(IH)/DGEHS)/180/DHS/2017/193032 dated 08-02-2017 (copy enclosed) that the pensionerresiding in areas not covered by the Delhi Health Schemes or who do not opt to become a

member of the Health Scheme would be eligible for the payment of fixed medical allowance

Sub: Regarding payment of fixed medical allowance to the pensioners.

With reference to your letter No. F 14(05)/2013/T-l/Pr. A.O./Pt. file/3707-3709 dated 16.11.2016,it is intimated that as per provisions given in DGEHS scheme, conveyed vide this office letter dated25/10/2007 (copy enclosed), Fixed Medical Allowance (FMA) is admissible to the pensionersresideing in areas not convered by the scheme or who do not opt to become a member of the schemew.e.f 01.093.2003. The point wise clarification sought is as under:-

. I. • The Fixed Medical Allowance is payable to the pensioners from the date of retirement or from01.09.2013, whichever is later, to the date of option of becoming the member of DGEHS, whereoptionof become the member of the scheme is not exercised at the time of their retirement

2. The matter regarding payment of Fixed Medical Allowance with effect from 01.12.1997 insteadof 01.09.2003, was examined earlier, but was not found feasible.

This is issued with prior approval of Directorate General of Health Services.

Subject: Delhi Govt Employees Health 8cheme: Provisions for retired employees.

A health scheme was formulated for serving employees/pensioners of Govt. of National Capital Territoryof Delhi in 1997. The scheme was communicated vide letters No. F.Np.27(9)/96-M&PH/ dated 13* March1997 S P.No.27ra/97-H&FW/ dated 26* September 1997. The scheme has subsequently been revised videDM No. F-342/81/2003/H&FW/3264 dated 6* October 2003 and OM NO.F342/126/2004/ H&FW/7553-63dated 21" February 2006.

Consequent to reyfewand ratfonallzatlon-ofdlfferentproyistonsand-procedures of the Delhi GovernmentEmployees Health Scheme, the.following provisions/ modifications In-the, Delhi Government EmployeesHealth Scheme are notified for better Implementation of the schema:

1.BENEFITS UNDER THE SCHEME:I. The beneficiary shall be provided the facility of out-patient care, emergency services, free

Supply of necessary drugs/appliances, laboratory and radiological investigations, specialistconsultation, family Welfare services and treatment In specialized hospitals as per provisions ofCS(MA) rules. :

It. The following benefits would be extended to the,beneficiaries:

a Free admission In all the hospitals/Nursing Homes under Govt, of NCT of Delhi accordingto entitlement and as per norms applicable to 3 serving Delhi Govt. servant of that Incomebracket. In case, any medicine is not available in, hospital/dispensary, the hospitalauthorities would purchase the same and supply it to the eligible beneficiary and in casethisls not possible, the beneficiary would be allowed reimbursement of such medicines.

b If the medical officer Incharge of the hospital/Dispensary feels that specialist treatment Isrequired, he/she will refer the patient to the authorised medical institution providing such

specialized treatment on priority basis,c In case of emergency treatment, the pensioner and his/her spouse/dependent family

member would be. entitled fp.the same benefits as are admissible to serving employeesunder CS(MA) Rules, 1944.,

d Similarly, In case of specialized medical care services like Coronary, bypass surgery,anglography, angloplasty, cancer treatment etc the benefits as admissible to the workingemployee of.Delhl Government shall also be made applicable to all its pensioners andtheir dependent family Ihembars. The treatment Id pensioner will be. provided onpreferential basis on production of the DGEHS Card.

e The treatment under Indian System of Medicine end Homeopathy will also be available asperGS(MA)Rules ' " -••-••••

111. The Office Memorandum Issued by Govt. of India, Ministry of Health & Family WelfareF.No:Re&24/2001/JD(M)/CGHS/Dei.hl/CGHS(P) daled 07.09.2001 shall form the basis for"Medical Facilities for serving Govt. Employees and Pensioners of Govt. of Delhi" with suchmodifications as are decided by the Government of Delhi from lime to time.

lv: Benefits of the scheme tare prospective- in nature i.e only after becoming, a member of thescheme. Reimbursement shall be made only in case the pensioner is a member of thescheme.

2.MEMBERSHIP OF THE SCHEME:\l All serving employees and pensioners (Including family pensioners) of the Govt. of NCT of

Delhi will be beneficiaries of the scheme.

