Detailed scope of work

A-Patients careSince I had been graduated from the faculty of medicine in 1991I spent one year as a House officer (Internship) at Al-Hussein University Hospital, Cairo- Egypt. During that year I spent 2 months course in each of the following departments: general surgery, General medicine, Obestetrics and Gynaecology, Pediatric medicice, Cardiology, and Ophthalmology.

Residency Program:In March 1993 I started my residency program in the Ophthalmology Department, Al-Hussein University Hospital, Cairo- Egypt, my Residency program ended in August 1996. During my residency I acquired the following skills:

Basic clinical and surgical skills as a general Ophthalmologist. Routine ophthalmic outpatient clinic work, Anterior segment surgery (including cataract extraction, intra-ocular lenses

implantation, glaucoma surgery, and strabismus surgery as well) Trained on Yag laser, Argon laser, Doing Basic Flourescein angiography, basic Ulrasonography. Assisting in a lot of conventional retinal detachment surgeries and vitrectomies

both for diabetic patients and complicated retinal detachments. I did only a few cases of conventional retinal re-attachment surgery

The rest of my training and surgical skills had been acquired during the rest of my career in the university hospitals later on between August 1996 and April 2002, where I acquired the following skills:

Excellent Experience in Glaucoma Surgery including both conventional and Shunt operations (Ahmad Glaucoma Valve, Baerveldt Glaucoma Implant).

Excellent experience in Pediatric ophthalmology and strabismus surgery including horizontal and Inferior oblique muscle surgery, and Ptosis surgery as well.

Doing small incision cataract surgery (SICS) and Phaco-Emulsification with foldable IOL implantation.

Doing laser surgery both for the anterior segment (as Argon laser trabeculoplasy(ALT), Selective laser Trabeculoplasty (SLT), YAG Capsulotomy and Iridotomy) and for the posterior segment (using Argon laser, Diode Laser).

Doing refractive surgery including PRK, LASIK and Phakic anterior chamber intraocular lenses for correction of high degrees of myopia, in addition to keratoplasty (Lasik and PRK skills were acquired in the private sector)

During my fellowship in the glaucoma unit at the University of Amsterdam , Between May 1999 and September 2001:

Routine glaucoma service (Diagnosis, treatment and surgical management both conventional and shunt surgery)

I acquired the experience to do, understand and interpret Optical coherence tomography (For Retinal nerve fiber layer assessment), Heidelberg Retinal Tomography and Flowmetry (for optic nerve head assessment). Also I am knowledgeable in visual field both, white/white, Blue/Yellow and Frequency Doubling Technology perimetry (FDT).

Senior Consultant and head of Ophthalmology department in Rustaq Hospital, Sultanate of Oman From 12/01/2007 till 29 th august 2011:

Al-Rustaq Hospital is a 2ry level hospital in South Batinah region for different specialties; in addition to Ophthalmology it provides General surgery, general medicine, cardiology, Obstetrics and Gynecology, Pediatrics, ENT, and Dental care for patients attending the hospital. It contains 250 with 7 beds allocated to the ophthalmic department (3 males, 3 females and one Pediatric bed). There are 4 theaters for different kind of operations one of them is allocated for ophthalmology for 2 days shared with the clean orthopedics cases in other days.

As a head of department and most senior of ophthalmic staff within the department my

work is divided into these categories:

A-Patients care

B-Educational and teaching activity

C-Administrative work

A-Patients care: OPD patient care: I usually have routine regular 3 OPD clinics per week in

addition to 2 irregular clinics after the OT days. In the OPD; my work is divided

between seeing patient with appointment with me either as follow up patients,

especially postoperative patients or referred from other staff members in the

departments. In the OPD I do routine clinical ophthalmic work of seeing the

patients and doing Ultasonography and fundus fluoresein angiography. Anterior

segment patients specifically Glaucoma patients are mainly followed up by me,

and sometimes shred by other specialist. In addition to seeing my patients I

always see difficult and complicated cases seen by other staff members and either

give opinion and instructions or completely take the responsibility and follow the

case up by myself. Glaucoma cases seen by other staff members are usually

referred to me at the beginning of diagnosis and at least twice a year for

controlled patients. Posterior segment cases are usually followed up by another

senior specialist since 2 years but we usually share the opinion in difficult cases.

Most of patients attending the OPD are either cataract patients, patients with

corneal opacities and ectazia, diabetic retinopathy, strabismus, pterygium etc. Any

case to be referred to another tertiary ophthalmic center either for intervention or

2nd opinion are always seen by me before sending the patient to make sure that no

case will be sent unnecessarily especially if the care can be provided in our unit.

Part of my regular OPD work is to see all the patients to be admitted for surgical

intervention for proper assessment and preparation. Routinely all post operative

patients are seen in the OPD on the 1st post op. day as there is no slit lamp

available in the surgical word.

Operating room work: In the OT I usually do routine surgery mostly under local

anesthesia which I give myself. I do the following types of surgeries;

Phacoemulsification and IOL implantation, small incision cataract surgery,

routine extra capsular cataract surgery and IOL implantation, occasionally I do

some intracapsular cataract surgery. In addition I do strabismus surgery,

pterygium surgery, Ptosis surgery (few cases) and trabeculectomy and combined

surgery as well. In addition to doing surgeries myself I do assist other staff

especially junior staff in surgeries as part of surgical skills transfer program. Also

I assist the senior staff in any new technique especially phaco. Additionally I

assist any staff for other surgeries which he is not exposed to it before after

assisting me in few cases (strabismus surgery). Beside this surgical work I keep

instructing the staff nurses about proper care and handling of ophthalmic

microsurgical instruments and dealing with ophthalmic equipments as well. Also I

help in orientation and training of new staff nurses as well.

