Opens a window to find patients whose data is recorded before

Shows current patient’s data in a PDF file format that can be print out

Opens a window in order to export patients’ data to SPSS

Explanation of some edit boxes fields

Edit Boxes are fields in which two types of data can be typed.1) Text : Any text can be typed in a field because texts do not affect statistics.2) Number : Just one number can be typed in a field because a number affects

statistics.* If a number value may not represent a field because it has minimum and maximum values, mean of the value should be type in the field. Typing numbers like “1-4” or “1,2,4” or “1.2.4” are incorrect.* Different countries use different decimal number separators. One of the decimal number separators - comma “,” or dot “.” - should be used.

Personal Identification Number:A unique number assigned for one patient. This number may include letters as well.

What is Personel IDIn statistical software there is no need to record Personal ID, name and last name of a patient. But in database software it is compulsory to record these three fields. While database software records patients data, it is also used for tracking (querying) patients data. In order to query recorded patients data, name and last name may not help to find correct patient. As an example: Name      Last Name--------   ------------------John       BrownHans       WhiteJohn       BrownCarol       GreenJohn       BrownMike        NavyJohn       Brown As you see there are four "John Brown" in this database. There is no way to distinguish any of them from others.  In this kind of case it is better to use a Personel ID. Personel ID can be used in three different way: (A) Personal ID may be produced by database. (B) Personal ID may be produced by your clinic. (Some clinics still use paper based files. There may be numbers on them.) In items (A) and (B) patients can be informed about their Personel ID. When the patient comes to clinic next time, clinic may ask the Personel ID. So none of "John Brown" confused since Personel ID is used. But in reality most of patients forget to bring their Personel ID or cannot remember their Personel ID. And the confusion goes on. (C) In Turkey, people have Personel ID which is given by government. This ID is used in anywhere like bank, government, large amount of shopping, health, vs. So most of Turkish people memorized their Personal ID or carry their Personel ID in their pocket. This prevents confusing "John Brown". Although it is suggested you to use item (C) which is your country's Personel ID to distinguish "John Brown"  and prevent confusing, it may not be lawful in some country for national security and your old paper based files may not hold these Personel ID. In this situation it is better to use item (B).

Duration (month):How many months you have recurrent headache disorders?

Previous Frequency (day/month):Mean frequency of attacks per month (only 1 number choose, preferably maximum values)

Last Frequency (day/month):Mean frequency of attacks on last month (only 1 number)

Duration Without Medication (hour):Mean duration of attacks without medication (only 1 number)

Smoking:Smoking habituation of subject should be given as packed per year

Alcohol:Habituation of drunk should be given as year

Hypertension:Duration of diagnosis as year

Diabetes Mellitus:Duration of diagnosis as year

Cardiac Disorder:Duration of diagnosis as year

Severity:Mean severity of headache attacks according to visual analog scale

Frequency:Mean frequency of attacks per month

Duration:Mean duration of attacks as hour

Date:Date of every examination

Department:The department where the patient will be consulted

After clicking the Save icon on the form, a plus icon becomes visible in the Detail Panel. This means that Detailed Panel is ready to be filled.

Advanced Search

There are two kinds of Jokers to do advanced search.1) _ (underscore) joker is used for only one character2) % (percentage) joker is used for at least one character

Example 1)Suppose a patient’s Name and Sirname is as followsName : AdianSirname : BrownBut by mistake sometimes names can be written wrong like Adien.If Adian is written like Adien, and if it is searched as Adian,this patient can’t be found.For this kind of confusion only one character joker can be used like this : Type Adi_n in name and Brown in surname field.

Example 2)Suppose a patient’s Name and Sirname is as followsName : AnneliseSirname : BrownBut by mistake sometimes names can be written wrong like Anneliese.If Annelise is written like Anneliese, and if it is searched as Annelise,this patient can’t be found.For this kind of confusion at least one character joker can be used like this : Type Annel%se in name and Brown in surname field.

Example 3)Suppose a patient’s Name and Sirname is as followsName : AndrewSirname : BrownFor some reason patients whose sirname is Brown are required. Type % in name and Brown in surname field.

Example 4)To see all patients type % in name and % in surname field.
