APPLICATION GUIDELINES
Follow the steps below to complete your application to QUTs Vice-Chancellors STEM Camp:
1. Complete the attached Application Form.
2. Write a 300 word Statement of Purpose describing your interest in science, technology, engineering and mathematics.
3. Ask your parent or guardian to sign your application.
4. Meet with your Teacher, Guidance Officer or Principal to discuss your application and ask them to complete the attached Letter of Recommendation Form supporting your application.
5. Provide us with a copy of your Term 4, year 10 and Term 1 year 11 high school academic report.
6. Post or email your application by 5pm, 6 May 2013.
PLEASE NOTE:
A completed application does not guarantee acceptance into the Vice-Chancellors STEM Camp.
Offers of a place will be made on the basis of information given on your application.
The decisions of the selection committee are final.
Students must be able to stay for the duration of the Vice-Chancellors STEM Camp.
Successful applications will be notified in writing in mid-May.

SUBMITTING YOUR APPLICATION
This application form must be completed in full, along with the supporting documentation, and returned by mail or email by Monday, 6 May 2013.
Incomplete or late applications cannot be processed.

STATEMENT OF OBLIGATION
If I am selected for the Vice-Chancellors STEM Camp, I agree to comply with the safety regulations and instructions of the Vice-Chancellors STEM Camp management and staff during my participation in the event.
In particular, I agree to comply with the rules: no smoking, no alcohol, no drugs, and appropriate mobile phone use. I understand that if I fail to keep to these rules, I may be returned home by the first available transport, with any costs incurred being payable by my parent/guardian.

DECLARATION OF PARENT/GUARDIAN
I, the parent/guardian of (name of applicant) approve of his/her participation in the Vice-Chancellors STEM Camp at the Queensland University of Technology, Monday 24 to Friday 28 June 2013.
If they are selected, I agree to deliver the student to the departure point (closest airport/train) in time for scheduled trip.
I understand that my child/ward, if offered a place, will be required to comply with the instructions and rules of the Vice Chancellors STEM Camp management and staff. I understand that this application does not guarantee acceptance into the Vice Chancellors STEM Camp, that an offer of a place is based on the information given in the application and that the decisions of the selection committee are final.
I note that my child/ward has accepted the Statement of Obligation set out in Section 1 of this Application. I endorse their undertaking and accept the consequences of a serious breach of their obligation.
