Senior Activity Centres Senior Activity Centres encourage the elderly to remain integrated in society through community outreach programmes and social recreational activities. They are commonly located at the void decks of selected 1- and 2-room HDB rental blocks. Elderly living in these blocks are encouraged to spend the day at the centres which provide opportunities for interaction and exercise. All Senior Activity Centres are linked to an alarm system which can be activated during emergencies.

Pusat Kegiatan Warga TuaPusat Kegiatan Warga Tua menggalakkan warga tua terus berintegrasi di dalam masyarakat melalui program-program penglibatan masyarakat dan kegiatan riadah sosial. Biasanya, ia diadakan di kolong-kolong blok flat sewa HDB 1 dan 2 bilik. Warga tua yang tinggal di blok-blok ini digalakkan untuk meluangkan masa di pusat-pusat tersebut yang menyediakan peluang untuk berinteraksi dan bersenam. Semua Pusat Kegiatan Warga Tua dilengkapi dengan sistem penggera yang boleh diaktifkan jika berlaku kecemasan.

Hospice Day Care Centres

Hospice Day Care services refer to services that provide comfort and support for patients who are terminally ill. The services include pain management and treatment for the patients, as well as helping to prepare the patient and his/her family for coping with end-of-life. The services include Hospice Day Care, Hospice Home Care and Inpatient Hospice Care. Hospice Day Care is for patients with cancer and other life-limiting illness. The patients are generally mobile and/or wheelchair dependent.

CREST (Community, Resource, Engagement and Support Team)CREST serves as a community safety network for people with and/or at-risk with depression and dementia, and support their caregivers to care for them so as to continue staying in the community. Each CREST comprises social service professionals to conduct outreach to raise public awareness of mental health, identify residents at risk of mental health issues such as dementia and depression and link them to services and assistance, and provides emotional support to clients and caregivers.

ASCAT (Assessment and Shared Care Team)ASCAT is a physician-led, multi-disciplinary team, comprising of psychiatrist or GDMP trained family physician and allied health professionals including nurses and therapists, to provide assessment, treatment and care to clients with mild and moderate mental health conditions. ASCAT may be sited in an accessible setting in the community, to enable clients to receive care closer to their home. This allows clients with mental health conditions to be detected and treated early.

CST (Community Support Team) CST provides community response for clients who are in pre-crisis mode and serves as a support network for clients with mental conditions and their caregivers by providing assessment, pre-crisis intervention and linking them to the appropriate mental health care and social support networks. Led by non-clinical professionals, the programme focuses on providing basic emotional support, promoting mental health awareness, conducting outreach events to perform basic recognition of signs and symptoms of mental conditions and conducting home visits for pre-crisis intervention and link them to appropriate help.

CST(社区支持小组)CST 通过提供健康评估、提前干预和帮助联系合适的精神健康护理机构和社会援助网络，为处于危机前模式的精神健康疾病患者及其看护者提供社区支持。社区支持小组由非临床专业人员领导，主要致力于提供基本的情感支持，提高公众对精神健康的意识，帮助公众了解精神健康疾病的迹象和症状，上门提供危机前干预治疗，帮助患者及其家属联系合适的护理和治疗机构。

Pasukan Sokongan Masyarakat (CST) CST menyediakan bantuan dalam masyarakat kepada klien yang berada pada tahap pra krisis serta menyediakan jaringan sokongan untuk klien yang mengalami masalah mental dan juga pengasuhnya melalui penilaian, intervensi pra krisis dan menghubungkan mereka kepada perkhidmatan penjagaan kesihatan mental dan jaringan sokongan sosialnya. Pasukan ini diketuai oleh karyawan bukan perubatan dengan menjalankan program yang bertumpu kepada sokongan emosi yang asas, menggalakkan kesedaran terhadap kesihatan mental, menjalankan acara penglibatan masyarakat untuk mengenali tanda-tanda dan gejala-gejala masalah mental serta menjalankan lawatan ke rumah untuk intervensi pra krisis dan menghubungkan mereka kepada khidmat bantuan yang diperlukan.

