5. Skills/jobtobelearnedintheprogram:Studentswillbetrainedasdoctoral-level
expertsinintegratedbehavioralhealthcare.Graduateswillbepreparedtoworkasbehavioralhealthconsultants,providers,andmanagersinprimaryhealthcaresettings,includinghospitals,PrimaryCareMedicalHomes,FederallyQualifiedHealthCenters,andspecialtymedicalsettings.Allstudentswillcompleteaculminatingprojectthatdemonstratesapplicationofevidence-basedpractice,qualityimprovementapproaches,and/oruseofinformaticstoinformandguidepatient-centeredcare.Thisprojectwillincludeabusinessplan,whichwillpreparegraduatestolaunchentrepreneurialeffortsaimedatleadinghealthcarereform.
6. Numbersofclockand/orcredithoursfortheprogram:Atotalof54credithours
willberequiredforgraduation.Thepracticumexperiencewillrequire400clock-hoursinaprimarycareorothermedicalsetting.Uptoanadditional12credithourswillbeallowedasneededtocompleteindependent/specialtystudyoradditionalappliedresearch.
AdmissionRequirementsTobeconsideredforadmission,applicantsmustmeetthefollowingrequirements:
1. Completionofabachelor’sdegreefromaregionallyaccreditedcollegeoruniversitywithacumulativeGPAof3.0orbetter.
2. Completionofamaster’sdegreeinabehavioralhealthprogram(Counseling,SocialWork,MarriageandFamilyTherapyorequivalentcourseworkinagraduatelevelpsychologyprogram)fromaregionallyaccreditedcollegeoruniversitywithacumulativeGPAof3.0orbetter.*
3. Licensedorlicenseeligible(e.g.intheprocessofpursuinglicensure)inabehavioralhealthdiscipline.
4. Threelettersofrecommendation.5. Serviceorientationasdemonstratedbypreviouswork,volunteerwork,orotherlife

*Individualswhohavepreviouslycompletedamaster'sdegreeinanareaotherthanonelistedabove(e.g.AlliedHealthfields)andarelicensedorlicenseeligible(e.g.intheprocessofpursuinglicensure)inaclinicaldisciplinemusthavetheirtranscriptsevaluatedtodetermineifpreviouscourseworkmeetssomeoralloftheStandardCurriculumrequirements.Theseapplicantsmaybeaskedtocompleteadditionalcourseworkinbehavioralhealthpriortoadmissionandenrollment.InternationalApplicantsTheCummingsInstitutewelcomesinternationalstudentsfromaroundtheworld.Applicantssubmittingdiplomas,degreesandcoursetranscriptsfromnon-U.S.institutionsarerequiredtohavetheiracademiccredentialsevaluated.Theevaluationwilldetermineifanapplicant’seducationmeetsadmissionrequirements,andifanytransfercreditswillbeawarded.Internationalstudentsshouldaccommodateacredentialevaluationinyourenrollmenttimeline.Admissionforinternationalstudentshasthefollowingrequirements:

EnrollmentPolicyAdmittedstudentshaveupto60daysfromthedateofadmissiontotheDBHProgramtocompletetheenrollmentagreementandregisterfortheirfirstcourse(s).Allpaymentarrangementsforthefirstcourse(s)mustbemadebeforethefirstcourse(s)begin(s).Thosestudentswantingtousefinancingoptionsmustmaketherequiredpaymentarrangementsbeforethefirstcourse(s)begin(s).Studentswhodonotcompletetheenrollmentagreementwithin60daysofadmissionareconsidered"nostarts."TransferStudentsTheCummingsInstitutewelcomestransferstudents.Thetransferofcredithasthefollowingconditions:

