    It is your responsibility to use your administrative account to create new users for authorized individuals who use the system and deactivate the users when they are no longer employed or in a role where they no longer need access.

  • Patient does not have insurance and has not signed forms

    Action :

    ● add insurance manually ● Send code to patient for them to enter all their

    information on their phone or computer

    In Patient Profile : Patient’s insurance information is

    complete but they have not signed the agreements yet.

    Action :

    ● Either, send code to patient for them to enter their own information

    ● Or, wait till check-in on day of testing to sign all documents

    At check-in : patient has not yet signed agreements and must do so

    before a test can be assigned to them

    Action :

    Turn computer enabling patient to be able to sign documents

    Send access code for them to sign via their phone or

    computer

  • If code is generated

  • You may also give this code to your staff

    Admin can write this unique code and give to their staff to be able to input this code on a computer and enter their information.

  • Surveillance Testing Appointment (existing patients)

  • Once a patient is in our system, you can make

    them appointments without the walkup link. To

    do so :

    1. Click on New Appointment

    2. Under Patient Information

    a. Click on Use Existing

    b. Begin typing the patient’s name into

    the field to find the existing patient

    Remember: If you do not use the existing patient

    option, you will create duplicate patient profiles in

    your portal

  • Checking in Patients

  • CHECKING IN SOMEONE WITH AN APPOINTMENT

  • Please select the correct sample collection your facility is performing for the given test

  • Scheduled → Checked In

    Once you have scanned in your barcode, you can click Return to Appointment List to move to the next patient. If you manually enter the barcode, please ensure accuracy and Click Confirm and Check In after.

  • Last step

    For each patient : write the appointment number on the biohazard bag in sharpie in large writing

  • If you need to replace a patient’s test kit

  • Replacing a Damaged Kit

    If you need to replace a kit due to contamination or incorrect patient, please Click Replace Damaged Test Kit and Scan in the new barcode.

    Please note: once you have replace a barcode, that kit/barcode is considered dead and cannot be used again.

  • To avoid duplicate patient profiles

    Reminder

    Once a patient is in our system, you can make them appointments without the walkup link. To do so :

    1. Click on New Appointment 2. Under Patient Information

    a. Click on Use Existingb. Begin typing the patient’s name into the field to

    find the existing patient

  • Results

  • Patient Results

    Patients will receive their results via email or text - please make sure the email inputted into the system is correct

  • Shipping

  • Curative COVID-19 test kit containing a patient sample should be stored at temperatures less than

    104°F so as not to damage the samples.

    Please store samples awaiting return shipment in a temperature controlled environment.

    They do not need to be refrigerated.

    Samples should be shipped back to the lab no longer than 24 hours from collection.

    Samples are only valid for 4 days.

  • Preparing your return box

    Write the following on the outside of the box:

    1. Your site name (e.g. Curative Long Term Care)2. Number of test kits in the box

    When everything is filled out click on: Process Shipment

    If you do not want to receive the label by email remove email from this dropdown.

    The labels will be produced once you click on process shipment.

    Thursday and Friday shipments : please click on Saturday delivery in order to assure their arrival at our lab.

  • Schedule a pick up if desired

    Please schedule your UPS pick up at least 2 hours before you would like it picked up

    Please remember, overnight delivery does have a cut off time. You must meet that cut off time or the package will not ship until the next day

    Requests for additional kits are managed by the State of Pennsylvania n coordination with Curative.

    Please do not reach out to Curative Customer Support regarding additional kits. Instead, additional kits can be requested by completing this form which is managed by the State of Pennsylvania .

    Please note: You do not need to submit a request for your initial shipment from Curative. The State has

    estimated the number of kits you will initially need which will arrive automatically.