It. Employees will give an option at the time of retirement for becoming a member of thescheme. The subscription will be deducted In lumpsum (equal to 10 year subscription)from those who.opt for lifetime membership, at the time of retirement Those who have

4. ISSUANCE OF MEMBERSHIP CARD/ATTCHEMENT TO DISPENSARIES:t. Oh the basis of Conlributioh, the; pensioners will be issued a OGEHS Card which would entitle

them ip free treatment In a, Dispensary/Hospital of the Goyt. of Delhi and trealment/diagnosticfacilities in recognized empanelled private hospitals/diagnostic centers as per provisions Dflhe

iacheme.

II. .All beneficiaries Shall be attached/ registered with Delhi Government Dispensaries (Allopathic)/ Hospitals (Allopathic). The beneficiaries residing In NCR area may opt for attachment of theircards lo any of Delhi Govt. Dispensaries (allopalhlcj/hbspltals (allopathic) Situated In Delhi lilt(he timei ^MAs are appointed In Ihe NCR area.

IB. The jurisdiction of dispensaries will be defined and no change shall ordinarily be permiliedexcept on account of change of residence or opening of new dispensary.

iy. The card will have photograph of the pensioner duly certified by him/her.

v.: For this purpose. DGEHS Cardsshall be provided by Directorate of Health Services. All Headsof Department may send their consolidated requirement directly to Director, Health Services.The cards to the beneficiaries will be Issued by the concerned departments (from whereretired) fill a centralized mechanism for issue of smart DGEHS cards is introduced.

OFFICE MEMORANDUMSubject: Delhi Govt. Employees Health Scheme: Provisions for serving employees

A health scheme was formulated tor serving employees/pensioners of Govt. of National Capital Territoryof Delhi in 1997. The scheme was communicated vide letters No. F.No.27(9)/96-M&PH/ dated 13* March1997 & F.NO.27/9/97-H&FW/ dated 26* September 1997. The scheme has subsequently been revised videOM No. F-342/81/2003/H&FW/3264 dated 6* October 2003 and OM NO.F342/126/2004/ H&FW/7553-63dated 21" February 2005.

Consequent to review and rationalization of different provisions and procedures of the Delhi GovernmentEmployees Health Scheme, the following provisions/ modifications in the Delhi Government EmployeesHealth Scheme are notified for better Implementation of the scheme:

1.BENEFITS UNDER THE SCHEME;

I. The beneficiary shall be provided the facility of out-patient care, emergency services, freesupply of necessary drugs/appliances, laboratory and radiological investigations, specialistconsultation, family welfare services and treatment in specialized hospitals as per provisions ofCS(MA) rules.

it. The following benefits would be extended to the beneficiaries:

a Free admission In all the hospitals/Nursing Homes under Govt of NCT of Delhi according toentitlement and as per norms applicable to a serving Delhi Govt. servant of that Incomebracket. In case, any medicine is not available in hospital/dispensary, the hospital authoritieswould purchase the same and supply it to the eligible beneficiary and in case this Is notpossible, the beneficiary would be allowed reimbursement of such medicines.

b If the medical officer incharge of the hospital/Dispensary feels that specialist treatment Isrequired, he/she will refer the patient to the authorised medical institution providing suchspecialized treatment on priority basis,

c The treatment under Indian System of Medicine and Homeopathy will also be available asper CS(MA) Rules

iii. The Office Memorandum Issued by Govt. of India, Ministry of Health & Family WelfareF.No.Rec-24/2001/JD(M)/CGHS/Delhi/CGHS(P) dated 07.09.2001 shall form the basis for"Medical Facilities for serving Govt. Employees and Pensioners of Govt. of Delhi" with suchmodifications as are decided by the Government of Delhi from time to time.

Iv. Benefits of the scheme are prospective in nature I.e only after becoming a member of thescheme. Reimbursement shall be made only in case the employee is a member of thescheme.

2.MEMBERSHIP OF THE SCHEME:

i. The membership of the scheme will be compulsory for all serving employees of theGovt of NCT of Delhi, If they are not otherwise covered by any such scheme.

it Officers of Indian Administrative Service/Indian Forest Service of AGMUT cadre, officersof DANICS/UTCS cadres, including their family pensioners can also opt for the scheme.

111. Autonomous/statutory bodies fully funded by Delhi Govt may opt for Delhi GovernmentEmployee Health Scheme.

The jurisdiction of dispensaries will be defined and no change shall ordinarily be permittedexcept on account of change of residence or opening of new dispensary.

IV. The card will have photograph of the employee duly certified by him/her.

v. For this purpose, DGEHS Cards shall be provided by Directorate of Health Services. All Headof Departments may send their consolidated requirement directly to Director, Health Services,the cards to the beneficiaries will be Issued by the concerned departments till a centralizedmechanism for issue of smart DGEHS cards is Introduced.

