Registering for HealthcareWhen active-duty military members

arrive in Hawaii, their personnel office willensure that they in-process through theirservice’s Military Treatment Facility (MTF)and learn about their TRICARE HealthcareSystem options.

TRICARE Prime, the military’s managedcare option, is available at Tripler ArmyMedical Center, Schofield Barracks HealthClinic, Marine Corps Base Kaneohe Clinic,the Navy’s Makalapa Clinic, the CoastGuard’s Sand Island Clinic and the AirForce’s Hickam Clinic.

Each MTF conducts orientation ses-sions at their enrollment sites to explainTRICARE Prime. Spouses are encour-aged to attend the orientation with active-duty service members. You and your

family are assigned a Primary Care Man-ager (PCM) at a clinic that will manageyour healthcare.

Clinic Orientations for Newcomers & Newly EligibleTRICARE Prime Enrollees

Tripler’s Adult Medicine, Family Medi-cine, and Pediatric Clinics conduct clinicorientations for new arrivals and newly eli-gible Tripler TRICARE Prime enrollees.These orientations aim to improve primarycare access and beneficiary satisfactionby allowing TRICARE Prime enrollees tomeet with their primary care teams and tolearn the best way to access the Triplerhealthcare system.

Orientations cover a variety of topics,including:• Clinic hours.• How to make an appointment.• How to contact your Primary Care

Manager (PCM).• After hours care options.

• How to change your PCM.• How to refill a prescription when away

from Tripler.• How to obtain a “Self Help” healthcare

guidebook.• Other hassle reduction and preven-

tion tips.

Army One Source: 24/7 Customer Service

The following services are available 24hours a day, seven days a week, withtrained individuals to assist with yourfamily-related issues and concerns. All ofthe One Source Web sites are password-protected. To gain entry to the Web site,you can use “army” as the user name and“onesource” as a password the first timeyou visit the Web site. Soldiers are enrolled in the DefenseEnrollment Eligibility Reporting System(DEERS) when they enter active-duty. Forfamily members to be enrolled, however,their military sponsors must verify theirfamily relationships by presenting legaldocuments (marriage certificates, birthcertificates or adoption papers, etc.) to

their local military personnel office. If thisis not done, family members may bedenied health benefits other than emer-gency care. Newly born infants should beenrolled in DEERS as soon as possibleafter birth.

Electronic Medical Records (CHCS II)

The Department of Defense’s newComprehensive Health Care System II(CHCS II) is being implemented at all DoDmedical facilities world-wide. In May 2005,Tripler and Schofield Barracks began thetransition, which will continue through var-ious phases for several years. Patients’medical records and charts, now mostlypaper-based, are being converted to elec-tronic records stored in centralized data-bases accessible to healthcare providers atmedical treatment facilities wherever thepatients are assigned.

Use the Nearest EntranceFinding your way around Tripler’s Many Wings

If this is your first time visiting Tripler,please read this section carefully, to findyour way successfully around the hospital.The lower half of Tripler (Wings A throughE) are part of the original design of Tripler,when it was built in 1948. Wings F throughH were expansions added in the 1980s.The two-phase construction and its site ona mountain slope have given Tripler uniquearchitectural features.

Main Trunk, A and G Wings. The pinkbuildings that comprise Tripler Army Med-ical Center has a north-south trunk, theinterconnected A and G “wings.” The AWing facing the flagpole is entered throughthe Oceanside main entrance.The G Wing,facing north, is entered through the Moun-tainside main entrance.

Clinic, the Army Dental Clinic, and theChapel are in the D Wing.

C and E Wings. The C Wing branchesto the east side from the A Wing. From abend in the C Wing, you can enter corridorson the first and third floors to reach theE wing. The E Wing also has its ownstreet-level entrance. The Veterans AffairsRegional Office is in the E Wing.

F, G and H Wings. The G Wing, thenorth half of Tripler’s A-G trunk, is usuallyentered from the Mountainside entrance onPatterson Road. The F (west) and H (east)wings are like the legs of a fat “H,” with theG Wing joining the two legs.

Going between A and G Wings.Because Tripler is built on a steep moun-tain slope, the Mountainside street-levelentrance is the on fourth floor of Tripler. Togo from the Oceanside entrance (A wing)to the Mountainside (G wing) entrance,take the A-wing elevator to the fourth floor.Then take the main corridor that connectsthe A wing to the G wing. You will knowyou are entering the other wing when youreach a short stairway (an elevator isnearby for those unable to use the stairs).Passageways from the 3rd and 4th floors

also connect the Oceanside B and Cwings to the Mountainside G Wing.

