAS MY PWP HE WAS AMAZING AT HIS JOB. He helped me out of a horrible lace and gave me the skills to help myself. Thank you ** Spire Walk

I was surprised at how simple some of the ideas given to me were. I was always told how good I was and how well I was doing. This is an excellent form of therapy and I would recommend it to anyone with problems

Spire Walk

My counsellor was friendly, helpful and approachable. She has given me some techniques which should help me if I get that low again Spire Walk

This service is remarkable, I only wish I came here sooner. I shall encourage anyone else to use this service. No improvements are necessary. The waiting list could be a bit sooner. The telephone interview leaves people wondering what to do - it is a bit vague

Spire Walk

Very satisfied indeed Spire Walk

I feel if someone needs counselling they should have some sort of support whilst waiting for their 1st appointment rather than being left without any

Spire Walk

I was quite surprised the sessions were so short (30 minutes) but they did cover useful techniques/issues Spire Walk

I enjoyed actually attending counselling the fact I hadn’t had any time for me and this made me find me again and I am happier now Spire Walk

the support I received from ** was excellent Spire Walk

By the time I was seen by a Therapist I was starting to recover with the help of family. I had 2 appointments and then y therapist was on holiday for 3 weeks. I did not have time to build a relationship and came away from each session feeling they had been a waste of tome

Spire Walk

Had approximately 6-7 sessions, which was just right in order for me to feel better, thank you Spire Walk

This service is remarkable, I only wish I came here sooner. I shall encourage anyone else to use this service. No improvements are necessary. The waiting list could be a bit sooner - the telephone interview leaves people wondering what to do - it is a bit vague

Spire Walk

I feel better knowing that I can use steps2change whenever I need it - support is everything Spire Walk

Really enjoyed it focused on real techniques Spire Walk

The course was fine and satisfactory Spire Walk

After my doctor referred me to Steps to Change. I did have to wait a long time for an appointment. There was a mix up with the referral. This was quite stressful

Spire Walk

I was made to feel very welcome and at ease straight away Spire Walk

Slightly disappointed with the 6 months or so waiting list but in this climate, recession I suppose most people would feel the same Spire Walk

I am still stuck with the problem I wanted to resolve but trying to accept I will just have to hope it lessens more and more with time. I didn’t feel the PWP understands that we all have different ways of relaxing/me time as she wanted me to do what she considered relaxing/me time which wasn’t relaxing/me time to me

Spire Walk

Extremely happy with the service provided and the therapist. Thank you so much Spire Walk

it was a very helpful experience to be able to discuss problems with counsellor in an empathic way Spire Walk

I was referred by my GP in Oct-Nov 11 - due to several reasons re-referral was necessary which took time - I was offered a lunchtime session which I could notnt attend then I had to wait again for 1:1 sessions. By this time I had managed to keep body & soul together. I was able to see a counsellor for 6 weeks. I found filling in the forms prior to each session very unhelpful as my scores were not very high I felt as though I should not be there and yet I obviously needed the chance to have counselling. It is a shame I was not able to attend when I most needed the sessions

Spire Walk

Great service, it is a shame the waiting list is so long - it would have been better if the GP still had a few appointments in between referral and CBT starting as I felt on my own and still had no help

Springs HC Clowne

Really good Stonebroom

I feel if it was not for this service my children would probably not have a dad this lady ** I really and truly believe may have saved my life. She is a truly remarkable woman and I cannot thank her enough

The Grange

To be honest I was initially surprised at how short the sessions were, but after a few weeks, I felt this was a really good idea: it helped keep me busy doing something, rather than dwelling for too long. Overall, its helped me a lot, and I am very grateful this service exists. Thank you

Welbeck Surgery

I felt that there wasn’t enough time to talk through my problems during my appointment and the person who I was seeing it was like she didn’t know what to say to me just gave me info to look at. Some of it was useful but I am dyslexics it took me a while to read and get an understanding of the info

After my 1st assessment I was put into anger management group and after just 1 session was unable to cope and it was clear it wasn’t the right 1st step. After a 2nd assessment it was found CBT would be a better option and that has been the case as it was too much to enter into a group setting if never done that before

Wheatbridge

At first I wasn’t sure and then after the first session. I knew I was at last going to get the help I needed Wheatbridge Surgery

Very helpful and enlightening Whittington Moor Health Surgery

The counsellor that I have been seeing for 12 weeks was fantastic & understanding & very easy to talk to. 12 weeks is not enough could do with more time with this counsellor

Whittington Surgery

This experience has helped me more than I thought I feel a lot more positive Wingerworth