TuitionwillbechargedforallcoursesatthecurrentratefortheDBHprogramatthetimeofcourseregistration.TheAdmissionsteamwilladvisetheapplicantifthereareprerequisitesforthecourserequested.IfthestudentlaterappliesforadmissiontotheDBHdegreeprogram,heorshewillhavetoformallyapplytotheDBHprogramandsubmitallrequireddocumentation.Anynon-degreestudywillbeevaluatedastowhetherthecourseworkwillbeappliedtothedegreerequirements.Thestudentwillbeevaluatedasanewapplicant,andmustmeettuitionrequirements,graduationrequirements,andpoliciesineffectatthetimeofmakingapplicationtothedegreeprogram.NondiscriminationinAdmissions
TheCummingsGraduateInstituteforBehavioralHealthStudieswelcomesallstudentsanddoesnotdiscriminateonthebasisofrace,color,nationalorigin,religion,disability,gender,age,sexualorientation,orinanyotherwayinanyofitspolicies,practicesorproceduresinvolvingapplicants,students,facultymembers,staffandthepublic.Wereservetherighttorefuseadmissiontoanyonewebelievedoesnotmeetouracademicandethicalstandards.GraduationRequirementsTheDoctorofBehavioralHealthdegreerequiresaminimumof54credithoursatthedoctoral/graduatelevelbeyondthemaster’sdegree.Studentsmustcompletetherequiredcourseworkandcredithourswithagradeof“B”orbetterandconsistentweeklyattendanceineachcourse,includingpracticumandculminatingproject,within4years.
Studentsmusthavecompletedtheirrequired400hoursattheirpracticumsite,andmusthaveearnedascoreof3(outof5)orbetterineachdomainofthePracticumStudentEvaluationinordertopassthepracticumandgraduatefromtheprogram.DetailedinformationonPracticumrequirements&evaluationwillbeprovidedtostudentsuponenrollmentintheDBHProgram.Completionofaculminatingprojectincludesanapprovedliteraturereview,anapprovedculminatingprojecttopic,anapprovedfinalprojectpaper,andaCulminatingProjectDefense.AftertheCulminatingProjectDefense,thestudent’scommitteewillconveneanddeterminetheoutcomeasfollows:1.Approvedwithoutrevisions,2.Approvedwithrevisions,or3.NotApproved.StudentsmustreceivefinalapprovalontheirpaperanddefensefromtheirChairand/orCommitteepriortobeingapprovedforgraduation.

Studentsmustapplyforgraduationthesemesterpriortotheirfinalsemester,andreceiveapprovalfromtheirFacultyAdvisor,CulminatingProjectChairand/orCommittee,EnrollmentSpecialist,Registrar,andProgramDirectortoensurethatallprogramrequirementshavebeenmet.Astudentmustresolveallincompletesinordertograduate,andalltuitionandfeesmustbepaidinfull.CompletionPeriodTheCummingsInstituteallows4yearstocompletetheDoctorofBehavioralHealthprogram.Normaltimetocompletetheprogramvariesdependinguponcoursetakerateandcreditstransferredin.Studentswilltypicallyenrollin2courses(6credits)persemesterperyear,totaling18creditsperyear,andcompletingtheprogramin3years.Studentswhochooseanacceleratedtrackcantake3ormorecoursespersemesterandfinishtheprogramin18-24months.TuitionCostsandFees