Elevators: Because the Tripler wingshave different number of floors, elevatorslocated in them do not go to all floors.

When you go to Tripler, take the entrancenearest to the wing in which you have anappointment.

Patient/Visitor Parking Areas. Patientsand visitors have priority in all Tripler parkinglots, including the lots nearest to the Moun-tainside and Oceanside entrances to Tripler.The Tripler civilian police officers issue cita-tions to Tripler employees not authorized topark in a patient lot.

VA Garage. Veteran Affairs (VA) andTripler patients may park their vehicles inthe Veterans Affairs’ parking structure onthe Mountainside of the hospital, at P1, P2and P3 levels.

Employee Parking. Tripler staff andcontractor personnel should park on thelower level Oceanside parking lots. Theymay also park in the basement and rooftop(PB and P4) levels of the VA garage.

Additional paved parking stalls arelocated at the site of the old officer’s clubon the hillside directly above Tripler’smountainside entrance. This parking lot isopen to patients and staff on a first-come,first-serve basis.

Parking lot security. Please lock yourvehicles when parking anywhere atTripler. Security personnel do patrol theparking areas, and vehicles left there forextended periods attract their attention,but they may also attract the attention ofvandals and burglars.

Overnight parking of vehicles. If youexpect to be at Tripler for an extendedstay, we recommend that you leave yourvehicle at home. If you decide to parkyour vehicle at Tripler during an extendedstay, you should:• Park the vehicle in the Oceanside

parking lot near the flagpole.• Ensure that all valuables are removed

from the vehicle or are placed outof sight.

• Ensure all windows are up and that thevehicle is properly secured.

• If the vehicle will remain within Tripler’sparking area longer than 72 hours, notifythe Tripler Police by calling 433-1111.

• Patients should be able to provide adescription of their vehicle and licenseplate number. This will help to ensurethe security of the vehicle and prevent itfrom being towed away as an aban-doned vehicle.Speed Limits. The maximum speed

limit in any Tripler parking lot is 10 mph.The speed limit on Tripler Hill is 25 mph.These limits are radar enforced. Viola-tors are issued summons, which requireappearance at the U.S. District Court.Drive carefully and be alert for pedes-trian traffic.

Which patients should go to theEmergency Room (ER)? The ER isdesigned to care for problems that requirerapid attention by a physician, such as asuspected heart attack, loss of conscious-ness, difficulty in breathing, a broken boneor other severe injuries.

Patients with emergencies have priorityover non-emergent conditions. Patientswith minor problems (most rashes, colds,etc.), problems that are a few days old, or

those who need prescription refills, shouldnot seek evaluation in ER.

Sick call in the Adult Medicine Clinic(AMC) is for:1. All active-duty Soldiers assigned to the

Sick call is for acute medical illnesses/injuries that might prevent military mem-bers from performing their assignedduties. The Hawaii State Department of Healthrequires that all birth certificates be filedwithin one week of the child’s birth. Birth

certificates are legal documents and, assuch, must be letter perfect.

Parents should complete the birth certifi-cate worksheet as soon as their baby isborn and take it to the birth certificateoffice. The birth certificate clerk will provideparents with a statement of birth in order toupdate personnel records and to enroll thenewborn baby into DEERS.

You can order certified copies of the birthcertificate from the state’s Department ofHealth by mail: State Department of Health,Vital Records Section, P.O. Box 3378,Honolulu, HI 96801 or through its Web site:http://www.hawaii.gov/health/vitalrecords/vitalrecords/vital_records.html.

It usually takes about four to six weeksbefore certified copies are available.

The Social Security card is ordered auto-matically and takes about 13 weeks toreceive, unless you do not want it automati-cally sent to you or if you had multiple births.In that case, you will need to apply for thecard at the Social Security office. Call thebirth certificate clerk at 433-2031 if you havequestions (i.e. unwed, divorced, alien, etc.).

Newly remodeled birthing suites providenew mothers with a comfortable and homeylabor and delivery experience. The large,wide-open rooms come furnished withstate-of-the-art medical equipment, cabi-nets for storage, baby warmers, over-stuffedsleeper chairs, and an island touch with amountain window view and seascape pat-tern on the ceiling. Currently several roomsoffer Jacuzzi tubs to ease labor discomfort.

• New mothers are offered a specialcelebratory meal for her and a guest.All staff physicians providing obstetrical

care are fully credentialed and are eitherboard-certified or board-eligible with theirnational organizations.

