• Taxpayer Name • Federal Employer Identification Number (FEIN)

Social Security Number (SSN) Colorado Account Number (CAN) Income Tax Identification Number (ITIN)

• Mailing Address • Recent Letter ID

o A letter ID is listed under the perforated line on the sales tax license.