AdmissionsmakestheinitialcontactwithinterestedpartiesandprospectivestudentstoexplaintheCummingsInstituteandDoctorofBehavioralHealthprogramandsystemofdelivery.Ouradmissionscounselorassistsprospectivestudentsthroughtheapplicationprocess.Enrollmentservicesprovidesfinaladmissionssupport,evaluatestransfercreditsandpriorcourseworkatotherinstitutionspriortoacceptanceattheCummingsInstitute,andhastheprimarygoaloftransitioningapplicantsintoenrolledstudents.GoDBHprovidesanorientationtotheonlinelearningplatform,whichassistsstudentswhoarenewtotheprograminansweringquestionsandnavigatingtheresourcesavailableontheonlinelearningplatform,accessingcourseshells,andcontactingfacultyandotherstudents.GoDBHwillbeavailabletoadmittedstudentsimmediatelyaftertheysignandsubmitanenrollmentagreementtoallowthemtobecomefamiliarwithprogramnavigationpriortobeginningtheirfirstcourse.StudentswillalsoreceivesupportandinstructionintheuseofGoDBHandtheonlinelearningplatformintherequiredCF-1000course.FacultyAdvisorsandAcademic/CareerGuidanceEachDBHstudentwillbematchedwithafacultymemberbasedonaGoodnessofFitModel.Studentsareinvolvedinthisprocessduringtheenrollmentprocess,astheircareerandresearchinterestsplayalargeroleindeterminingtherightFacultyAdvisormatch.FacultyAdvisorswillassiststudentsthroughouttheircourseworkincreatingaprogramofstudy,includingindependentorspecialtystudywithfacultymembersintheirfieldofinterestthatsetseachstudentupforatailored,thoroughlearningexperience.AlloftheCummingsInstitutefacultywillplayanactiveroleinadvisingDBHstudentsinboththeiracademicprogressaswellastheirpost-graduatecareers;however,eachstudentwillbeassignedformallytoaFacultyAdvisorwhowillberesponsibleforthefollowingroles:
• AssistingstudentswiththepoliciesandpracticesoftheCummingsInstitute.• Addressingquestionsorconcernsregardingcourserequirementsandexpectations,
performancecriteria,academicstanding,andprofessionalism.• Providingfeedbackonstudentprogressincourseand/orclinicalrequirements,
facultyexpectations,graduatecompetenciesandprogramgoals.• Providingsupportforstudentpersonalandprofessionalgrowth.Thissupportmay
includereferralstointernalorexternalsourcesasneeded.• Discussingacademicperformanceinanefforttooptimizelearnersuccess.• Assistingstudentsdeemedtobeat-riskbyprovidingguidanceandsupport.
Studentswillmeetwiththeirfacultyadvisorasneededinthefirstyeartopromoteprofessionaldevelopmentandself-reflection,addressacademicperformance,receivesupport,andpromotethefacultyadvisor-adviseerelationship.Itisthestudent’sresponsibilitytocontacttheiradvisorwhenissuesneedtobediscussedatanytime.AstudentmaychangeadvisorsuponrequesttotheAssociateDirector.Shouldanotheradvisorbeavailable,thestudentmaybereassignedtopromoteamoreproductivementoringexperience.