A team of pediatricians attends all cae-sarean deliveries and other high-risk deliv-eries. Labor and Delivery provides painmanagement services with 24-hour avail-ability and highly skilled nurse anesthetistsand anesthesiologists.

The OB/GYN department has severalwomen’s health nurse practitioners andcertified nurse midwives. Women may alsochoose to be followed through their preg-nancy by their family practice physician.Currently Family Medicine sees obstetricalpatients at Tripler, Schofield Barracks,Kaneohe Bay and Pearl Harbor.

Same-Day Emergency ClinicThe same-day emergency clinic offers

the patient reports to the Outpatient Phar-macy. Handwritten prescriptions written bycivilian providers are filled on the new pre-scription side as well.

The first step is to take a ticket from theticket printer located next to the column inthe lobby. Have a seat and wait for yournumber to be called. An automated voicewill direct you to a window. Present to staffat the window the military ID of the patientwhose prescriptions are being filled.

Prescriptions will not be filled withoutthe patient’s ID. While you are at thewindow, your prescription will be filled anddispensed.

The Outpatient Pharmacists’ busiesttimes are Monday through Friday from10 am to 1 pm and 3 to 5 pm, and Sat-urday from 8:30 am to noon. In order toavoid longer waits, patients should planahead to pick up their prescriptions attimes other than these.

As part of our commitment to serveyou better, we offer a will-call service fordropping off prescription requests whichwill be ready in one hour for pick up. Touse this service, speak to our pharmacyreceptionist.

The Lean Program This comprehensive day treatment pro-

Patients learn healthy nutrition, behaviormodification, healthy exercise and relapseprevention.

Pain Rehabilitation ProgramA pain psychologist directs this compre-

hensive center for pain diagnosis, treatment,research and education. A multidisciplinaryteam of physicians, nurses and other healthprofessionals are dedicated to evaluatingand treating persons seeking relief fromchronic pain. The program aims to reduceor eliminate pain and suffering, restoreoptimum levels of function, facilitate returnto work, promote judicious use of pain med-ications and enhance the patients’ qualityof life.

COPE Program(Coping with and Overcoming Pain Effectively)

This six-week outpatient group treat-ment program assists individuals in chronicpain understand the mind-body connec-tions in how they perceive pain. They learntechniques to improve how they managechronic pain, in conjunction with their med-ical treatment.

Topics include the effect of chronic painand stress on the body; how to reduce thephysiological effects of pain and stress;lifestyle habit changes, such as sleep, exer-cise and nutrition that are all affected bypain. Individuals learn coping strategies

Tobacco Cessation ProgramThis 12-week group program addresses

the physical, psychological and socialaspects of successful tobacco cessation.Since 1984, this program has helped thou-sands quit, by teaching smokers copingskills and providing them with support andnicotine replacement. Meetings are held onWednesday from 2 to 3 pm, or Tuesdayfrom 4:30 to 5:30 pm.

Adult PsychologyAdult Psychology provides psychological

evaluations and treatment for individuals,families and couples. Treatment includesindividual and group therapies for depres-sion, anxiety and other conditions thatcause distress. Active-duty should seekservices from their own mental health clinic.Referrals to this Tripler program should bemade through the primary care physician.

Neuropsychology ServiceClinical neuropsychology is a branch of

psychology that systematically focuses onhuman brain functioning and provides valu-able clinical data regarding known or sus-pected problems with the brain.

Neuropsychological evaluation is oftenrequested for persons with brain injury,stroke, infectious CNS disease, dementia,seizures, brain tumors, movement disor-ders and for any other conditions that mayaffect the brain.

Neuropsychological test results can beuseful in accurate diagnosis, predictingprognosis, documenting recovery, providinginformation for medical boards, neurobe-havioral management, neuropsychologicalrehabilitation, for resolving return-to-work orreturn-to-duty questions, and in formulatingtreatment plans.

Services are available to both inpatientand outpatient DoD and Veterans Affairsbeneficiaries. Services are available for the

full age span of adulthood. A memory dis-orders clinic for evaluation and treatmentrecommendations is available for those withmemory problems or cognitive concerns.Neuropsychology works closely with allmedical clinics, including Family Medicine,Neurology, Psychiatry and Neurosurgery,as part of a multidisciplinary approach todiagnosis and treatment.