•• y^^. In case of transfer of employee to another department, there will not be a need to issue freshDGEHS Card to the beneficiary. An endorsement on the existing DGEHS Card by the newdepartment will be required under intimation to concerned attached dispensary/hospital and toDGEHS Cell, Directorate of Health Services. The concerned departments, the concerneddispensary/ hospital and DGEHS Cell will update their records accordingly.

vti. In case of eligible officers as per para 2(ii) above, who are not serving in Government of NCTof Delhi, the DGEHS cards shall be issued by Directorate of Health Services,

5. ENTITLEMENT UNDER THE SCHEMEt The entitlement under the scheme for employees is as under:

4. ISSUANCE OF MEMBERSHIP CARD/ATTCHEMENT TO DISPENSARIES:

i Oil the basis of contribution, the employee would be issued a DGEHS Card which would entitlethem to free treatment in a Dispensary/Hospital of the Govt. of Delhi and treatment/diagnosticfacilities in recognized empanelled private hospitals/ diagnostic centers as per provisions of thescheme.

it All beneficiaries shall be attached/ registered with Delhi Government Dispensaries (Allopathic)/Hospitals (Allopathic). The beneficiaries residing in NCR area may opt for attachment cf theircards to any of Delhi Govt. Dispensaries (allopathic)/hospitals (allopathic) situated in Delhi tillthe time AMAs are appointed in the NCR area.

8. RECOGNISED HEALTH FACILITIES:-

i. All health facilities (hospitals/dispensaries) run by the Govt. of NCT of Delhi and autonomousbodies under Delhi Government, local bodies, viz. MCD, NDMC, Delhi Cantonment Board,Central Government and other Government bodies [such as AIIMS, Patel Chest Institute(University of Delhi) etc.] are recognized under the scheme. In addition, some privatehospitals/diagnostic centers notified from time to time are also recognized/ empanelled asreferral health facilities.

II. Employee beneficiaries residing in NCR areas outside Delhi are allowed to get treatment evenduring non-emergent conditions from Government Institutions and CGHS recognized centersafter referral from Government Institutions In concerned areas or in Delhi as per provisions ofthe scheme.

ill. The list of private recognized hospitals/diagnostic centers in Delhi has been displayed on DelhiGovt. web site. Any changes in the list shall be notified from time to time. An updated list maybe seen on Delhi Government web site at http://heaith.delhigovtnic.ln/health/files/dgehs.htm.

iv. Private recognized hospitals/diagnostic centers In NCR region outside Delhi for beneficiariesresiding In these areas will be same as for CGHS beneficiaries as notified by CGHS from timeto time, (list of CGHS recognised centres available at CGHS website :http://mohfw.nic.in/cghs.htm)

7.DGEHS OPERATIONS IN AREAS UNDER NCRi. Delhi Govt. Employees Health Scheme has been extended to NCR area In the interest of Delhi

Govt. Employees and pensioners residing in these areas allowing these beneficiaries to gettreatment even during non-emergent conditions from Govt. Institutions and CGHSrecognized centers after referral from Government Institutions in concerned areas or in Delhiand in addition to existing provisions for emergency medical treatment as per CS(MA) Rules asapplicable to Delhi and for reimbursement of the expenditure incurred to the concernedbeneficiaries as per CS(MA) Rules w.e.f. 2.1.2005.

8.MODE OF PROVIDING MEDICINES:-

I.In case of chronic illnesses requiring prolonged treatment, medicines may be supplied for onemonth at a time. However, In case of senior citizen beneficiaries with chronic conditions,requiring prolonged treatment, the same may be dispensed up to three months also on thedecision of the Incharge of the attached dispensary/hospital.

II.Medicines to the beneficiaries shall be provided by the concerned attacheddispensary/hospital. In case, any medicine is not available In hospital/dispensary, thehospital/dispensary authorities would purchase the same and supply it to the eligiblebeneficiary and in case this is not possible, the beneficiary would be allowed reimbursement ofsuch medicines.

9.MODE OF MEDICAL TREATMENT:-

i. The attached dispensary/hospital shall be the single window for taking care of healthrequirements of the beneficiary (primary health care, referrals, supply of medicines, etc.). TheAMA at the attached dispensary/hospital will be competent to accord permission/authorizationfor treatment/tests in DGEHS recognized private hospitals/diagnostic centers. There will notbe any need of permission of the concerned department from where the beneficiary Isworking in such cases.