Person-Centred Home-Based InterventionHome Intervention programme providew behavioural interventions to clients and support for caregivers of people living with dementia. The inclusion criteria include a diagnosis of dementia and clients exhibiting behavioural issues resulting in caregiver burnout or stress. It provides support, training and education to caregivers to impart the knowledge and skills to tackle issues of behavioural issues exhibited by the client.

ASCAT (Assessment and Shared Care Team) ASCAT is a multi-disciplinary shared care team led by a

doctor providing medical treatment and care to clients with mild and moderate mental health conditions in the community.

Caregiver Services Caregiver Services are designed to meet caregiver needs

and to support them in their care giving journey. This includes training and social support services.

Caregiver Support Support groups help caregivers in their caregiving

journey. Enabling them to talk about their problems and listen and help others. Support groups also facilitate the sharing of knowledge and support, which can be invaluable.

Caregiver Training Caregiver training programmes are conducted by service

providers to help individuals learn important techniques and useful tips on how one can provide a safe, healthy and supportive environment for their care recipient. Caregiver training is typically conducted at the patient’s bedside or at training facilities. The training is usually conducted by a multi-disciplinary team, comprising of nurses, physiotherapists and occupational therapists.

Caregivers Training Grant (CTG) The Caregivers Training Grant (CTG) provides subsidies

for caregivers of seniors or persons with disabilities to attend training that builds their capabilities to care for the physical and socio-emotional needs of their care recipents.

Centre-Based Weekend Respite Care Centre-based weekend respite care centres provide

support and care over weekends. Services include assistance with activities of daily living (such as eating, toileting and moving around), medication and general administration. General exercise, social activity programmes and meals are also provided.

Chronic Sick Hospitals Chronic Sick Hospitals provide long term skilled daily

nursing and frequent medical care to older persons with advanced, complicated medical conditions with poor functional ability (i.e. highly dependent or unable to perform activities of daily living)

COMIT (Community Intervention Team) COMIT comprises of allied health professionals such as

mental health counsellors, nurses and psychologists. Who provide counselling, psychosocial therapeutic interventions and support for clients with mental health needs and their caregivers.

Community Health Assist Scheme (CHAS) CHAS enables middle to lower income Singaporeans to

receive subsidies for treatment at participating General Practitioners (GPs) and Dental clinics near their homes.

Community Hospitals Community Hospitals are intermediate care facilities. They cater to patients who are fit for discharge from government, restructured or private hospitals but still require short-term, stay-in care and therapy for recovery, often from a stroke or fracture.

Community Services Community Services link residents to health and

eldercare services in the community.

Community Mental Health Community Mental Health services provide accessible

and integrated mental health support and intervention for clients and their caregivers.

Counselling Service Counselling services aim to enhance the total wellbeing

of seniors and their caregivers by focusing on the psychological and social aspects of their health. It provides an avenue for seniors and their caregivers to voice their concerns and problems, reducing their stress, anxiety and tension.

CREST (Community, Resource, Engagement and Support Team)

CREST seeks to raise mental health awareness to the public through outreach events, targeted activities and home visits. The team also helps to identify residents at risk of mental health issues such as dementia and depression and links them to services and assistance,

and provides emotional support to clients and caregivers.

Community Support Team (CST) Led by non-clinical professionals, the CST focuses on

providing basic emotional support, promoting mental health awareness, conducting outreach events to perform basic recognition of signs and symptoms of mental conditions and conducting home visits for pre-crisis intervention and link them to appropriate help.

Day Centres Day Centres provide allied health, social, mental health

and basic nursing care services for their clients.

Day Care Centres (Social) Day Care Centres (Social) provides care and maintenance

exercise to frail elderly who may need supervision while their families and caregivers are at work. Such centres also give the elderly opportunities to socialise and take part in recreational activities, enabling them to remain in the community for as long as possible.