Medical Social WorkMedical Social Work has three main

Tripler Army Medical Center operates the U.S. Army Health Clinic – Schofield Barracks(USAHC-SB), located on Waianae Avenue and McCormack Road. The clinic is now oneof the largest outpatient clinics in the U.S. Army. Military personnel and their familiesassigned to other installations should inquire about routine care at the health clinic serv-icing their installation.

The clinic was constructed between 1928 and 1929 as a hospital and was used exten-sively during World War II. In 1965, hospital services were transferred to Tripler ArmyMedical Center, and the Schofield clinic expanded its outpatient services.

Family Medicine Every active-duty family whose sponsor

is stationed at Schofield Barracks, or whosesponsor resides at Schofield Barracks, mayenroll in the Family Medicine Servicethrough TRICARE. Retired military and theirfamilies enrolled in TRICARE may choosethis clinic for their healthcare needs.

A personal physician is assigned to everyfamily enrolled in Family Medicine.The clinicprovides medical services to family mem-bers of all ages (infants to adults), includingroutine medical care, gynecological care,

routine physical examinations and wellbaby checks, school physicals, and minorsurgeries and wart removal.

Health Promotion Program. The Health-care Promotion Program staff focuseson education and prevention of tobaccoand alcohol use, stress, nutrition, safety,family violence, physical fitness and repro-ductive health.

The US Army Medical DepartmentActivity-Japan (MEDDAC-J) provides outpa-tient primary care and limited specialty careservices to active-duty personnel, retiredmilitary, Department of the Army civilians,and their families. Although the main serviceof MEDDAC-J is primary care, physicaltherapy, optometry and behavioral healthservices are also available.

StaffingMEDDAC-J is staffed by Family Medi-

cine and general medicine physicians anda nurse practitioner who see all authorizedbeneficiaries. The Behavioral Health Ser-vice is staffed by a clinical psychologistand social workers who provide individual,marital, family, and group counseling.

Host Country Medical ServicesWhile active-duty and their family

members and retirees and their eligiblefamily members receive medical care atthe clinic at no cost, civilians are chargedfor care administered at either the militaryor Japanese healthcare facilities. TheMEDDAC-Japan Community RelationsOfficer will liaison between the host nationhospital and the patient to ensure promptpayment to the hospital. Payment mecha-nisms differ, depending on the beneficiarycategory of the patient. If you have anyquestions concerning payments, pleasecontact the MEDDAC-J Community Rela-tions Officer at 263-8197.

Other Military Treatment Facilities

Specialists in pediatrics, surgery, internalmedicine, obstetrics, dermatology, urology,allergy and radiology are available at localU.S. Air Force and Navy hospitals. Referralby a MEDDAC-J provider is necessary foran appointment to see a specialist. How-ever, MEDDAC-J personnel will assist inobtaining these appointments once theprimary care manager refers the patient tospecialty care. MEDDAC-J also provides adaily shuttle to and from Navy and Air ForceMedical Treatment Facilities.

In-Hospital CareBeneficiaries requiring in-hospital care

are referred or evacuated to the 374thMedical Group Yokota, U.S. Naval HospitalYokosuka or host nation medical facilitiessuch as Kitazato University Hospital inSagamihara City if the required care is notavailable in the military system.

Emergency CareMEDDAC-J does not have an emer-

gency medicine department. Emergencymedical services may be obtained bycalling 911. For 911 calls initiated onCamp Zama, a MEDDAC-J ambulancewill transport the patient to the appro-priate host nation hospital with the capa-bility and capacity to meet the medicalneed. For 911 calls initiated on Sagami-hara Family Housing Area or SagamiDepot, a host nation ambulance willtransport the patient to the appropriatehost nation hospital with the capabilityand capacity to meet the medical need. Inall cases where a U.S. patient is taken toa host nation hospital, MEDDAC-J willsend a translator to the hospital to facili-tate communication between the patientand the clinical staff.

Once you have met your deductible, yourTRICARE benefits begin to help pay foryour healthcare costs. At the beginning ofeach fiscal year (Oct. 1), you start a newdeductible.

A “cost share” is the cost of healthcareyou are responsible for. It is dependentupon your sponsor’s military status andpay grade. Your cost share is usuallybased on a percentage of the CHAMPUSallowable charge. This option works best ifyou have other health insurance.

To be eligible for TRICARE Standardbenefits, you do not need to enroll in anyprogram. You must simply be eligible forCHAMPUS.