I). The authorised medical attendant (AMA) for referral purposes will be the CMO/MO Incharge ofthe dispensary/hospital where the beneficiary is attached. For this purpose all hospitals shalldesignate a Staff Physician who will function as AMA for beneficiaries attached to the hospital.

til. The beneficiary will have the option of availing facility of treatment /diagnostic procedures etc.In recognized institution of his/her choice.

hr. Treatment anywhere outside Delhi will be considered for reimbursement in case of emergencyonly as per provisions of CSMA rules.

vf. Cashless treatment facility as per entitlement in recognized empanelled private hospitals/diagnostic center in Delhi will be available to serving employees in emergent conditions onproduction of valid DGEHS Card. These hospitals will be sending the bills to concerneddepartments (where employee is serving) for making payments which have to be made within60 days as per terms and conditions of the agreement entered Into with thesehospitals/diagnostic centers. The itemized bills will have to be accompanied by the followingdocuments (in duplicate):

1.Photocopy of valid DGEHS card of the beneficiary

2.Referral/authorisation from AMA or emergency certificate issued by thehospital concerned, as the case may be,

3.Essentiality certificate as per CS(MA) rules,

4.Certificate by the hospital to the effect that:

a. The treatment given by the hospital has been as per entitlement ofthe beneficiary and the rates charged have been as per DGEHSapproved rates.

5.Stickers/Invoices Indicating Serial Nos./Manufacturer etc. of implants etc. asper practice being followed under CGHS.

The bills should be itemwise and should be authenticate/ verified by the beneficiary or in caseof inability/death of patient, the bills should be authenticated/verified by the next of kin/legalheir of the beneficiary or attendant of the beneficiary.

A.OPD Treatment in nonamergent conditions:

L OPD specialist treatment In private recognized hospitals of the choice of the beneficiaryshall be available on the advice of the concerned AMA for a period not exceeding sixmonths In each instance. Any OPD treatment in private hospitals not recognized/empanelled under the scheme shall not be admissible for reimbursement.

B.IPD Treatment In nonemergent conditions:

i. Indoor treatment as per entitlement shall be available in private recognized hospitals ofthe choice of beneficiary during non-emergent conditions on the advice of the concernedAMA or on the advice of doctors of other recognized health facility duly endorsed by theconcerned AMA. The concerned AMA shall issue a referral/authorization to the optedHospital/ Diagnostic Centre for treatment. Follow up treatment subsequent to anyspecialized procedure or for illness shall ordinarily be valid for six months. '

ii. No claim from non-recognized /non-empanelled private hospitals/ diagnostic centres shallbe entertained.

iii. At present cashless treatment facility shall not be available for OPD/IPD treatment innonemergent conditions in private recognised hospitals/diagnostic centres.

C.Treatment in emergent conditions:

i. In emergent condition beneficiary can go to any of the recognized institution of his/herchoice directly without being formally referred by AMA. Cashless treatment facility inemergent conditions will be available to all working beneficiaries in recognizedempanelled private hospitals/ diagnostic center on Delhi on production of valid DGEHSCard.

ii. Follow-up treatment subsequent to any emergent treatment/ procedure or for the illnessshall be on authorization of concerned AMA.

lit Treatment lft private hospitals not recogntzed/hot empanelled under the scheme -1ft'medically emergent conditions wilt also be admissible when treatment is necessitated insuch hospitals being situated near the place-of illness/trauma and When no olherrecognized facility is available nearby or due to circumstances beyond the control of thebeneficiary.•

lr. However, reimbursement in such cases shall be made by the concerned departmentwithin the ceiling of DGEHS rates and there shall also be a provision for medical advance

- by the concerned departments (head of the department) of 90% of the estimatedexpenditure in Such conditions,

D. In any otner circumstances not mentioned above, the case shall be referred to the DirectorHealth Services, Govt:of NCI of Delhi for advice.

10.REIMBURSEMENT UNDER THE SCHEME:-i. Reimbursement under the scheme to Serving employee benefidartes will continue to be made

through concerned departments,

IL The treatment taken In recognized Government and recognized private hospitals/diagnosticscentres shall be reimbursable in full to the beneficiaries as per entitlement.

Ill The approved DGEHS rates for reimbursement will be In accordance with OM No.F342/7/2002-H&FW /9422 dated 25.10.2002 or as modified/notified from time to thine. Thelatest approved rates are available on Delhi Govt web site,

11.DELEGATION OF POWER TO SANCTION MEDICAL ADVANCEI The Heads of Departments of Delhi Government are competent to,sanction medical advance

to the extent of 90% of estimated cost of treatment for major/serious Illnesses In accordancewith the provisions of CS(MA) Rule 1944,

12.APPLICABLE RULES UNDER THE SCHEME
