How to begin a COVID-19 Test in FormFox:
Occupational Health Services Icon
• Click on the ‘Occupational Health
Services’ icon
• Select COVID-19 testing
• Enter your CRL COVID-19
laboratory account number
• Enter the individual’s information
• Click ‘Search’
*At this time, the ‘Test to be
performed’ dropdown will appear on
the screen after you click the ‘Search’
button.

How to begin a COVID-19 Test in FormFox:
Patient Check-In
• Click on the ‘Patient Check-In’ icon
• Enter patient’s information
• Select the service to be performed
• Enter your CRL COVID-19
laboratory account number
• Select the panel from the ‘Test to
be performed’ dropdown menu
• ‘Click’ add
*The COVID-19 workflow does not
have patient fillable information at this
time.

Information Tab
The ‘Patient Information’ section and the ‘Step 1’ section will ask for the same information no matter the COVID-
19 service. Make sure all required fields are completed. Capture the patient’s signature.

It is important to enter a mobile phone
number in the ‘Patient Information’
section of the ‘Information’ tab. Having
a mobile phone number in the system
will allow the COVID-19 results to be
sent directly to the patient via a
text/voice message.
Neglecting to do so could lead to
unreported results.
IMPORTANT

Patients under 18 years old
If patient is under 18, the guardian first and last name, relation, and phone is required.

Instruction Tab: Step 2 – Obtain Supplies Needed

At the bottom of the
‘Information’ tab is Step 2. This
is where you will see the
supplies needed to complete
the collection. Each service
(DBS, Serum, and Saliva) will
show a different supplies list.
If you would like to print out the
list as a reference, click on the
‘Print List’ link.

Specimen Collection Tab: Step 3 – Perform Collection
The ‘Specimen Collection’ tab will list the specific instructions for the selected panel.
Please read the instructions thoroughly. Pay attention to the illustrations in the tab to make certain you have the correct supplies for the collection.

The Identification Key
It is CRUCIAL to write the unique identification key on the barcode. This key will help the laboratory identify
the specimen and its corresponding electronic form.

Serum Collections
• Use your FormFox Labels – Smaller labels found in
the top right-hand section.
• You will notice that three of these labels have an X,
Y, or Z character as part of the barcode. If you have
placed one of these labels on the tube, make sure
when you scan the barcode that the X, Y, or Z is
displayed as the very last character of the
specimen ID on the screen if your barcode includes
an X, Y, or Z. For example, the label on this page
will be entered as CC01605003X.
What do I write on the barcode?
• 3 letter code

Unable to Collect
If you are unable to complete the test, click on the ‘Unable to
Complete’ button found at the bottom of the ‘Specimen’ tab.
Selecting ‘Yes’ in the pop-up will cancel the event in
FormFox. If you accidentally click this button, you will need to
begin the test again. FormFox is unable to retrieve the
cancelled test.

Step 4: Register Container
Step 4 is where you will enter the barcode ID
used to identify the specimen. It is important
to have both the barcode ID and the unique
identification ID on the specimen in order to
be processed at the laboratory.
This window will display the supplies
associated with the selected test.
Remember, the blood draw/serum test is the
only COVID-19 test that uses a FormFox
label. DBS and saliva will have the barcode
ID in the kit received from the lab
After you affix the barcode ID on the
specimen container, enter the barcode ID
number into this text field. Remember to
include the X, Y, or Z character if it appears
on the label that is on the container.

Summary Tab / Step 5 - VERIFICATION
The ‘Summary’ tab is where you will
verify that the barcode value matches
the barcode ID that is on the specimen
container. If the IDs do not match, click
the ‘Edit’ link and enter the correct
barcode ID value.
Additionally, make sure that the
identification key is written on the
barcode label affixed to the specimen
container. MUST match the
specimen being sent to
the lab.

Summary Tab / Step 5 – PACKAGING AND SHIPPING
Confidential Information
The Instructions at the bottom of the
‘Summary’ tab will display the
packaging and shipping instructions for
the selected test. Please read the
instructions thoroughly.
*If you have a CRL courier that
transports other samples to CRL, you
can send the COVID-19 tests with your
CRL courier. If you don’t, please use a
CRL FedEx Air Bill.
Make sure to click the ‘Finish’ button
when the test has been completed in
FormFox.

Where is the printout?
There is no printout to send to the laboratory! The COVID-19 clinic workflow is completely
PAPERLESS! This is why it is vital to verify that the barcode ID and the identification key match
between the submitted specimen and the information entered into FormFox.

Where can I find a report?
Click the ‘Complete/Search’ button.
Click on the ‘Donor ID’ link.
Click on the ‘Report’ button.