TRICARE ExtraAs a TRICARE Extra beneficiary you

can save money by selecting a physician

TRICARE Dental ProgramThe TRICARE Dental Program (TDP) is

a voluntary, comprehensive dental insur-ance program offered worldwide by DoD tofamily members of all active-duty uniformedservice personnel and selected Reserveand Individual Ready Reserve (IRR) mem-bers and/or their family members.

United Concordia administers this pro-gram through which family members andreservists receive dental care from civiliandentists in the local community. Active dutymilitary are encouraged to enroll their familymembers in TDP; otherwise they should beprepared to pay 100 percent of family mem-bers’ dental costs in the civilian community.

Food Stamp/FSSA (Family Subsis-tence Supplemental Allowance) Work-shop assists families in completingforms that may qualify them to receivefood stamps.

Financial Readiness Program providesa variety of services to the Army familycommunity, including consumer complaintand problem resolutions, individualizedfinancial counseling for newly assigned Sol-diers and military families to address payproblems, budgeting, transition budgeting,and asset analysis and financial planningsessions.The program provides informationon a variety of financial subjects such aslow to moderate income housing and guide-lines on managing household expenses.ACS also has a consumer library withbooks, magazines and pamphlets on var-ious financial subjects.

Information and Referral Service isthe “go-to” starting place to find answersto any problem, concern or question. Thestaff assesses your situation and providesinformation and/or referrals to other serv-ices, military and civilian, that can help inresolving personal, financial, family andmarital problems.

Lending Closet provides temporaryloans of household items to personnelarriving and departing Hawaii, including alimited number of child car seats. The loanis usually for 60 days.

Relocation Services assists familiesnew to Hawaii. Before they arrive in Hawaii,the service helps them gather informationabout their new duty location. ACS alsoprovides deployment briefings, welcomepackets, a military resource library, arrivalbriefings, and sponsorship training.

Tripler medical residents compriseabout 14% of the Army’s annual trainingpool. The Internal Medicine training pro-gram is the oldest training program atTripler and was initially accredited in Feb-ruary 1949. Tripler also supports threepsychology postdoctoral fellowship pro-grams, a clinical psychology internship, apharmacy residency, a graduate programin nursing anesthesia, and a residency inhealth care administration.

Tripler participates in combined Neonatal-Perinatal Medicine, Addiction Psychiatryand Forensic Psychiatry fellowship pro-grams sponsored by the University ofHawaii. Tripler HistoryThe origins of Tripler Army Medical

Center date back to 1907 when severalwooden structures at Fort Shafter wereused as a hospital. The facility was namedin 1920 after Brigadier General CharlesStuart Tripler, in honor of his contributionsto Army medicine during the Civil War.

At the start of World War II, the hospitalat Fort Shafter had 450 beds.

Plans for a new Tripler hospital atopMoanalua ridge were drawn in 1942 andconstruction was completed in 1948. Thearchitecturally distinctive coral pink struc-ture, nestled on the southern slopes ofOahu’s Koolau range, was dedicated onSept. 10, 1948 and has become a familiarlandmark on the island of Oahu.

Tripler Army Medical Center Today

Tripler Army Medical Center is thelargest military medical treatment facility inthe entire Pacific Basin. Located in centralOahu, Tripler is well situated to serve its pri-mary beneficiaries, the active-duty per-sonnel of the U.S. military armed forces andtheir families. Tripler is also the regionalmedical command for the U.S. Pacific Com-mand. Its geographic area of responsibilityspans more than 52 percent of the earth’ssurface, from the western coasts of theAmericas to the eastern shores of Africa.

Nearly 400,000 people in the PacificBasin are eligible to receive care at Tripler,the Pacific Regional Medical Command’spremier teaching medical center. Becoming Kama’ainaUpon arrival, you will be considered “malihini,” which is Hawaiian for “visitor” and is

applied to newcomers and tourists. Long-time residents and those born in Hawaii are called“Kama’aina,” which means “child of the land.”

It is not an insult to be called malihini, but it is a distinction that allows locals, kama’aina,to help newcomers and visitors become familiar with the lifestyle and culture of Hawaii. So,how do you become a kama’aina? Some people say when you get your first Hawaii driver’slicense, or eat kimchee with chopsticks. Start collecting aloha shirts and only wear ties andformal wear when required by duty. Understand directions, “mauka,” “makai,” “DiamondHead” and “Ewa.”

Say “shave ice” instead of snow cone. Understand basic Hawaiian pidgin such as puka,talk story and da kine. Know how to make the “shaka” hand sign. Order saimin with Spaminstead of chicken noodle soup.