StudentswhorequireanALOA,andwhoarenoteligibleforamilitaryleaveofabsence,mustcompleteawrittenRequestforLeaveofAbsenceviaemailtotheDirector.Notallrequestswillbeapproved.Additionaldocumentationmayberequired,andallLeaveofAbsencedecisionsarefinal.WhenaLeaverequestisapproved,theDirectornotifiesthestudent,faculty,Registrar,andOfficeofFinancialAssistance.Atthattime,theDirectorspecifiesthelengthoftimeapprovedfortheALOA,andtheapprovedALOAreturndate.CoursesinProgressAstudentwhoisinanactivecourseatthetimeheorsheisgrantedanacademicleaveofabsence(ALOA)receivesa“WL”gradeprovidedtherequestismadeonorbeforethelastdaytowithdrawfromthecourse.Otherwise,alettergradebasedoncourseworkcompletedtodatewillbegiven.Whencompleted,the“WL”willbereplacedbythefinalacademicperformancegrade.LengthofLeaveThepermissiblelengthofeachALOAisevaluatedonacase-by-casebasisbytheDirector.CalendardaysareusedtocalculatethelengthoftimeastudentspendsonALOA,andthemaximumnumberofcalendardayspermissibleinanycaseis90perALOA.TheallowedlengthoftheALOAwillbedeterminedduringtheapprovalprocessbytheDirectorbasedonthecircumstancesofeachleaverequested.ThedatespecifiedasthebeginningdateoftheAcademicLeaveofAbsenceisthedatethestudentsubmitstherequestforleave.However,thetotalamountoftimeonleavecoversthestudent'stotaltimeoutofattendance.Thereforethetotalamountoftimeonleaveiscalculatedfromthedayfollowingthestudent'slastdateofattendance(LDA).RegardlessofwhethertheALOAbeginswhileastudentisinacourseorinbetweencourses,themaximumallowabletimeoutofacourseisonesemester.ThestudentmustreturnfromtheALOAonastartdatesuchthatthesemesterleavelimitisnotexceeded.Back-to-backrequeststhatcomprisemorethantwosemestersofleavetotalarenotpermitted.ThereturnfromtheALOAmustcoincidewithavalidcoursestartdate.TimespentonALOAcountstowardprogramlengthandisincludedwhendeterminingifastudentcancompletehisorherdegreeprogramwithinthemaximumtimelimits.ThetotalnumberofALOAsapprovedforstudentsintheDBHProgramisevaluatedonacase-by-casebasis.Noleavesarepermittedforstudentsinnon-degreeprograms.AccesstoCummingsInstituteResourcesduringLeaveWhendoctoralcandidatesarenotactivelyenrolledinacourse,theyhavelimitedcommunicationwithsupportstaffandmaynotengageincertainactivitiesrelatedtotheirdegreeprogress.Specifically,doctoralcandidatesmustbeactivelyenrolledinacoursetohavecontactwiththeirfaculty.Doctoralcandidatesmustalsobeactivelyenrolledina

Underthesecircumstances,studentsmayreapplytoresumetheDBHprogramwithoutadjustmentbeingmadetothecoursesrequiredtocompletetheprogram.SAPdoesnotrestart.Militarystudentsreturningtotheinstitutionfrommilitaryservice-relatedtimeawaywillbereviewedonacase-by-casebasistodetermineifacomparisonisneeded.Inmanycases,thesestudentsmayre-enterintotheirpreviousprogramversion.DefinitionofSubstantialChangeAsubstantialchangeisachangetotheDBHprogramthatisfundamentallydistinctfromtheDBHprogramatthetimeinwhichthestudentwaspreviouslyenrolledattheCummingsInstitute.TransferofCreditstoReentryProgramTheCummingsInstitutedetermineswhichpreviouslycompletedcoursesaregivencreditinthestudent’sre-entryprogramandcreatesanewdegreeplanaccordingly.ReadmissionafterWithdrawalforNon-PaymentAllpriorfinancialobligationsmustbepaidbeforeawithdrawnstudentmayresumeattendanceattheinstitution.Itisthereturningstudent’sresponsibilitytoworkwiththeAccountingOfficedirectlyregardinganypreviousfinancialobligationtotheinstitution.Studentprobation,suspensionorexpulsionAsnotedintheSatisfactoryAcademicProgress(SAP)trackingpolicy,studentsmaybeplacedonAcademicProbationorExtendedProbation,ormaybeAcademicallyDismissedduetoUnsatisfactorySAPEvaluationStatuses.AstudentmayalsobeadministrativelydismissedduetoviolationoftheCodeofConductpolicy.CodeofConductTheCummingsGraduateInstituteforBehavioralHealthStudieshasestablishedthefollowingCodeofConductforstudents.EachstudentisexpectedtounderstandthetermsandconditionssetforthinthisPolicy.TheCummingsInstituteiscommittedtomaintainingacommunitywithexceptionalethicalstandardsofprofessionalandacademicconduct.Substantiatedviolationsmayresultindisciplinarysanctions,uptoandincludingexpulsionfromtheinstitution.Doctoralcandidatesareexpectedtoconductthemselvesprofessionally,andrefrainfromactsofmisconductincludingbutnotlimitedtothefollowingsixcategories:• Dishonesty,cheating,plagiarism,misrepresentationorfurnishingfalseinformation,

StudentGrievanceProcedures