On call duties: I usually do 2nd or 3rd on call duty according to the availability of

enough staff in the department. All serious emergency cases (hyphaema, infective

ulcers, penetrating eye injuries and acute glaucomas) should be notified to me

either to attend surgery or to attend the case or review the treatment before

admission or sending home. We have protocols for these cases. Seriously

damaged penetrating eye injuries I usually attend and do the surgery myself, or

see the case and be around in case if the attending surgeon needs any help.

Others: in addition to clinical and surgical work I regularly and randomly check

the patient files for those seen by other staff member, revise them and see if any

defects are there? In case of any defects I always counsel the treating doctor after

revising the file with him and always ask everyone to follow the standard

protocols that we have for seeing patients, ordering investigations and prescribing

medications. (eg: we have adapted form of the protocol of the (WHO regional

office for South East Asian region for management of corneal ulcer) and the

European Glaucoma Society Guidelines for glaucoma management.

Auditing: Routinely there is a monthly audit for surgical cases in addition to the

yearly auditing report.

4-Teaching and instructing the Optometrists (I had trained 4 Optometrists for visual field

testing)

C-Administrative workBeing a head of department; I am responsible for

Regulating the work within the department

Regulating the work between the department and hospital administration.

Regulating supervising the work both in Rustaq hospital ophthalmic department

and Rustaq polyclinic.

Planning and follow up of the department needs and requests for medications and

equipments in collaboration with the pharmaceutical and surgical store.

Planning and doing the duty roster for the staff members

Planning the annual leaves for different staff to keep smooth running work in the

department especially during leaves.

Cooperation with the administration in applying the ministry’s policies and

regulations.

Supervising the patients’ files to ensure its completeness.

Educational and teaching activity

A-Teaching experiences in Egypt:

As a Faculty member of the Ophthalmic Department at Al-Azhar University, Faculty of Medicine, Cairo-Egypt, I had been involved as part of my job in teaching both, Under-graduate and Post-Graduate Students in the following manner:

1. From 2002 April till December 2006 : I had been involved in teaching both undergraduate students and postgraduate students attending the Diploma, Master and Doctorate Courses in the ophthalmic Department, Faculty of Medicine, Al-Azhar University, Cairo, Egypt. Undergraduate students (4th Year Students): giving oral theoretical

lectures, clinical rounds, examination of students at the end of the round and at the end of the year (MCQ, Clinical, and Oral Exams). Also I was participating in correcting written final exams.

Postgraduate students (Diploma, Master and Doctorate): Giving oral lectures, Clinical Rounds, surgical skill transfer, and participating in Clinical Exams only). I was generally one of the anterior segment team, and responsible for the Glaucoma course (Medical, Investigations and Surgery) in conjunction with others (VF course, and Imaging in Glaucoma was solely given by me).

Teaching Ophthalmic Residents and assistant lecturers and clinical demonstrators. Both (Theoretical, Clinical, surgical).

2. From August 1996 till April 2002. As an assistant lecturer or clinical demonstrator I had been involved in teaching both undergraduate students and postgraduate students attending the Diploma, Master) Undergraduate students (4th Year Students): giving oral theoretical tutorial

lectures, clinical rounds, and preparation of cases for exam. Postgraduate students (Diploma, Master): Giving oral theoretical tutorial

lectures, Clinical Rounds. Teaching Ophthalmic Residents and assistant lecturers and clinical

demonstrators. Both (Theoretical, Clinical, surgical Skills transfer). 3. From April 1993 till August 1996, During My residency in the ophthalmic

Department, Al-Hussein University Hospital, Al-Azhar University. I was participating in the teaching process only for preparation of cases for clinical rounds and occasionally demonstrating physical signs for undergraduate students.

4. From May 1999 till October 2001, During My Fellowship in the Academic Medical Center (AMC) University of Amsterdam, the Netherlands. I was occasionally involved in the clinical teaching process for undergraduate medical students and occasionally give lectures in the Bimonthly Departmental evening meeting (Thursday meeting). Also this meeting was held twice a year in conjunction with the Vreij Universiteit Amsterdam (the VU) and I represented the AMC as a speaker once in 2.5 years.

B-Teaching Experience in Oman:

Since I joined the MOH on the 12th of January 2006 I had been involved and supervised the following educational and teaching activities:

1. Regular rounds both pre and post operative rounds in the ophthalmic Department

2. Regular bimonthly intradepartmental continuous medical education meeting and presentation about the new and important topics in the ophthalmic field.

3. I participate in Sunday CME education activity which is a bimonthly general meeting for the hospital staff of Rustaq hospital. I gave 3 lectures: 1) Ocular involvement in Rheumatic Disorders. 2) Diabetes and the eye. 3) Fundus examination and systemic disorders.

4. Teaching of Junior staff in the Ophthalmic department. 5. Teaching the Omani Interns as part of the surgery course.6. Teaching the Ophthalmology course for Undergraduate student of

Oman Medical College (5th and 6th year students) 7. Continuous medical education and orientation of GPs . Usually there

are 2 courses (presentations one Day) and one or two days clinical attachment in the ophthalmic OPD. I usually take responsibility for 2 topics, Glaucoma and the eye in diabetic patients.