Day Rehabilitation Services Day Rehabilitation Centres provide rehabilitation services

to patients who require therapy such as physiotherapy and/or occupational therapy to help them regain their ability to perform daily activities such as walking, dressing and showering.

Dementia Day Care Centres Dementia Day Care Centres provide day care

programmes for elderly with dementia and their caregivers, teaching and training them to care for themselves at home.

Elder Protection Services Elder Protection Services aim to help individuals and

families build safe relationships. They offer a range of services for people who are trying to manage issues of family violence, elder abuse and elder care.

Eldersitter The eldersitter programme provides respite for

caregivers of people living with dementia while promoting cognitive stimulation of clients with dementia. Clients with dementia will be engaged by eldersitters in meaningful and therapeutic activities to maintain their cognitive function through activities that are designed to meet their interests and abilities.

Enhancement For Active Seniors (EASE) EASE was introduced by HDB to enhance the safety and

comfort of elderly living in HDB flats. Flat-owners can choose the type of improvement they need for their home and the cost of the improvements are subsidised by the government.

Foreign Domestic Worker (FDW) Levy Concession (Persons with Disabilities)

FDW Levy Concession enables families to pay a lower monthly levy when employing full-time caregivers to help look after their loved ones with disabilities.

Foreign Domestic Worker Grant (FDW Grant) The FDW Grant aims to provide more support to families who care for frail elderly or person with disabilities, as long as he/she has moderate disabilities.

Home Care Services Home Care Services refer to healthcare services that are provided within the homes of the elderly.

Home Medical Home Medical Services cater to the frail or bedridden elderly who need medical consultation and treatment but are unable to leave the house.

Home Modification Home modifications aim to make the home safer for care recipients by reducing the incidence of accidents. This is done by modifying the home to help care recipients perform daily activities within his/her ability safely.

Home Nursing Home Nursing Services are nursing services for the elderly who are homebound or bedridden and require nursing care such as wound dressing, injections, changing of feeding tube, etc.

Home Therapy Home Therapy Services provide homebound elderly who are unable to go to a Day Rehabilitation Centre but require rehabilitation to improve/maintain their activities of daily living like dressing, feeding, mobility, etc.

Hospice Day Care Centres Hospice Day Care is for patients with cancer and other life-limiting illness. The patients are generally mobile and/ or wheelchair dependent.

Hospice Home Care Hospice Home Care cater to those who are homebound and requires care and support in managing with end-of-life. It also includes training for the caregiver of the patient and support for the caregiver.

Hospice Care Hospice Care Services refer to services that provide comfort and support for patients who are terminally ill. The services include pain management and treatment for the patients, as well as helping to prepare the patient and his/her family to cope with end-of-life matters. The services include Hospice Day Care, Hospice Home Care and Inpatient Hospice Care.

Neighbourhood Links Neighbourhood Links serve as an extended social support system for elderly living in the community. They provide a local network of support and promote active volunteerism at the neighbourhood level.

Person-Centred Home-Based Intervention The Person-Centred Home-Based Intervention programme provides behavioural interventions to clients and support for caregivers of people living with dementia. It provides support, training and education to caregivers to impart the knowledge and skills to address issues of behavioural issues exhibited by the client.

Pioneer Generation Disability Assistance Scheme (PioneerDAS) Provides $100 a month to Pioneers who permanently need three or more of these activities: eating, bathing, dressing, transferring from bed to chair and vice versa, toileting and walking or moving around.

Private Hospitals A private hospital is a hospital owned by a profit company or a non-profit organization and are often privately funded through payment for medical services by patients themselves, by insurers and sometimes by government.

Private Nursing Homes The elderly can be admitted into a nursing home if they need daily skilled nursing care and/or assistance in activities of daily living and they have no caregiver to look after them at home. The elderly must be semi-ambulant, wheel-chair bound or bed bound. Elderly sick with medical conditions, e.g. stroke, diabetes mellitus with complications, head or spinal injury etc, and who require nursing care, are eligible for nursing home care.

The nursing homes provide a range of services to meet the needs of the residents. The services include medical care, nursing care, physiotherapy, dietary services and dental care. There two types of nursing homes.

Private Nursing Homes (which are not under the MOH portable subsidy scheme) These are nursing homes, which cater to full-paying patients.

Psychiatric Day Care Centres Psychiatric Day Care Centres are set up with the aim

of returning psychiatric patients to a normal life in the community and relieving strain on family and caregivers. Programmes along with a therapeutic team cater to the individual’s needs wherever possible, catering to a broad range of issues.

Psychiatric Homecare Service Psychiatric Homecare Services provide stay-in care for

patients with psychiatric conditions. The care provided includes nursing and medical care, as well as therapy services.

Psychiatric Nursing Homes Psychiatric Nursing Homes provide stay-in care for patients

with psychiatric conditions. The care provided includes nursing and medical care, as well as therapy service.

Psychiatric Rehabilitation Homes Psychiatric Rehabilitation Homes provide structured

rehabilitation programmes to re-integrate patients into the society and their normal work settings. The care services provided enable them to return home to

stay with their families.

Respite Care Services Respite Care Services provide caregivers who are taking a short break and need temporary care service for the elderly they are taking care of.

Public Hospitals Public hospitals runs as private companies wholly-owned by the government. These hospitals receive an annual government subvention or subsidy for the provision of subsidized medical services to the patients.

Senior Activity Centre Senior Activity Centres encourage the elderly to remain integrated in society through community outreach programmes and social recreational activities. They are commonly located at the void decks of selected 1- and 2-room HDB rental blocks. All Senior Activity Centres are linked to an alarm system which can be activated during emergencies.

Senior Care Centres Senior Care Centres are integrated day care facilities which feature many different eldercare services in one convenient location, enabling clients to benefit from these services and enhance their overall social, mental, and physical wellbeing. Services include active rehabilitation, dementia care and community nursing for clients with higher care needs.

Seniors Activity Centres Seniors Activity Centres are located at the void decks of selected 1- and 2-room HDB rental blocks. Elderly living in these blocks are able to spend the day at the centres which provide opportunities for interaction and exercise. These centres encourage the elderly to remain integrated in society through community outreach programmes and social recreational activities.

Seniors Activity Centres (Cluster Support) Senior Activity Centres (Cluster Support) are facilities which are equipped with social work professionals to provide casework management, care planning and counselling services. They are run by VWOs who work with other service providers and local community partners to develop a network of support for elderly in need of assistance.

Seniors Home Care Seniors Home Care offers a variety of personal care options to support caregivers in looking after their frail elderly at home. Caregivers can choose from dedicated service packages such as meal delivery or escort, or an ensuite package, which includes a range of personal care services like housekeeping and personal hygiene.

Sheltered Homes for the Aged Sheltered Homes for the Aged provide residence to ambulant elderly who are without next-of-kin or are unable to live with their family members. These Homes also provide some support services to assist and enable the elderly to maintain their independence within the community.

SPICE In SPICE, an inter-disciplinary team delivers an integrated suite of medical, nursing, rehabilitation, social and personal care services, both in the centre and at the participant’s home, dependent on the needs. A comprehensive assessment, regular re- evaluation and detailed individualised care plan is tailored and implemented to enable these frail elderly to avoid institutional care.

Stay-In Facilities Stay-In Facilities offer 24-hour access to healthcare services.

VWO Nursing Homes VWO nursing homes provide a range of services to meet the needs of the residents. The services include medical care, nursing care, physiotherapy, dietary services and dental care.

Some VWO-operated Nursing homes do not receive MOH subsidies, but are funded through their own fund-raising, The VWOs are able to provide the necessary financial and social assistance to patients who are unable to afford nursing home fees.
